The search for a harmonious and balanced face grows more and more and the demand for aesthetic treatments increases in the same proportion. Procedures that use hyaluronic acid as a filling material have high acceptance and rapid postoperative recovery, bringing excellent results. This article reports the clinical case of a young male patient, Pattern II, according Capelozza, due to mandibular retrognathism, whose main complaint was the recessed position of the chin. Restructuring with highly cross-linked hyaluronic acid in the affected region has been suggested. The remodeling of the chin together with its increase in volume through the application of hyaluronic acid was able to significantly improve the profile and the complaint-neckline of the patient.

Objective Current recommendations for cochlear hydrops treatment include systemic glucocorticoids and diuretics. Cochlear cells express dopamine receptors, although their role is unknown in the pathophysiology of cochlear hydrops. Case Description We report the case of remission of recurrent right-sided cochlear hydrops in a young male patient treated with bromocriptine due to pituitary macroprolactinoma. Transient improvement was observed after oral steroid and diuretic treatment, but cochlear hydrops recurred until the dose of bromocriptine was increased to 10 mg daily. Conclusion Bromocriptine may stimulate dopamine receptors in cochlear cells with potential therapeutic role in patients with cochlear hydrops. There are no widely accepted and effective treatments for endolymphatic hydrops, and identifying potential new and efficacious therapeutics is of high relevance.

SummarySevere acute respiratory syndrome coronavirus type 2 (SARS-CoV-2), the etiological agent of coronavirus disease 2019 (COVID-19), produces protean manifestations and causes indiscriminate havoc in multiple organ systems. This rapid and vast production of proinflammatory cytokines contributes to a condition termed cytokine storm. A 35-year-old, otherwise healthy, employed, male patient was tested positive for COVID-19. He was admitted to the hospital on disease day 10 due to retarded verbal reactions and progressive delirium. On account of these conditions and the need for noninvasive/invasive ventilation, a combination treatment with baricitinib and remdesivir in conjunction with standard of care was initiated. The cytokine storm was rapidly blocked, leading to a vast pulmonary recovery with retarded recovery of the central nervous system. We conclude that the rapid blockade of the COVID-19-induced cytokine storm should be considered of avail as a principle of careful decision-making for effective recovery.

Abstract Introduction Onycholysis is a chronic nail disorder characterised by the progressive separation of the nail plate from the underlying nail bed. Apart from causing pain and discomfort, it affects the quality of life of an individual due to its visualised cosmetic appearance. Case Summary A 45-year-old female presented with complete loss of nail plate of the left middle finger for 1 month. At the first visit, homoeopathic medicine Tuberculinum bovinum was prescribed and the patient was followed up periodically for 4 months. A series of photographs captured at the first visit and in subsequent visits objectively document the gradual regrowth of her nail plate. Various aspects of her subjective improvement were documented by three validated outcome measurement scales. The patient herself assessed that, after 4 months of follow-up, she is ‘back to normal’ for her main complaint as well as for the overall well-being, which is documented as a +4 score in ‘Outcome in Relation to Impact on Daily Living’ (ORIDL) scale. The Dermatology Life Quality Index (DLQI) score was reduced from 20 at the first visit to 1 after 4 months of treatment. Modified Naranjo Criteria for Homeopathy (MONARCH) score recorded at the final visit (+8 in ‘-6 to +13’ scale) is suggestive of a high likelihood that the improvement in the patient can be attributed to the homoeopathic treatment provided. Conclusion This clinical case report is presented here to critically document the effect of individualised homoeopathic treatment in onycholysis within a plausible timeframe.

Pregnancy in non-communicating rudimentary horn is extremely rare. It carries grave risk to mother and fetus due to rupture of rudimentary horn in second trimester of pregnancy. Here is a case of ruptured rudimentary horn pregnancy at gestation of 14 weeks. Laparotomy was done and the rudimentary horn was excised. Postoperative recovery was uneventful.   Keywords: Two unicornuate uteruses; rudimentary horn; rupture DOI: 10.3329/bmj.v39i1.6234 Bangladesh Medical Journal 2010; 39(1): 47-48
