The extent and severity of motility disorders remains heterogeneous in the different parts of the gut, and in most cases failures in gut motility do not correspond with the severity of the symptoms. If diarrhea or fecal incontinence is the leading symptom, or the blood glucose level varies frequently and considerably despite the treatment efforts, the motility of the stomach and bowels is seriously disturbed. The clinical aspects, detailed pathogenesis, diagnostic approach and treatment modalities of gastrointestinal motility disorders in diabetes mellitus are reviewed to help and improve the everyday medical practice. Pain is the leading symptom with degenerative-dystrophic diseases of the vertebral column. Now for the treatment of vertebral pain using pharmacological and non-pharmacological treatments. However, in some cases, the application of these techniques is contraindicated or ineffective. In this regard, relevant search for new methods of therapy of pain syndrome. It is known that arginine-vasopressin (AVP), along with the hormonal effects shows properties of neuromodulator. The experiment shows that AVP showed analgetic properties. AVP reduced the severity of pain in patients with renal colic, with migraine, pains due to cancer [24, 36, 37]. There is not data on the impact of AVP on vertebrogenic pain syndrome. In this context, the aim of the research was to assess the effectiveness of the agonist V2-receptors, 1-dezamino-8-D-arginine-vasopressin, DDAVP, in correction of pain syndrome in patients with degenerative-dystrophic diseases of the vertebral column. Treatment received 23 patients. DDAVP is injected intranasal drip in a daily dose of 1•10 -7 g. The control group consisted of 10 patients with vertebral pain who received traditional therapy. Therapeutic effect of DDAVP was in 87 % of cases. Pain decreased after 2.5-3 hours from the beginning of treatment in patients with moderate and mild pain. Along with reduction of pain regressed fatigue-depressive disorders. Thus, it was shown that the DDAVP was effective and safe in the treatment of vertebral pain.

AbstractAmyloidosis is a rare disease (incidence about 0.8/100 000) characterized by extracellular tissue deposition of fibrils composed of low molecular weight subunits of a variety of serum proteins. Clinical manifestations are largely determined by the type of precursor protein, the tissue distribution and the amount of amyloid deposition. Gastrointestinal (GI) manifestations of amyloidosis are even more uncommon (3 % of all amyloidosis patients). Symptoms of GI amyloidosis are nonspecific, heterogeneous, and include weight loss, GI bleeding, heartburn, early satiety, diarrhea and abdominal pain. The histopathological examination of biopsy specimens from the GI tract leads to the diagnosis.Herein we report the case of a 63-year-old woman with recurrent diffuse gastric bleeding. Endoscopic biopsies revealed distinct amyloid deposits in the mucosa of the stomach. Further histochemical assessment confirmed systemic light chain (AL) amyloidosis with clinically predominant gastrointestinal manifestation. An induction therapy with bortezomib and dexamethasone was initiated.Our report illustrates the importance of the multidisciplinary approach for diagnosis and management of AL amyloidosis. Current treatment of systemic AL amyloidosis is based on cytostatic targeting of immunoglobulin producing plasma cells. Therapeutic options are limited and highly toxic, making the development of novel treatment approaches an urgent need.

Cytomegalovirus (CMV) infection rarely manifests with skin ulcerations. We describe a case report of a 64-year-old woman with chronic immunosuppression for treatment of mixed connective tissue disease, presenting with new onset leg ulcerations after a recent change in immunosuppressive regimen. She subsequently developed fulminant hepatitis, encephalopathy, and pancytopenia and was found to have severe systemic CMV viremia. Skin ulcer biopsy was positive by immunohistochemical staining for CMV infected endothelial cells. Both systemic disease and skin ulcer rapidly improved after stopping immunosuppression and administering intravenous ganciclovir. New onset skin ulcers in an immunosuppressed individual, especially with recent changes in immunosuppressive regimen, should raise the suspicion of reactivation of CMV.

Uterine fibroid are benign tumors that consist most of the tumor occurrences in pelvic cavity of women. Possible courses of treatment include: medicament treatment, surgical and new intra-arterial embolisation method. The aim of the paper is to present achieved results in intra-arterial embolisation treatment of Uterine fibroid. Material and methods: A prospective study was performed in the period from October 2007 until October 2008. It included 36 women with symptomatic uterine fibroid. They were treated by intra-arterial embolisation. All patients had control MRI examinations after treatment. Embolisation was performed by 'cross-over' technique, using bilateral punction of femoral arteries, a selective catheterization a. uterine, and application of 750-900m Bead Blocks. Results: Average age of patients was 38.5 years. The most of fibroids had intramural localization (39%). solitary fibroids were seen in 56% of patients, multiple in 44%. Gynecological hemorrhage was the leading symptom with 34 patients (94%). One year after receiving intra-arterial embolisation treatment, fibroid regression of 50% was registered in all patients. There were no serious complications noted in our study, with the exception of postembolisation syndrome which occurred in 11 (30%) patients in a relatively mild form Conclusion: Intra-arterial embolisation of uterine fibroma is a method that in large percent of patients removes symptoms of uterine fibroma presence, prevents its growth, and safely helps women enter menopause when activity of hormone dependant tumor ceases.

22 patients diseased with secondary arthrosis, developed from dislocation of TMJ disc, were observed by the authors of the article. Between these patients were 17 females and 5 males. The age of patients was from 21 to 60 years. In the clinical case, the leading symptom was constant or periodical pain, located at the injured joint, limitation of mandibular movement, aggravation of pain while moving jaw and was attached with clicking in joint. In the complex treatment of this problematical disease, successfully and for the first time was used autoplasmolifting. Authors injected autoplasma once in 5-6 days, 6 injections on one course of treatment. After noted treatment, 18 patients all the symptoms were released, 4 patients condition got better there was slight pain left, while moving jaw. In 1 patient's case, the neuralgia of III branch of the trigeminal nerve was diagnosed and he received suitable treatment. In 3 cases there was necessary to include orthopedic/orthodontic treatment.
