Factors Related to Diabetes Management in Mentally Ill Patients with Type 2 Diabetes: Focusing on Diabetes Knowledge

BACKGROUND Type 2 diabetes mellitus (T2D) can be managed through diet and lifestyle changes. The American Dietetics Association acknowledges that knowing what and when to eat is the most challenging aspect of diabetes management. While current recommendations for self-monitoring of diet and glucose levels aim to improve glycemic control among people with T2D, tracking all intake is burdensome and unsustainable. Equally effective, but lower burden, dietary self-monitoring approaches should be explored. OBJECTIVE To examine the feasibility of abbreviated dietary self-monitoring in T2D where only carbohydrate-containing foods are recorded into a diet tracker. METHODS We used a mixed methods approach to quantitatively and qualitatively assess general and diet-related diabetes knowledge and the acceptability of reporting only carbohydrate-containing foods in N=30 men and women with T2D. RESULTS The mean Diabetes Knowledge Test score was 83.9±14.2%. Only 6 of 30 (20%) participants correctly categorized 5 commonly-consumed carbohydrate-containing and 5 non-carbohydrate containing foods. The mean perceived difficulty of reporting only carbohydrate-containing foods was 5.3 on a 10-point scale. Approximately half of the participants (53.3%, n=16) preferred to record all foods. A lack of knowledge about carbohydrate-containing foods was the primary cited barrier to acceptability (40%, n=12). CONCLUSIONS Abbreviated dietary self-monitoring, where only carbohydrate-containing foods are reported, is likely not feasible due to limited carbohydrate-specific knowledge and a preference of the majority to report all foods. Other approaches to reduce the burden of dietary self-monitoring for people with T2D that do not rely on food specific knowledge could be more feasible.

The purpose of this study was to learn from 29 Black women how to develop effective Type 2 diabetes programming. Three focus groups were held in Des Moines, Iowa, during fall 2012. Results highlighted themes related to diabetes knowledge, diabetes management and prevention, physical activity, diet, and diabetes management programming. Opinions were shared as to whether family members should be included in programs for supporting those diagnosed with diabetes. These results provided guidance and ideas to scholars and health care professionals aiming to improve effectiveness of diabetes programs for Black women and families.

We investigate the relationship of diabetes knowledge, diabetes management self-efficacy and diabetes self-management with blood glucose control among people with Thai type 2 diabetes mellitus (T2D). Seven hundred outpatients from diabetes clinics from large university and small community hospitals in two provinces of Thailand (Khon Kaen and Bangkok) were interviewed to evaluate their diabetes knowledge (DK), diabetes management self-efficacy (DMSE) and diabetes self-management (DSM). In addition, patient medical records were accessed to obtain other patient characteristics including patients’ HbA1c levels. Bivariate and multivariable logistic regression modelling was conducted and unadjusted and adjusted odds ratios obtained, respectively. Over half (52.4%) of the patients in our sample failed to control their blood glucose (HbA1c > 7%). All three psychometric measures (DK, DMSE and DSM) were identified as associated with blood glucose control in the bivariate analysis (ORDK(unadj) = 0.89, 95%CI: 0.82, 0.96; ORDSM(unadj) = 1.64, 95%CI: 1.46, 1.82; ORDMSE(unadj) = 2.84; 95%CI: 2.43, 3.32). However, after mutual adjustment and adjustment for other patient characteristics, of the three psychometric measures, only diabetes management self-efficacy remained associated with blood glucose control (ORDMSE(adj) = 2.67; 95%CI: 2.20, 3.25). Diabetes management self-efficacy is shown to be strongly associated with blood glucose control in the Thai Type 2 diabetes population. Current early diabetes interventions in Thailand tend to focus on disease knowledge. A stronger emphasis on enhancing patients’ disease management self-efficacy in these interventions is likely to lead to substantial improvement in both diabetes self-management and blood glucose control, thereafter reducing the risk, or prolonging the development, of chronic diabetes complications.

Purpose This qualitative study utilizes the health belief model to explore how individuals with type 2 diabetes mellitus and low diabetes numeracy achieve and maintain good glycemic control. Methods Participants (n=10) participated in semi-structured interviews and completed the Patient Diabetes Knowledge Questionnaire, the Health Belief Questionnaire, and the Social Support Assessment Tool. Results Results indicate that participants had low levels of diabetes knowledge, high levels of perceived social support, and strong self-efficacy related to type 2 diabetes management, despite low numeracy. Self-designed routines mitigated potential educational, socioeconomic, and knowledge barriers. Self-efficacy and social support were strong protective factors.

Rates of type 2 diabetes mellitus continue to rise around the world, largely due to lifestyle factors such as poor diet, overeating, and lack of physical activity. Diet and eating is often the most challenging aspect of management and, when disordered, has been associated with increased risk for diabetes-related complications. Thus, there is a clear need for accessible and evidence-based interventions that address the complex lifestyle behaviors that influence diabetes management. The current study sought to assess the efficacy and acceptability of a pilot lifestyle intervention for women with type 2 diabetes and disordered eating. The intervention followed a cognitive behavioral therapy guided-self-help (CBTgsh) model and included several pillars of lifestyle medicine, including: diet, exercise, stress, and relationships. Ten women completed the 12-week intervention that provided social support, encouraged physical activity, and addressed eating behaviors and cognitions. Results indicate the lifestyle intervention was a feasible treatment for disordered eating behaviors among women with type 2 diabetes and was also associated with improved diabetes-related quality of life. The intervention was also acceptable to participants who reported satisfaction with the program. The current CBTgsh lifestyle intervention is a promising treatment option to reduce disordered eating and improve diabetes management.
