22 patients diseased with secondary arthrosis, developed from dislocation of TMJ disc, were observed by the authors of the article. Between these patients were 17 females and 5 males. The age of patients was from 21 to 60 years. In the clinical case, the leading symptom was constant or periodical pain, located at the injured joint, limitation of mandibular movement, aggravation of pain while moving jaw and was attached with clicking in joint. In the complex treatment of this problematical disease, successfully and for the first time was used autoplasmolifting. Authors injected autoplasma once in 5-6 days, 6 injections on one course of treatment. After noted treatment, 18 patients all the symptoms were released, 4 patients condition got better there was slight pain left, while moving jaw. In 1 patient's case, the neuralgia of III branch of the trigeminal nerve was diagnosed and he received suitable treatment. In 3 cases there was necessary to include orthopedic/orthodontic treatment.

The article suggests using superelastic active elements of titanium niсkelid to move teeth in patients with secondary dental displacements due to periodontitis. It is emphasized that such superelastic instrument only has a sparing effect on periodontium and makes this procedure altogether nontraumatic. Devices featuring such active elements are considered indispensable for treatment of morbid thin-biotype periodontium. Presented is the original clinical case of chronical generalized periodontitis of medium severity, secondary vestibular displacement of upper front teeth, absence of upper masticatory teeth. The photographs show the patient’s condition before and after the complex treatment of the patient, thin periodontal biotype.

Associated with pregnancy is breast cancer, which was first detected during pregnancy, during the first year after childbirth or at any time against lactation. Diagnosis of the disease in the first trimester is an indication for abortion. The detection of the disease after 20 weeks and the desire of the woman to maintain pregnancy is the basis for conducting a total mastectomy followed by polychemotherapy with doxorubicin with cyclophosphamide or with fluorouracil. Radiation therapy during pregnancy is not applied. The timing and method of delivery are determined individually and depend on the stage of the process and the period of pregnancy, when it was identified. A clinical case of a patient with edematous-infiltrative form of breast cancer of the IV stage, diagnosed for the first time in 22 weeks of pregnancy, is presented.

Currently, there is no consensus on the place of prostatectomy in the complex treatment of patients with metastatic prostate cancer. A description of a clinical case of complex treatment and observation of a patient with prostate cancer with an unfavorable baseline prognosis and the presence of bone metastases with a good clinical result is presented.

The article presents the results of clinical and microbiological studies and immunological features of the state of periodontal tissues in patients with moderate periodontitis, which developed as a complication of orthodontic treatment. It was found that at the later stages of orthodontic treatment with the development of periodontitis, a complex of persistent changes in the periodontal tissues at different levels of pathogen-esis was revealed in patients. In this connection, in order to plan the complex treatment of patients with periodontitis, which has developed as a complication of orthodontic treatment, and to predict the course of the disease, it is necessary to influence the established and mutually supportive links and indicators of pathogenesis.

Objective: This case report aims to present the microdontia treatment of tooth 35 through pre-prosthetic orthodontics, followed by re-anatomization with an indirect restoration. Case report: A 21-year-old female patient presented with microdontia in tooth 35, with a distalized and vestibularized crown. In addition, tooth 34 was gyroverted with mesial diastema. Orthodontic mechanics with segmented arch were used to centralize tooth 35 in the alveolar ridge, close the diastema in the mesial of tooth 34, and correct its gyroversion. For lingualization of tooth 35, a first-order fold (in set) was performed, and for mesialization, an open-coil spring was used between teeth 35 and 36. A closed-coil spring was used to delimit the movement of tooth 35 to mesial. The force applied by the open spring for mesialization of tooth 35 was transmitted to tooth 34, through the closed spring, resulting in its mesialization; this, closed the diastema between teeth 33 and 34 and corrected the gyroversion. Subsequently, the coronary preparation was performed with diamond tips 1014 and 3146, and confection of the temporary crown in acrylic resin. For the ceramic system, IPS™ e.max Press was used. The ceramic crown was luting using the adhesive technique, followed by light curing for 40 seconds on each face of the tooth. Conclusion: It can be concluded that the re-anatomization of a small tooth should be the first treatment option. However, pre-prosthetic orthodontic treatment may be necessary.

The article presents a clinical case of the diagnostics and treatment of the temporomandibular joint dysfunction (TMJ) in patient after orthodontic treatment. The hypertonia of masticatory muscles, limit of mouth opening and deviation of the jawbone to the right were observed during physical examination. Adhesion of the disk to the articular tubercle of the right temporal bone was detected on MRI. Hyperrotation of the articular head and the deviation of the jawbone to the right were observed on the charts during axiography. Disappearing of the feeling of heaviness in the right TMJ, free mouth opening, absence of deviation symptom, and the reduction of the rigidity of the masseter muscles were observed on the basis of diagnostic examinations after treatment. Presented clinical experience lets us make a conclusion that it is necessary to prescribe taking axiography and MRI during diagnosing and treating temporomandibular joint dysfunction.

Summary Fetuin-A is a negative acute phase reactant, while procalcitonin is an indicator of severe bacterial infection. Diagnosis of bacterial infection in febrile seizure (FS) is important for choosing the most suitable treatment. In this study, serum fetuin-A was estimated, for the first time, in the inpatients with FS and compared with procalcitonin and blood culture tests. A total of 60 children (28 male and 32 female) with FS in addition to 30 sex- and age-matched children participated in the study. Patients were classified according to sex, age, PCT level (high PCT>0.5ng/mL), C-reactive protein (CRP, positive >6mg/L), and according to the results of the blood culture. Fetuin-A level decreased and PCT level increased in FS patients in comparison to those in the control group. These changes are significantly increased (p<0.05) in the positive CRP group compared with that of the negative CRP group. Kernel density estimation showed that procalcitonin is a better indicator of the infection in FS children than fetuin-A . Procalcitonin is more sensitive and specific than fetuin-A and when used together they produce 100% sensitivity and specificity for the diagnosis of bacterial infection in FS patients. Fetuin-A is low in FS patients and can be used with procalcitonin in the diagnosis of bacterial infection in FS.

ABSTRACT This study discusses the role of orthodontic treatment as an adjunct to the control and treatment of periodontal disease conditions, and describes a clinical case of severe anterior mandibular crowding and periodontal disease followed up for nine years and three months after orthodontic treatment completion. Malocclusion impaired proper dental hygiene, which led to bone loss and development of a periodontal abscess between mandibular canines and lateral incisors. After scaling and root planing, orthodontic treatment was initiated with extraction of the four second premolars, to correct the deficiency detected in cephalometric and model analysis. Treatment objectives were met, and facial and dental esthetics was satisfactory. Adequate periodontal management, hygiene control and tooth movement ensured ideal occlusion and facilitated the control of biofilm.
