The article analyzes modern methods of personnel health management and provides examples of the implementation of the most interesting methods in foreign and Russian companies. The rationale is presented that the launching of healthcare management system can contribute to the improvement of competitiveness and growth of employee productivity, strengthening of corporate spirit, as well as creates a positive image and reputation of the company. The authors analyze the dynamics of the VHI market as one of the most demanded practices on care for the health of employees in Russia. The calculations are made on the dynamics of concluding VHI contracts with individuals and legal entities. The authors determine the dynamics of the average premium on VHI with legal entities for the last three years and the rates of increase of the volume of contracts from 2015 to 2019. Also the rolling factor of insurance payments is calculated. The forecast of the volume of VHI contracts with individuals for 2020–2022 is made. Also examples of the application of international medical voluntary insurance policies (MMS) in the performance of offi duties in dangerous working conditions, which can lead to the emergence of professional or serious diseases, are provided. The data on medical start-ups, which are able to positively aff the care of companies about the health of employees, are summarized. The authors provide data on practices on employee health care in domestic companies Lukoil, Rostelecom, Sberbank, Seventh Continent, etc., as well as data on cost savings of companies in the implementation of the system of employee health management. Some recommendations on the application of methods of managing the health of employees in domestic companies are presented.

This chapter focuses on conducting worksite investigations, including walkthrough surveys, and provides occupational health and safety personnel, employees, and employers the opportunity to identify and assess current workplace conditions and employee health concerns and make recommendations on how to reduce or eliminate any identified workplace hazards. The methods described cover ways to implement corrective actions necessary for preventing future adverse incidents and to identify shortcomings in safety and health management programs. Various specific examples are provided. The National Institute for Occupational Safety and Health Hazard Evaluation Program, which has experience with all types of workplace hazards, is described. Preparing for and conducting workplace investigations is described in detail.

Action research (AR) can be an effective form of ‘on the job’ training. However, it is critical that AR cycles can be appropriately recorded in order to contribute to reflection and learning. One form of recording is for coresearchers to keep a diary. We found no previous literature describing the use of diaries in AR in sub-Saharan Africa. We therefore use this paper to reflect on how diaries were used by district health management teams in the PERFORM project. We share five lessons from our experience. First, it is important to foster ownership of the diary by the people who are responsible for filling it in. Second, the purpose of keeping a diary needs to be clear and shared between researchers and practitioners from the very beginning. Third, diaries should be allowed to evolve. Fourth, it is a challenge for busy practitioners to record the reflection and learning processes that they go through. Last, diaries on their own are not sufficient to capture reflection and learning. In conclusion, there is no best way for practitioners to keep a diary; rather the focus should be on ensuring that an AR recording process (whether diary or otherwise) is locally owned and complements the specific practice setting.

Oil and gas industries are constantly aiming at improving the efficiency of their operations. In this respect, focus is on the development of technology, methods, and work processes related to equipment condition and performance monitoring in order to achieve the highest standards in terms of safety and productivity. To this aim, a key issue is represented by maintenance optimization of critical structures, systems, and components. A way towards this goal is offered by Condition-Based Maintenance (CBM) strategies. CBM aims at regulating maintenance scheduling based on data analyses and system condition monitoring and bears the potential advantage of obtaining relevant cost savings and improved operational safety and availability. A critical aspect of CBM is its integration with condition monitoring technologies for handling a wide range of information sources and eventually making optimal decisions on when and what to repair. In this chapter, a CBM case study concerning choke valves utilized in Norwegian offshore oil and gas platforms is proposed and investigated. The objective is to define a procedure for optimizing maintenance of choke valves by on-line monitoring their condition and determining their Remaining Useful Life (RUL). Choke valves undergo erosion caused by sand grains transported by the oil-water-gas mixture extracted from the well. Erosion is a critical problem which can affect the correct valve functioning, resulting in revenue losses and cause environmental hazards.

SummaryIntroduction Spending on pharmaceuticals in the US reached $373.9 billion in 2014. Therapeutic interchange offers potential medication cost savings by replacing a prescribed drug for an equally efficacious therapeutic alternative.Methods Hard-stop therapeutic interchange recommendation alerts were developed for four medication classes (HMG-CoA reductase inhibitors, serotonin receptor agonists, intranasal steroid sprays, and proton-pump inhibitors) in an electronic prescription-writing tool for outpatient prescriptions. Using prescription data from January 2012 to June 2015, the Compliance Ratio (CR) was calculated by dividing the number of prescriptions with recommended therapeutic interchange medications by the number of prescriptions with non-recommended medications to measure effectiveness. To explore potential cost savings, prescription data and medication costs were analyzed for the 45,000 Vanderbilt Employee Health Plan members.Results for all medication classes, significant improvements were demonstrated – the CR improved (proton-pump inhibitors 2.8 to 5.32, nasal steroids 2.44 to 8.16, statins 2.06 to 5.51, and serotonin receptor agonists 0.8 to 1.52). Quarterly savings through the four therapeutic interchange interventions combined exceeded $200,000 with an estimated annual savings for the health plan of $800,000, or more than $17 per member.Conclusion A therapeutic interchange clinical decision support tool at the point of prescribing resulted in increased compliance with recommendations for outpatient prescriptions while producing substantial cost savings to the Vanderbilt Employee Health Plan – $17.77 per member per year.Therapeutic interchange rules require rational targeting, appropriate governance, and vigilant content updates.Citation: Stenner SP, Chakravarthy R, Johnson KB, Miller WL, Olson J, Wickizer M, Johnson NN, Ohmer R, Uskavitch DR, Bernard GR, Neal EB, Lehmann CU. eprescribing: reducing costs through in-class therapeutic interchange.

Managed care is evolving in ways that pose unique ethical challenges to those interested in the intersection of clinical and organizational ethics. For example, Disease Management (DM) is a form of managed care that has emerged in response to chronic illness. DM is a healthcare management tool that coordinates resources across an entire health care delivery system and throughout the life cycle of chronic disease. Health Maintenance Organizations have reduced some costs in the delivery of acute care, but real cost savings will result only with greater efficiencies in the delivery of costly chronic care. DM is a systematic, population-based approach that identifies persons at risk of chronic ailment, intervenes with specific programs of care, measures clinical and other outcomes, and provides continuous quality improvement. Characterized as a movement to patient-driven services, DM involves a complex web of provider relations.
