22 patients diseased with secondary arthrosis, developed from dislocation of TMJ disc, were observed by the authors of the article. Between these patients were 17 females and 5 males. The age of patients was from 21 to 60 years. In the clinical case, the leading symptom was constant or periodical pain, located at the injured joint, limitation of mandibular movement, aggravation of pain while moving jaw and was attached with clicking in joint. In the complex treatment of this problematical disease, successfully and for the first time was used autoplasmolifting. Authors injected autoplasma once in 5-6 days, 6 injections on one course of treatment. After noted treatment, 18 patients all the symptoms were released, 4 patients condition got better there was slight pain left, while moving jaw. In 1 patient's case, the neuralgia of III branch of the trigeminal nerve was diagnosed and he received suitable treatment. In 3 cases there was necessary to include orthopedic/orthodontic treatment.

Back pain is a common disorder in pregnancy, pregnant women may have had back pain before. You can do yoga exercises which are useful for forming an upright posture. This  research was an experimental one group pretest posttest design. The sample of this study used accidental sampling with a sample of 16 trimester III pregnant women at Masita Clinic. The results of this study showed that the pain scale in the pretest was on a scale of 1 (slight pain) as many as 11 pregnant women (68.75%) and the pain scale in the postest for some of the respondents were on a pain scale of 0 (no pain) as many as 8 pregnant women (50%). Based on the results of the study, it can be concluded that there was a decrease in the level of back pain in third trimester pregnant women after doing yoga.

Currently, the problem of treatment and prevention of mammary gland diseases in cows is relevant. In this regard, the task of developing new drugs to combat mastitis, with a wide spectrum of antimicrobial activity and minimal side effects, remains relevant. Authors studied subchronic toxicity of Primalact preparation with intracisternal administration to lactating cows. The experiment was performed on 12 clinically healthy cows at the 3rd-4th months of lactation. 3 experimental and 1 control groups were formed. Animals in the experimental groups for 9 days were intracisternally injected in doses of 5, 10 and 20 ml, respectively. Examination data showed the absence of significant changes in the clinical state of the experimental animals and breast tissue in the two experimental groups. In the group with a dose of 20 ml on the 5th day, an increase in the temperature of the udder lobes, their densification, slight pain, the appearance of flakes in the secretion of the udder and a decrease in dairy productivity were detected. In this case it indicated the presence of an irritating effect. The administration of Primalact for 9 days in a dose of 5 ml and 10 ml to lactating cows did not cause any changes in the morphological and biochemical blood parameters. The results of the studies in the fourth experimental group were reflected in the instructions for the application of the drug, which indicated that the frequency of administration of the drug was 3-4 days. Thus, Primalact, when intracisternially administered to lactating cows in doses of 5 and 10 ml for 9 days, does not have subchronic toxicity.

Abstract Background: Although various methods have been introduced, the management of chronic tibial osteomyelitis remains a challenge. This study aims to assess a combined treatment method, local debridement combined with antibiotic-loaded calcium sulfate implantation, for the management of the local (Cierny-Mader type III) tibial osteomyelitis.Methods: 42 patients (43 limbs) with type III tibial osteomyelitis, from January 2012 to December 2018, who received the treatment method mentioned above were included in the study. The infection remission rate, recurrence rate, complication rate and bone healing rate were respectively analyzed. Results: With a mean follow-up of 42.8 months, 38 limbs (37 patients) (88.4%, 38/43) achieved infection remission without recurrence. Among those patients pain, limitation of movement, sinus tracts, topical redness and swelling were generally eliminated. Only 4 patients felt slight pain after a long-distance walk, while another 6 patients showed minor but acceptable discomfort in affected limbs. 5 patients (11.6%) suffered from osteomyelitis recurrence that required secondary surgical and medical treatment, but no amputation was necessary to eliminate the infection. Prolonged aseptic drainage was the most frequent complication that was observed in 13 patients (30.0%). They were successfully managed by appropriate wound caring in 10 patients and by surgical intervention, months later, in 3 patients. According to the final X-ray’s examination, bone losses caused by local debridement were generally repaired, though the shape of the tibia was not well restored to its initial form in 17 limbs. No fracture was recorded during follow-up. Conclusion: Local debridement combined with antibiotic-loaded calcium sulfate implantation is effective and safe in a single-stage treatment of chronic Cierny-Mader III tibial osteomyelitis.

Underfills in endodontic treatment increases failure rates by 14%. A 23-year-old female patient came with chief complaint of slight pain on #35. The periapical radiograph showed underfilled root canal and periodontal ligament space widening. The root canal filling was removed with reciprocal file system. Follow-up visit showed no recurrence pain and the tooth function was fully restored using a fiber-reinforced post and an all-ceramic crown.

Nodular fasciitis (NF) is a self-limiting fibrous neoplasm that can be mistaken for a soft tissue sarcoma. It is characterised by rapid growth, slight pain and local tenderness. Although it is frequently found in the forearm, a lesion distal to the wrist is quite rare. We present two unusual cases of NF involving the palm, supported by detecting ubiquitin specific protease 6 gene rearrangement. The first patient had non-intraneural NF presenting as peripheral neuropathy affecting the digital nerve while the second patient suffered from painless, non-tender NF in the palm, which had not regressed spontaneously during the five months prior to surgery.

The case of a 54-year-old cardiac patient is reported, who was admitted to hospital with a complaint of sudden pain in the legs associated with edema, paresthesia, and coldness. Arterial embolism of the lower limbs was diagnosed and the patient was submitted to bilateral embolectomy. The patient evolved with a burning sensation, hypersensitivity in the right leg, swelling, and difficulty bending and stretching the sole of the foot and the knee. A physical examination detected edema and increased tension in the anterior, lateral, and posterior compartments. Treatment using intermittent massage of the leg during the evaluation of the patient was chosen in an attempt to stimulate lymphatic and venous drainage. After a few minutes of stimulation, there was significant improvement in the pain and edema. In 40 minutes, there was total reduction of the pain in the posterior and lateral compartments and improvement of over 50% in the anterior compartment. After this, the patient started to bend the knee without pain and bend the sole of the foot with slight pain. On the following day, the patient was walking around the hospital ward without difficulty. It seems that intermittent massage is a therapeutic option in selected cases of compartment syndrome.

A 66-year-old woman with HIV-1 infection recently commenced on antiretroviral therapy (CD4+ 25 cells/mm3 was referred to the Dermatology Clinic the following month due to a well-demarcated nodule in the extensor surface of the left arm with evident fluctuation but only slight pain on palpation, with no increase in temperature. Surgical drainage was performed with aspiration of yellowish-green exudate, with no characteristic smell. In culture of cutaneous exudate, Mycobacterium intracellulare was isolated. Upon careful review of the laboratory tests that were in progress at discharge, the same agent was isolated in one of the bronchoalveolar lavage cultures. The diagnosis of cutaneous abscess caused by M. intracellulare from hematogenous dissemination of lung infection was made. The patient was treated with clarithromycin, ethambutol and rifabutin for 24 months. M. intracellulare species and Mycobacterium avium constitute the Mycobacterium avium–intracellulare complex (MAC), responsible for the majority of human infections by atypical mycobacteria. They are ubiquitous bacteria and MAC infection mainly affect immunocompromised patients, with M. intracellulare being isolated in <5% of HIV patients with MAC infection. Cutaneous infection is rare and may present clinically with erythematous plaques, chronic ulcers or abscesses. When present, skin involvement is usually secondary to pulmonary infection.

Presented clinical case laparoscopic decompression of celiac trunk syndrome of Dunbar. In most cases, the diagnosis of syndrome of compression of the celiac trunk is by exclusion of other gastrointestinal diseases. Sure, it tells about lack of awareness among the medical community about this specific and rare pathology, as well as a possible skeptical attitude towards it related professionals. The main methods of diagnostics of a syndrome of Dunbar are: ultrasonic duplex scanning of the abdominal aorta, multislice computed tomography and angiography with a respiratory samples. The indication for surgical treatment is the ineffectiveness of conservative therapy, the preservation of the leading symptoms of abdominal ischemia on the ground proved critical in compression of the celiac trunk. Currently, laparoscopic decompression of celiac trunk syndrome Dunbar is the operation of choice. The obvious advantages of laparoscopic surgery are less blood loss, a slight pain syndrome in the postoperative period, short hospital period and better cosmetic effect. In the clinic, where surgeons proficient in the technique of laparoscopic procedures, the surgery can be performed quickly, safely, and efficiently. Additional intervention in the form of endovascular correction is not required in all patients and can be performed in a delayed order.

Objective: Premature uterine contraction caused by spontaneous rupture of subserous uterine vein resulting intrauterine asphyxia.Method & Results: A primigravida with uneventful pregnancy having regular antenatal care attended a private clinic at her 38 week of pregnancy with slight pain in whole abdomen and hardening of uterus. After giving rest in left lateral position & oxygen inhalation, hardening of uterus persisted. By that time she developed fetal tachycardia and had to undergo caesarean section, there was hemoperitonum and an asphyxiated male baby was delivered. On exploration a subserous uterine vein was detected on the posterior wall of the uterus. Complete hemostasis was achieved with interrupted sutures and electrocauterization of the bleeding points. Post-operative period was uneventful for the mother but the baby was managed in neonatal care unit. Both of them were well during discharge.Conclusion: Monitoring of pregnant women at last trimester is very important. Any deviation from normal like hardening of uterus should be carefully taken care of for good maternal and fetal outcome.Bangladesh J Obstet Gynaecol, 2012; Vol. 27(2) : 87-89
