Background and Objectives Vocal process granulomas (VPGs) are benign lesions of the larynx, typically contact granulomas (CG) and intubation granulomas (IG). The two diseases are known to have different clinical manifestations despite having the same pathological features. The purpose of this study was to analyze the treatment results for CG and IG and to obtain clinical information.Materials and Method We retrospectively reviewed the medical records of patients diagnosed with VPG between January 2015 and December 2018. The patient’s age, sex, medical history, lesion size, lesion type, reflux finding score, response to treatment, duration of treatment, and follow-up period were compared.Results In total, 32 patients were included in the study, of which 18 were CG and 14 were IG. In the CG group, males were dominant (n=15, 83.3%), whereas in the IG group, females were dominant (n=11, 78.6%) (p=0.0009). The response to medical treatment using proton pump inhibitor and steroid inhaler was better in the IG group (11/14, 78.6%) than in the CG group (7/18, 38.9%) (p=0.036). Of the 14 patients who did not respond to medical treatment, 5 received botulium toxin injections, and all 5 had complete remission. The duration of medical treatment was significantly longer in the IG group (p=0.0029).Conclusion IG was more common in female, and CG was more dominant in male. IG had better response to medical treatment using proton pump inhibitor and steroid inhaler than CG.

We describe a case of bilateral vocal process lesions in a 65-year-old man. His history was strongly suggestive of vocal process granulomas: previous gastroesophageal reflux, intubation, smoking, and oral squamous cell carcinoma. Medical management with a proton-pump inhibitor, reflux precautions, voice therapy, and adequate hydration yielded no results. Subsequent surgical intervention revealed that he had squamous papillomas. We also provide a brief review of vocal process granulomas and squamous papillomas.

Flat warts are mainly caused by HPV type 3 and 10. The condition is usually seen in young people in visible skin areas such as the back of hands, face, and forehead. Medical treatment may be prolonged in some situations for months and rarely for years making patients unsatisfactory Surgical approaches such as electrocoagulation, cryosurgery, and laser are used when medical treatment fails to control the disease, but surgical removal carries the risk of scar formation and is usually costly. The current study was planned and carried out to evaluate the response of plane warts to 5% imiquimod cream in a sample of Iraqi patients with plane warts. The present cohort study included 20 patients with plane warts. Those patients were selected from the pool of patients visiting the dermatology consultation unit at Al-Diwaniyah teaching hospital, Al-Diwaniyah Province, Iraq during the period from September 2018 through June 2019. Age, gender, and duration of disease were the main variables included in the study; the outcome was a response to treatment classified as complete, partial, and incomplete. All patients were treated by topically applied 5% imiquimod cream once daily and were followed up for a period of time, ranging from 2 weeks to 12 weeks. The results of the current study revealed that treatment duration and response to treatment is shown in table 2. Mean duration of treatment was 7.95 ±3.33 weeks, and it ranged from 2 to 12 weeks. Complete response was seen in 60 %, partial response was identified in 30 %, and no response was the outcome in 10 % of patients. Treatment response was not correlated to the age or gender of patients, but longer duration of disease predicted poor response to treatment.  In view of available data in the current study and in previous reports, 5% topical imiquimod cream appears to be an efficient and safe mode of treatment for flat warts.

Aim. Description of the endoscopic and clinical traits of heterotopic gastric mucosa (HGM) observed in cervical oesophagus.Key points. HGM in proximal oesophagus can be asymptomatic or have various clinical manifestations. A 40-yo female patient consulted a gastroenterologist with complaints of cough and globus sensation. For several years she was visiting an otorhinolaryngologist and psychotherapist, with therapy ineffective. Esophagogastroduodenoscopy (EGDS) at the last visit revealed several foci of HGM in cervical oesophagus of 1.2 x 0.8 cm maximal size. The patient was prescribed a combined prokinetic — proton pump inhibitor therapy, which relieved the symptoms. EGDS in a 21-yo patient without active complaints revealed a 2 cm-wide HGM of 4/5 cervical oesophageal lining with acidproducing zones.Conclusion. Two different scenarios of cervical oesophageal HGM are described, the first one manifested with laryngopharyngeal reflux, and the second devoid of clinical manifestations despite a large heterotopic site.

Introduction. The two compartments, disco-temporal and condilo-discal, which make up the temporo-mandibular joint, fulfill the functions of the stomatognathic system, namely: phonation, mastication and swallowing. The temporo-mandibular degenerative pathology is one of the 5 clinical entities given by the American Academy of Orofacial Pain in 1993. The symptoms are varied, dominated by pain, impaired joint mobility, cracks, sensation of muscle fatigue in the masseter and temporal muscles. Most cases benefit from conservative treatment, consisting of anti-allergic and anti-inflammatory medication, specific orthodontic treatment, electrotherapy (TENS, ultrasound, magnetodiaflux, laser), kinesiotherapy. 2-5% of patients require surgery. Material and method. We performed a retrospective study, over a 9-year period (January 2010-March 2019), in which we included 503 patients who went to the Timisoara Oro-Maxillofacial Surgery Clinic, with a definite diagnosis of temporomandibular algodisfunctional syndrome, of different etiologies, between the ages of 6 and 85 years. We evaluated the etiology of temporomandibular algodisfunctional syndrome, the prevalence of clinical manifestations in temporomandibular osteoarthritis and their evolution after 10 days of conservative treatment. Results and discussions. Pain is present in 100% of cases, the values obtained in these patients with temporomandibular osteoarthritis are between 10 and 40mm. Cracks are present in women in the percentage of 40.8% and 37.9% in men; mobility limitation is found in women in 11% of cases, and in men in 15% of cases. Conclusions. The existence of a protocol for the diagnosis of TM osteoarthritis is essential to establish the treatment plan and prognosis of the disease. The pain from osteoarthritis TM is low intensity, present in all patients; cracks and limiting mobility are present in comparable proportions, regardless of gender. The response to treatment was optimal at 21 days of treatment.

Within the framework of the educational and research project "Real Clinical Practice of Treatment of Acid-Dependent Diseases", H. pylori-associated pathology and their dynamics after the eradication the nature and severity of clinical manifestations were evaluated. In the analysis 1,474 patients were included aged 18 to 87 years old (average age 45.7 years) with confirmed H. pylori infection, who were observed on an outpatient basis in 188 doctors in Moscow, the Moscow Region and St. Petersburg. Diagnosis of H. pylori and subsequent eradication control were carried out with the use of methods available in real clinical practice. The severity of symptoms was assessed using “7×7” Questionnaire (seven symptoms for seven days). On the first visit, the average score was 12.5, on the second 4.5, and on the third 1.7 points. The significant relationship between smoking and the severity of dyspeptic manifestations was revealed. Rabeprazole was the most commonly prescribed proton pump inhibitor (84.3%) in the eradication regimens.
