Abstract Background: Sex hormones may be associated with a higher incidence of ischemic stroke or stroke-related events. In observational studies, lower testosterone concentrations are associated with infirmity, vascular disease, and adverse cardiovascular risk factors. Currently, female sexual hormones are considered neuroprotective agents. The purpose of this study was to assess the role of sex hormones and the ratio of estradiol/testosterone (E/T) in patients with acute ischemic stroke (AIS). Methods: Between January 2011 and December 2016, 146 male patients with AIS and 152 age- and sex-matched control subjects were included in this study. Sex hormones, including estradiol, progesterone, and testosterone, were evaluated in the AIS patient and control groups. We analyzed the clinical and physiological levels of sex hormones and hormone ratios in these patients.Results: The E/T ratio was significantly elevated among patients in the stroke group compared to those in the control group (P = 0.001). Categorization of data into tertiles revealed that patients with the highest E/T ratio were more likely to have AIS [odds ratio (OR) 3.084; 95% Confidence interval (CI): 1.616-5.886; P < 0.001) compared with those in the first tertile. The E/T ratio was also an independent unfavorable outcome predictor with an adjusted OR of 1.167 (95% CI: 1.053-1.294; P = 0.003).Conclusions: These findings support the hypothesis that increased estradiol and reduced testosterone levels are associated with AIS in men.

    THE EFFECT OF HOLY QUR’AN HEARING ON CLINICAL OUTCOMES IN ACUTE ISCHEMIC STROKEABSTRACTIntroduction: Qur’an recitation has a high spiritual and religious value and effect to Islam believer, and also has beautiful and regular tone, so that it is believed to be useful to cure various diseases. Depression and anxiety are common, and add to worsening outcomes of stroke. There was spiritual correlation between decreased emotional distress and depression associated with poor outcome of stroke. Qur’an recitation could be used as spiritual relaxation technique, thus improve the effect of mental health.Aims: To determine effect of Qur’an recitation audio on anxiety, depression and National Institute of Health Stroke Scale (NIHSS) in acute ischemic stroke in Medical Faculty of Diponegoro University/dr. Kariadi Hospital, Semarang, Central Java Indonesia.Method: This was an intended to treat research with randomized pretest-posttest with control group design. Subjects were acute ischemic stroke patients with onset less than 72 hours, divided into treatment and control groups. Treatment was carried out for 7 days by playing Juz ‘Amma Qur’an recitation through headphone twice a day for 30 minutes at 06.00 am and 06.00 pm. Demographic data, risk factors, and Hospital Anxiety and Depression Scale (HADS) and NIHSS before and after treatment were assessed. The correlation of HADS and NIHSS between treatment and control were analyzed with Chi-square/Fisher test. Multivariate analysis was used to analyzed other factors affecting clinical outcomes.Result: Forty subjects were randomized into treatment or control groups. There was significant correlation between listening Juz ‘Amma Qur’an recitation with improvement of anxiety HADS and NIHSS. There was no correlation with improvement of depression HADS. Logistic regression analysis showed improvement of NIHSS after listening Juz ‘Amma Qur’an recitation.Discussion: Listening to the Qur’an recitation improved anxiety symptoms, improved clinical outcomes by 9.025 times compared to controls in acute ischemic stroke, thus can be considered to support the recovery of stroke patients.Keywords: Acute ischemic stroke, HADS, NIHSS, Qur’an recitationABSTRAKPendahuluan: Bacaan Al-Qur’an memiliki nilai spiritualitas dan religiositas yang tinggi serta memberikan efek ketenangan selain nadanya indah dan teratur, sehingga dapat digunakan untuk penyembuhan berbagai penyakit. Bacaan Al-Qur’an dapat digunakan sebagai teknik relaksasi spiritual sehingga didapatkan efek meningkatkan kesehatan mental.Tujuan: Untuk mengetahui pengaruh murottal Al-Qur’an yang diperdengarkan pada pasien stroke iskemik akut terhadap luaran klinis neurologis menggunakan skor National Institute of Health Stroke Scale (NIHSS), gangguan depresi dan ansietas di Fakultas Kedokteran Universitas Diponegoro/RSUP dr. Kariadi, Semarang, Jawa Tengah, Indonesia.Metode: Penelitian ini merupakan penelitian perlakuan randomized pretest-posttest control group design. Subjek adalah pasien stroke iskemik akut awitan kurang 72 jam, dibagi menjadi kelompok perlakuan dan kontrol. Perlakuan dilaksanakan 7 hari dengan memperdengarkan murottal Al-Qur’an Juz ‘Amma melalui headphone sehari 2 kali, durasi30 menit pukul 06.00 dan 16.00. Data yang diambil meliputi data demografi, faktor risiko stroke serta penilaian Hospital Anxiety and Depression Scale (HADS), dan NIHSS sebelum dan setelah perlakuan. Pengaruh perlakuan terhadap HADS, NIHSS menggunakan uji Chi-square/Fisher. Faktor-faktor lain yang berpengaruh pada luaran klinis juga dianalisis secara multivariat.Hasil: Sebanyak 40 subjek dilakukan random alokasi sebagai kelompok perlakuan berjumlah 20 subjek dan kelompok kontrol berjumlah 20 subjek. Terdapat hubungan bermakna antara memperdengarkan murottal Al-Qur’an dengan perbaikan HADS ansietas, dan perbaikan NIHSS. Tidak terdapat hubungan antara memperdengarkan murottal Al-Qur’an dan perbaikan HADS depresi. Analisis logistik regresi menunjukkan, memperdengarkan murottal Al-Qur’an berpengaruh pada perbaikan NIHSS pasien stroke iskemik akut.Diskusi: Memperdengarkan murottal Al-Qur’an memperbaiki gejala ansietas, memperbaiki luaran klinis sebesar 9,025 kali dibanding kontrol pada stroke iskemik akut sehingga dapat dipertimbangkan untuk menunjang pemulihan pasien stroke.Kata kunci: Murottal Al-Qur’an, NIHSS, skor HADS, stroke iskemik akut

AIM. To evaluate the effectiveness of antibiotic prophylaxis in closure of preventive intestinal stomas. MATERIALS AND METHODS. In a main (I-first) group included 99 patients who did not undergo antibiotic prophylaxis (ABP). In the control (II-second) group were 96 patients who underwent ABP by a single intravenous injection of 1,2 g of amoxycillin / clavulanic acid in 30 minutes before the operation. RESULTS. The overall incidence of complications in the main and control groups was 23 (23,3 %) and 27 (27,9 %), respectively (p=0,53). Among them, the surgical site infection (SSI) was the first place in the frequency of complications: in group I in 12 (12,2 %) in group II in 11 (11,4 %) patients (p=0,35 ). Postoperative diarrhea with frequency of stools 5 or more times per day in second place infrequency, but has evolved into two less in the main than in the control group - in 4 (4,0 %), and 8 (8.3 %) patients, respectively (p=0,212). Febrile fever was detected more frequently in the first group than in the second - 6 (6,1 %) and 1 (1,0 %) cases, respectively (p=0,06). Other complications were the paresis of gastrointestinal tract (p=0,76), bleeding from the formed anastomosis area (p=0,149), pneumothorax (p=0,324), cystitis (p=0,309) and lobar pneumonia (p=0,309). There were no statistically significant complications associated with one of two groups. CONCLUSION. The results of the study not prove the effectiveness of the ABP in patients undergoing closure preventive intestinal stoma. This does not allow us to recommend its routine use. The question of prophylactic antibiotics should be taken individually, considering all the possible risks.

Elevated fibrinogen levels is related to the blood hyperviscosity, that causes low blood velocity. Non-contrast-enhanced transcranialDoppler (TCD) is used to evaluate blood flow from the cerebrovascular system. To know the relationship between fibrinogen levels andexamination of TCD in acute ischemic stroke through evaluation. A cross sectional study was admitted from July 2012-Juny 2013.The researchers determined the differences between fibrinogen and TCD in the stroke group and control. The researchers examinedthe relationship between fibrinogen and TCD examination in the stroke group. The fibrinogenwas measured with Clauss method. TheTCD was examined due to middle of the cerebral artery (MCA) and the Internal Carotid one Artery (ICA). The patients were diagnosedas ischemic stroke from head CT-scan. Statistical analyses employed the Independent T test, Anova test and Pearson correlation. Theresearchers had 24 patients and 24 controls that the Fibrinogen levels in stroke group is 549.16±104.84 mg/mL and in the control groupis 385.64±16.80 mg/mL. The researchers examined MCA in the stroke as well as the control and found the mean velocity 43.12±21.03and 56.97±6.22 (p=0.05), the peak velocity 74.17±32.58 and 94.55±14.11 (p=0.05) end diastolic velocity 23.27±12.66 and35.30±7.34 (p=0.00). In ICA, the stroke group and control and found the mean velocity 31.40±8.86 and 43.07±8.06 (p=0.00), thepeak velocity 54.99±11.50 and 75.04±16.04 (p=0.00) end diastolic velocity 18.23±7.67 and 25.64±5.24 (p=0.00). The correlationbetween fibrinogen and TCD in the stroke group was not significant on MCA and ICA, P>0.05. It can be concluded that the differencesbetween fibrinogen levels and TCD in the stroke group and control are significant. But there is no correlation between the fibrinogenand TCD in the stroke group.

Objective: This study evaluated the efficacy of pentoxifylline, a hemorrheologic agent, for treating acute ischemic stroke in patients with bloodhyperviscosity.Methods: Patients were randomly allocated within 3 days of stroke onset to study or control treatment. All the patients received the standard acuteischemic stroke treatment. The study treatment was intravenous pentoxifylline 1200 mg/day for 5 consecutive days and oral pentoxifylline 800 mgin two divided doses for 23 days. Blood viscosity was evaluated on days 7 and 30. The outcomes were assessed by the National Institutes of HealthStroke Scale (NIHSS), modified Rankin Scale, and Barthel index (BI) criteria.Results: The median baseline viscosity was 6.89 poise in the study and 6.46 poise in the control groups and had decreased in all the patients ondays 7 and 30. The decrease in the study group on day 7 (1.13 poise) was greater than that in the control group (0.57 poise); the difference was notsignificant. The NIHSS score improved by 32% in the study group (relative risk [RR]=1.00, 95% confidence interval [CI]: 0.421–3.556; p=1.00). At1-month, 67% of the study group had a good functional outcome (RR=1.026, 95% CI: 0.656–1.605; p=0.909).Conclusion: Pentoxifylline decreased blood viscosity and improved clinical outcome in this patient series.

Parkinson’s disease (PD) is one of the most common neurodegenerative disorders in neurology. It is possible that multifactorial and genetic factors are related to its pathogenesis. Recently, there have been reports of SLC6A3 genetic variants leading to PD. However, the role of 3′ end of SLC6A3 in PD is less studied in different ethnic groups. To explore the roles of 3′ end of SLC6A3 in PD development, 17 SNP sites in 3′ end of SLC6A3 were analyzed in 360 PD patients and 392 normal controls of Han population residing in northwest of China. The significant difference of gene type and allele frequencies between the PD and control groups was detected only in rs40184 (P = 0.013 and 0.004, respectively; odds ratio 2.529, 95% confidence interval 1.325–4.827). The genotype and allele frequencies of the other 16 SNP sites were not found to be different between the PD group and the control group. rs2550936, rs3776510, and rs429699 were selected to construct the haplotypes; no significant difference was found in a frequency of 5 haplotypes between the PD group and the control group. These results suggest that the SLC6A3 variant in rs40184 A allele may increase the risk of PD in northwest Han population and may be a biomarker of PD.

Background. The aim of this study was to investigate the association between iron deficiency anemia andH. pyloriin patients with normal gastrointestinal tract endoscopy results.Materials and Methods. A total of 117 male patients with normal gastrointestinal tract endoscopy results were included in this retrospective study. The study and control groups included 69 and 48 patients with and without iron deficiency anemia, respectively. The prevalence ofH. pylori, the number of RBCs, and the levels of HGB, HTC, MCV, iron, and ferritin were calculated and compared.Results. There was no statistically significant difference found between the groups according to the prevalence ofH. pylori(65.2% versus 64.6%,P=0.896). Additionally, the levels of RBCs, HGB, HTC, MCV, iron, and ferritin in the patients in the study group were lower than those in the control group (P<0.05). Finally, there was no association between iron deficiency anemia andH. pylori(OR 1.02, Cl 95% 0.47–2.22, andP=0.943).Conclusion.H. pyloriis not associated with iron deficiency anemia in male patients with normal gastrointestinal tract endoscopy results.
