Abstract BackgroundThe burden of chronic diseases seriously affects the health and quality of life of the geriatric. Many studies have pointed out that traditional Chinese medicine(TCM) can alleviate the problem of expensive medical treatment. It is unknown that how situation of the utilization of TCM in geriatric patients with chronic diseases and the impact of the use of TCM on their medical burden.MethodsThrough stratified multi-stage sampling method, 1336 institutions in Hunan Province of China were surveyed to obtain expenditure inpatient’s of geriatric with chronic diseases in 2018. Possion regression model was used to analysis the utilization of TCM. Kolmogorov-Smirnov test was performed to observe the medical burden in TCM user’ group and TCM nousers’ group. The relationship between proportion of expenses of TCM to expenses of drugs(Ptcm) and medical burden were analyzed using Spearman’s correlation analyses.ResultsIn geriatric inpatients with chronic diseases, female (RR=1.009, RR95%CI: 1.006-1.013), patients who have not undergone surgery(RR=1.246, RR95%CI:1.237-1.255), the elderly and the insured patients were more likely to use TCM for treatment. The distribution of Totalexp in the TCM users group and the TCM nousers group was different in various types of chronic diseases (all p<0.001). For some types of chronic diseases, the Ptcm has a negative correlation with Totalexp, cost of medicine,OOPs(rs<0 and p<0.005).ConclusionThere were differences in the utilization of TCM treatment of geriatric patients with chronic diseases in terms of gender, age, institution and insurance status. Treatment mainly used TCM for geriatric chronic diseases of respiratory system, digestive system, circulatory system, ear and neurological has obvious cost-benefit advantages, and conducive to reducing the burden of patients.

: Mitophagy plays an important role in maintaining mitochondrial quality and cell homeostasis through the degradation of damaged, aged, and dysfunctional mitochondria and misfolded proteins. Many human diseases, particularly neurodegenerative diseases, are related to disorders of mitochondrial phagocytosis. Exploring the regulatory mechanisms of mitophagy is of great significance for revealing the molecular mechanisms underlying the related diseases. Herein, we summarize the major mechanisms of mitophagy, the relationship of mitophagy with human diseases, and the role of traditional Chinese medicine (TCM) in mitophagy. These discussions enhance our knowledge of mitophagy and its potential therapeutic targets using TCM.

Abstract Background: Large-scale epidemics have changed people’s medical behavior, and patients tend to delay non-urgent medical needs. However, the impact of the pandemic on the use of complementary and alternative medicine remains unknown. Methods: This retrospective study aimed to analyze the changes in the number of traditional Chinese medicine (TCM) patients and examine the epidemic prevention policy during the coronavirus disease 2019 (COVID-19) pandemic. We analyzed the number of TCM patients in Taipei City Hospital from January 2017 to May 2020. We tallied the numbers of patients in each month and compared them with those in the same months last year. We calculated the percentage difference in the number of patients to reveal the impact of the COVID-19 pandemic on TCM utilization. We used the Mann­–Whitney U test to examine whether there was a significant difference in the number of patients during the COVID-19 pandemic. Results: We included a total of 41 months and 1,935,827 patients in this study. During the COVID-19 pandemic, the number of patients decreased significantly, except in February 2020. The numbers of patients during the COVID-19 pandemic had fallen by more than 15% compared with those in the same months last year. March and April had the greatest number of patient losses, with falls of 32.8% and 40%, respectively. TCM patients declined significantly during the COVID-19 pandemic, and mobile medicine provided to rural areas fell considerably. Among all the TCM specialties, pediatrics and traumatology, as well as infertility treatment, witnessed the most significant decline in the number of patients. However, the number of cancer patients has reportedly increased. Conclusions: The COVID-19 pandemic decreased the utilization rate of TCM, especially for mobile healthcare in rural areas. We suggest that the government pay attention to the medical disparity between urban and rural areas, which are affected by the pandemic, as well as allocate adequate resources in areas deprived of medical care.

Paeoniflorin (PF) is the major active ingredient in the traditional Chinese medicine Radix. It plays a neuroprotective role by regulating autophagy and the ubiquitin-proteasome degradation pathway. In this study, we found PF significantly reduced cell damage caused by MPP+, returning cells to normal state. Cell viability significantly improved after 24 h exposure to RAPA and PF in the MPP+ group (allP<0.01). CAT and SOD activities were significantly decreased after PF and RAPA treatment, compared with MPP+ (P<0.001). In addition, MPP+ activated both LC3-II and E1; RAPA increased LC3-II but inhibited E1. PF significantly upregulated both LC3-II (autophagy) and E1 (ubiquitin-proteasome pathway) expression (P<0.001), promoted degradation ofα-synuclein, and reduced cell damage. We show MPP+ enhanced immunofluorescence signal of intracellularα-synuclein and LC3. Fluorescence intensity ofα-synuclein decreased after PF treatment. In conclusion, these data show PF reversed the decline of proteasome activity caused by MPP+ and significantly upregulated both autophagy and ubiquitin-proteasome pathways, promoted the degradation ofα-synuclein, and reduced cell damage. These findings suggest PF is a potential therapeutic medicine for neurodegenerative diseases.

Polycystic ovary syndrome (PCOS) is a touchy clinical and public health problem worldwide, which adversely affects women’s health and health-related comorbidities for lifetime, and represents a tremendous burden for both the family of the patient and for society. According to the diagnostic criteria used and the population studied, the prevalence rate of PCOS is between 6% and 21%. However, current conventional modern medicines for PCOS are only moderately effective at controlling the signs and symptoms, while they are not thoroughly able to prevent complications. Therefore, many patients have turned to complementary and alternative medical (CAM) treatments. CAM use is wide spread among patients with PCOS, and more than 70% of patients use CAM at one point during their diseases. The patients’ primary motivations include dissatisfaction with available medications, perceive higher risk of drug side effects and crushing health burden and economic costs, desire for symptom relief, pursuit of shortening the course of disease, and the belief that CAM therapy is in accordance with the patients’ values and beliefs. At present, several CAM methods have been used in women with PCOS, which has achieved obvious effects. However, biologically plausible mechanisms of the action of traditional Chinese medicine- (TCM-) associated CAM for PCOS have not been systematically reviewed. This review briefly summarizes the current progress of the impact of herbal medicine on the outcomes of PCOS and introduces the mechanisms.
