Introduction. In order to provide the most favorable conditions for bone healing following implant loading, new techniques such as the use of growth factors to directly or indirectly regulate cell and tissue growth are becoming increasingly popular. Simplicity in producing concentrated thrombocytes derived from platelet-rich plasma has led to increasing application of growth factors derived from thrombocytes, such as PDGF, TGF-?, IGF and VEGF, in order to increase the percentage of bone-to-implant contact that provides better implant stability and possibly earlier functional loading. The aim of the study was to determine the effects of platelet-rich plasma and guided tissue regeneration on bone regeneration in the case of bone defects in early dental implant insertion. Materials and Methods. This experimental study was conducted on 10 study dogs that received 40 BCT implants, i.e., 4 implants per dog. The study included early dental implant placement in the sites of the extracted premolars. Artificial bone defects, resembling peri-implantitis defects, were made on the mesial side of each site, and the defects were filled using suitable guided tissue regeneration protocols with platelet-rich plasma derived from each experimental animal an hour before surgery. Histopathologic and histomorphometric evaluations were performed 10 months following the implantation. Results and Conclusion. The applied protocols had different effects on bone healing. According to the results obtained, the use of platelet-rich plasma combined with deproteinized bovine bone and bovine-derived resorptive membrane provided superior bone defect ossification

Background/Aim. Osseointegration is a result of cellular migration, differentiation, bone formation, and bone remodeling on the surface of an implant. Each of these processes depends on platelets and blood coagulum. Platelet-rich plasma (PRP) is used to improve osseointegration and stability of implants. The aim of the research was to test the influence that PRP and guided tissue regeneration in bone defects have on bone defect filling and the level of bone resorption in early implant insertion. Methods. This experimental study included 10 dogs. A total of 40 BCT implants were inserted, 4 in each dog (two on the left side and two on the right side), with guided tissue regeneration. Radiologic analyses were done immediately after the insertion and 10 weeks after the insertion. Bone defect filling was measured by a graduated probe 10 weeks after the implant insertion. The following protocols were tested: I - PRP in combination with bovine deproteinized bone (BDB) and resorptive membrane of bovine origin (RBDM), II - BDB + RBDM, III - PRP + RBDM and IV - RBDM. Results. The applied protocols affected differently the bone defect filling and the level of bone resorption. Significantly better results (the lowest bone resorption) were achieved with protocol I (PRP + BDB + RBDM) in comparison with protocols III (PRP + RBDM) and IV (RBDM), but not with protocol II (BDB + RBDM). On the other hand, no significant difference was found among protocols II (BDB + RBDM), III (PRP + RBDM) and IV (RBDM) in the level of bone tissue resorption. Conslusion. The bone defect filling was largest and the level of bone resorption was lowest in the protocol with PRP applied in combination with BDB and RBDM.

AbstractPlatelet-rich plasma (PRP) has been shown to be a beneficial growth factor for bone tissue healing and is used in implantology. The aim of this study was to investigate the effects of PRP on bone defects in rabbits. Twenty rabbits were used to establish the implant bone defect model in this study. An intrabony defect (5mm × 5mm × 3mm) was created in alveolar bone in the lower jar of each rabbit. The wound was treated with PRP. The expression of platelet-derived growth factor BB (PDGFBB) was assessed by enzyme-linked immunosorbent assay (ELISA). Focal adhesion kinase (FAK) and related phosphatidylinositol 3-kinase (PI3K)/AKT (protein kinase B) levels were measured by Western blot. The results show that PRP could significantly improve the bone healing process when compared with control, and 10% PRP could markedly increase fibroblast proliferation 48-h post treatment. PDGFBB was higher in the PRP group than that in the control group. PRP treatment also could elevate the phosphorylation of FAK and PI3K/AKT, although the inhibitor of PDGFR could reverse this trend. These results suggest that PRP treatment improves the bone healing process through the FAK/PI3K/AKT pathway.

Platelets release several growth factors which stimulate tissue regeneration. Several techniques for platelet concentrates such as platelet rich plasma (PRP), plasma rich in growth factors (PRGF), platelet rich fibrin (PRF) and concentrated growth factors (CGF) have been introduced in dental surgeries for the prevention of hemorrhage and acceleration of tissue regeneration. However, a fabricating growth factors-enriched bone graft matrix which is called “sticky bone” has been demonstrated to provide stabilization in bony defects. In this article, we presented the method of preparing and utilizing CGF and sticky bone and evaluate the effect of CGF mixed with bone graft and CGF barrier membrane in periodontal surgeries.Methodology: We used websites such as PubMed, Scopus, and ISI Web of knowledge to get related articles about this subject. The research process involved specific key words " concentrated growth factor”- “Platelet rich fibrin”- growth factors-enriched bone graft” - “sticky bone” to find more articles which published from 2007 to March 2018.Results: We reviewed 48 articles, 43 articles were excluded. Only five articles have been conducted. Original human studies and case report were included.Conclusion: We concluded that the use of sticky bone and CGF is effective in bone grafting and implant.

Latar belakang. Masalah pulpa dan periodontal menyebabkan lebih dari 50% kehilangan gigi. Kasus pada laporan kasus ini merupakan lesi primer endodontik dan lesi sekunder periodontal dan secara klinis terdapat sinus tract. Kondisi ini merupakan kasus yang secara kolaborasi dikerjakan oleh bidang konservasi gigi dan bidang periodonsia untukmemdapatkan hasil yang maksimal. Kasus. Terdapat tiga kasus lesi endodonti-periodontal, dua merupakan kasus (gigi 21 dan 37) yang setelah beberapa tahun dilakukan perawatan saluran akar (PSA) mengalami pembentukan sinus tract, dan satu kasus (gigi 47) yang setelah PSA namun tidak memeperlihatkan perbaikan sinus tract yang terbentuk di gingiva. Tindakan bedah flap periodontal dilakukan untuk mencari penyebab, menghilangkan jaringan granulasi, dan memperbaiki kerusakan tulang alveolar yang terjadi. Seluruh kasus terlihat terdapat kerusakan tulang anguler di daerah furkasi pada gigi posterior (37 dan 47), dan daerah interdental pada gigi anterior (21); satu kasus (gigi 21) mengalami fenestrasi di fasial. Defek tulang anguler dan daerah fenestrasi setelah dibersihkan dari jaringan granulasi yang terinfeksi, diisi dengan graf tulang dan ditutup oleh membran pericardium sebagai guided tissue regeneration (GTR), kemudian dijahit. Kontrol 14 hari setelah tindakan bedah, sinus tract  telah hilang dan warna gingiva normal. Kesimpulan. Lesi endodontik-periodontal yang memperlihatkan sinus tract yang persisten setelah perawatan saluran akar merupakan indikasi adanya kerusakan periodontal yang kompleks. Defek periodontal kompleks bisa diperbaiki dengan tindakan bedah regeneratif.Kata kunci. Lesi endodontik-periodontal, sinus tract, defek tulang anguler, fenestrasi Abstract Background. Dental pulp and periodontal problems account for more than 50% of tooth loss. The cases in this case report were primary endodontic lesions and secondary periodontal lesions and clinically contained a sinus tract. This report is a collaborative carried out by the conservative dentistry and periodontics to obtain maximum results. Case. There were three cases of endodontic-periodontal lesions, two were cases (teeth 21 and 37) where after several years of root canal treatment had sinus tract formation, and one case (tooth 47) after endodontic treatment but did not show any improvement in the sinus tract in the gingiva. Periodontal flap surgery is performed to find the cause, remove the granulation tissue, and repair the alveolar bone damage that has occurred. All cases showed angular bone defects in the furcation areas of the posterior teeth (37 and 47), and the interdental areas of the anterior teeth (21); one case (tooth 21) had facial fenestration. Angular bone defects and areas of fenestration after cleaning of infected granulation tissue, filled with bone graft and covered by pericardial membrane as guided tissue regeneration (GTR), then sutured. Control 14 days after surgery, the sinus tract was gone and the gingival color was normal. Conclusion. Endodontic-periodontal lesions showing persistent sinus tracts after root canal treatment are indicated of complex periodontal damage. Complex periodontal defects can be corrected with regenerative surgery.Keywords. endodontic-periodontal lesions, sinus tract, angular bone defect, fenestration
