Abstract Background: Autistic people and those with high autistic traits are at high risk of experiencing suicidality. Yet, there are no suicidality assessment tools developed or validated for these groups.Methods: A widely used and validated suicidality assessment tool developed for the general population (SBQ-R), was adapted using feedback from autistic adults. The adapted tool was refined through 9 interviews, and an online survey with 251 autistic adults, to establish clarity and relevance of the items. Subsequently, 308 autistic, 113 possibly autistic, and 268 non-autistic adults completed the adapted tool online, alongside self-report measures of autistic traits (AQ), camouflaging autistic traits (CAT-Q), depression (PHQ-9), anxiety (ASA-A), thwarted belongingness and perceived burdensomeness (INQ-15), lifetime non-suicidal self-injury, and the original version of the suicidality assessment tool (SBQ-R). Analyses explored the appropriateness and measurement properties of the adapted tool between the groups.Results: There was evidence in support of content validity, structural validity, internal consistency, convergent and divergent validity, test retest validity, sensitivity and specificity (for distinguishing those with or without lifetime experience of suicide attempt), and hypothesis testing of the adapted tool (SBQ-ASC) in each group. The structure of the SBQ-ASC was equivalent between autistic and possibly autistic adults, regardless of gender, or use of visual aids to help quantify abstract rating scales.Limitations: The samples involved in the development and validation of the adapted tool were largely female, and largely diagnosed as autistic in adulthood, which is not representative of the wider autistic population. The SBQ-ASC has been developed for use in research and is not recommended to assess risk of future suicide attempts and/or self-harm.Conclusions: The SBQ-ASC is a brief self-report suicidality assessment tool, developed and validated with and for autistic adults, without co-occurring intellectual disability. The SBQ-ASC is appropriate for use in research to identify suicidal thoughts and behaviours in autistic and possibly autistic people, and model associations with risk and protective factors.

Abstract BackgroundDepression can be hard to accurately identify in autistic adults due to overlapping characteristics of autism and depression, and depression tools developed for the general population not including unique signs of depression experienced by autistic people. MethodsThree focus groups and 15 cognitive interviews with autistic adults identified response difficulties and missing autism specific items in a widely used depression assessment tool developed for the general population (PHQ-9). Feedback informed new items in the Autistic Depression Assessment Tool (ADAT-A). A further 9 cognitive interviews and two large online surveys with autistic adults refined the ADAT-A items. Subsequently, 236 autistic adults (87 male, 113 female, 33 non-binary, 18-61 years) completed the ADAT-A online, alongside self-report measures of camouflaging autistic traits (CAT-Q), Intolerance of Uncertainty (IUS-12), Suicidality (SBQ-ASC), Defeat and Entrapment (DES). Analyses explored structural validity, internal consistency, convergent and divergent validity of the ADAT-A in a community sample of autistic adults.ResultsExploratory factor analysis of the ADAT-A showed evidence in support of a three-factor solution, capturing cognitive-affective and somatic depression symptoms, and autistic specific depression symptoms. Internal consistency of each subscale and total scores were excellent (.87 - .94). The ADAT-A was significantly correlated with related constructs including self-reported suicidality, defeat and entrapment (rs>.49). The ADAT-A total score and subscales were significantly more strongly correlated with hypothesised proximally related compared to distally related constructs.LimitationsThe samples involved in the development and validation of the ADAT-A were largely female, and largely diagnosed as autistic in adulthood, which is not representative of the wider autistic population. The ADAT-A has initially been developed and validated for research purposes, and has not been validated for use in clinical contexts to screen for possible depression diagnosis.ConclusionsThe ADAT-A is a self-report autism specific depression assessment tool, developed and validated with and for autistic adults, without co-occurring intellectual disability. There is promising initial evidence in support of the measurement properties of this tool for use in research. Future research must explore whether the ADAT-A is useful in better identifying depression in autistic people in clinical settings, compared to other tools developed for the general population.

Camouflaging is a frequently reported behaviour in autistic people, which entails the use of strategies to compensate for and mask autistic traits in social situations. Camouflaging is associated with poor mental health in autistic people. This study examined the manifestation of camouflaging in a non-autistic sample, examining the relationship between autistic traits, camouflaging, and mental health. In addition, the role of executive functions as a mechanism underpinning camouflaging was explored. Sixty-three non-autistic adults completed standardised self-report questionnaires which measured: autistic traits, mental health symptoms, and camouflaging behaviours. In addition, a subset (n=51) completed three tests of executive function measuring inhibition, working memory, and set-shifting. Multiple linear regression models were used to analyse data. Results indicated that autistic traits are not associated with mental health symptoms when controlling for camouflaging, and camouflaging predicted increased mental health symptoms. Camouflaging did not correlate with any measure of executive function. These findings have implications for understanding the relationship between autistic traits and mental health in non-autistic people and add to the growing development of theory and knowledge about the mechanism and effects of camouflaging.

Autism spectrum conditions (ASC) were once seen as a predominantly male. This has caused a paucity of information on common events in the lives of women, such as menstruation and menopause. Some smaller studies indicate that autistic individuals might suffer from increased difficulties surrounding these events. This study aims to investigate, whether autistic individuals experience more frequent premenstrual dysphoric disorder (PMDD), and increased complaints surrounding menopause. In 70 individuals (ASC n=28, comparisons n= 42) we did not find an increased prevalence of PMDD in autistic individuals (14.3%) compared to non-autistic individuals (9.5%) . Autistic individuals 40 (n=75, ASC n=30, comparisons n= 35) did experience higher levels of menopausal complaints. In autistic individuals, higher menopausal complaints were associated with higher levels of depression and autistic traits. In non-autistic individuals, menopausal complaints were associated with increased inattention, hyperactivity/impulsivity (i.e., ADHD traits), and depression. With this work, we show the important role that major reproductive milestones can have in an autistic woman’s life.

Abstract Background: Autistic traits (ATs) include symptoms associated with autism spectrum conditions (ASCs), which are assumed to be continuously distributed across the general population. Studies had indicated the cultural differences in the expression ATs. To our knowledge, this is the first study designed to compare the expression of autistic traits between different ethnicities from the same country. Methods: Using the Autism-spectrum Quotient (AQ-28), we examined the possible cultural differences in the expression of autistic traits from four groups of students with different ethnicities backgrounds, including Turkish (n = 262), Persian (n = 290), Kurdish (n = 300), and Luri (n = 307) students. Results: Behaviors associated with autistic traits were reported overall higher for males than females. Also, significant cultural differences in autistic traits were found that were different for males and females. Furthermore, while the medical sciences student group scored significantly than the humanities group in the Imagination dimension, the humanities group had significantly high scores in Number/Pattern dimensions than the engineering and medical sciences groups. Limitations: First, other ethnicities (e.g., Arabs, Baloch) were not studied because of the lack of access. Second, for data gathering, we used only the self-report method. Third, our study included only a student sample but not the community and clinical samples from different ethnicities. Finally, our study sample included only students who are not representative of their entire ethnicity.Conclusions: Altogether, our results provide further support for the idea that the expression of ATs is significantly influenced by culture.

There is a higher than chance representation of autistic people and people with elevated autistic traits in Science, Technology, Engineering and Mathematics (STEM) industries. Medical students, despite studying a STEM subject, have lower autistic traits than other STEM students. Medicine is heterogenous, covering technique-oriented specialties (e.g., surgery) with little patient interaction, person-oriented specialties (e.g., pediatrics), and general practice. We present an online survey in which 502 UK university students (medicine, n = 344; STEM, n = 94; non-STEM, n = 64) reported their study area and career aspirations and completed the Autism Spectrum Quotient (AQ), a quantitative self-report measure of autistic traits. Our main findings were that medical students had significantly lower AQ scores than other STEM ( p < .001, d = 0.614) and non-STEM students ( p < .001, d = 0.874), and that medical students aiming to pursue technique-focused career paths had significantly higher AQ scores than medical students aiming to pursue person-oriented career paths ( p = .009, d = 0.318). Each of these effects remained statistically significant after adjusting for alpha inflation. The findings of this study corroborate those of previous research reporting a link between autism and STEM; they also provide evidence that autistic traits are a predictor of medical students’ career ambitions, with those students with high AQ scores being more likely to pursue technique-focused (as opposed to person-focused) roles. This may be informative for developing and optimizing the strengths of individuals with differing levels of autistic traits.

Weather is important to all people, including vulnerable populations (those whose circumstances include cognitive processing, hearing, or vision differences, physical disability, homelessness, and other scenarios and factors). Autism spectrum conditions (ASC) affect information-processing and areas of neurological functioning that potentially inhibit the reception of hazardous weather information, and is of particular concern for weather messengers. People on the autism spectrum tend to score highly in tests of systemizing, a psychological process that heavily entails attention to detail and revolves around the creation of logical rules to explain things that occur in the world. This article reports the results of three preliminary studies examining weather salience–psychological attention to weather–and its potential relationships with systemizing in autistic people. Initial findings suggest that enhanced weather salience exists among autistic individuals compared to those without the condition, and that this may be related to systemizing. These findings reveal some possible strategies for communicating weather to autistic populations and motivate future work on a conceptual model that blends systemizing and chaos theory to better understand weather salience.

Abstract Background Autistic people show diverse trajectories of autistic traits over time, a phenomenon labelled ‘chronogeneity’. For example, some show a decrease in symptoms, whilst others experience an intensification of difficulties. Autism spectrum disorder (ASD) is a dimensional condition, representing one end of a trait continuum that extends throughout the population. To date, no studies have investigated chronogeneity across the full range of autistic traits. We investigated the nature and clinical significance of autism trait chronogeneity in a large, general population sample. Methods Autistic social/communication traits (ASTs) were measured in the Avon Longitudinal Study of Parents and Children using the Social and Communication Disorders Checklist (SCDC) at ages 7, 10, 13 and 16 (N = 9744). We used Growth Mixture Modelling (GMM) to identify groups defined by their AST trajectories. Measures of ASD diagnosis, sex, IQ and mental health (internalising and externalising) were used to investigate external validity of the derived trajectory groups. Results The selected GMM model identified four AST trajectory groups: (i) Persistent High (2.3% of sample), (ii) Persistent Low (83.5%), (iii) Increasing (7.3%) and (iv) Decreasing (6.9%) trajectories. The Increasing group, in which females were a slight majority (53.2%), showed dramatic increases in SCDC scores during adolescence, accompanied by escalating internalising and externalising difficulties. Two-thirds (63.6%) of the Decreasing group were male. Conclusions Clinicians should note that for some young people autism-trait-like social difficulties first emerge during adolescence accompanied by problems with mood, anxiety, conduct and attention. A converse, majority-male group shows decreasing social difficulties during adolescence.

Abstract Background Individuals on the autism spectrum are reported to display alterations in interoception, the sense of the internal state of the body. The Interoception Sensory Questionnaire (ISQ) is a 20-item self-report measure of interoception specifically intended to measure this construct in autistic people. The psychometrics of the ISQ, however, have not previously been evaluated in a large sample of autistic individuals. Methods Using confirmatory factor analysis, we evaluated the latent structure of the ISQ in a large online sample of adults on the autism spectrum and found that the unidimensional model fit the data poorly. Using misspecification analysis to identify areas of local misfit and item response theory to investigate the appropriateness of the seven-point response scale, we removed redundant items and collapsed the response options to put forth a novel eight-item, five-response choice ISQ. Results The revised, five-response choice ISQ (ISQ-8) showed much improved fit while maintaining high internal reliability. Differential item functioning (DIF) analyses indicated that the items of the ISQ-8 were answered in comparable ways by autistic adolescents and adults and across multiple other sociodemographic groups. Limitations Our results were limited by the fact that we did not collect data for typically developing controls, preventing the analysis of DIF by diagnostic status. Additionally, while this study proposes a new 5-response scale for the ISQ-8, our data were not collected using this method; thus, the psychometric properties for the revised version of this instrument require further investigation. Conclusion The ISQ-8 shows promise as a reliable and valid measure of interoception in adolescents and adults on the autism spectrum, but additional work is needed to examine its psychometrics in this population. Abstract Background There is inconsistent evidence for a clear pattern of association between ‘camouflaging’ (strategies used to mask and/or compensate for autism characteristics during social interactions) and mental health. Methods This study explored the relationship between self-reported camouflaging and generalised anxiety, depression, and social anxiety in a large sample of autistic adults and, for the first time, explored the moderating effect of gender, in an online survey. Results Overall, camouflaging was associated with greater symptoms of generalised anxiety, depression, and social anxiety, although only to a small extent beyond the contribution of autistic traits and age. Camouflaging more strongly predicted generalised and social anxiety than depression. No interaction between camouflaging and gender was found. Limitations These results cannot be generalised to autistic people with intellectual disability, or autistic children and young people. The sample did not include sufficient numbers of non-binary people to run separate analyses; therefore, it is possible that camouflaging impacts mental health differently in this population. Conclusions The findings suggest that camouflaging is a risk factor for mental health problems in autistic adults without intellectual disability, regardless of gender. We also identified levels of camouflaging at which risk of mental health problems is highest, suggesting clinicians should be particularly aware of mental health problems in those who score at or above these levels.
