The use of dental implants in dentistry is becoming increasingly widespread and popular for the treatment of patients with doomed dental elements. The biotechnology applied to the development of macro and micro design of implants allows its use in increasingly borderline beds, such as in post-extraction sockets with reduced bone remainder, and may even allow the making of provisionals in order to confer aesthetics and help in maintenance of support tissues. This paper aims to present a clinical case report of minimally invasive extraction of a maxillary central incisor with root fracture, immediate implantation in a post-extraction socket, subepithelial connective tissue graft and lyophilized bovine bone to fill the GAP, as a pillar installation. angled and immediate provisional. The technique proved to be effective and satisfactory, I try in the aesthetic aspect as to the functional, promoting the health and stability of the adjacent tissues.

ABSTRACT This article presents a clinical case of horizontal root fracture, in an adult patient, male, in the apical third, due to the practice of sports, which, along with other factors, has led to pulp necrosis. Initially, the patient sought emergency care, and then he underwent treatment by endodontics, using calcium hydroxide between sessions. After a monitoring period, root canal obturation was performed and the tooth is clinically and radiographically stable 2 years after the trauma. In this case, monitoring is long-lasting, divided into 12-month periods for 10 years, with clinical and radiographic evaluation. Dental fracture in children and teenagers from 10 to 14 years is very common, especially during the practice of sports and recreation activities. It is expected that young male individuals have greater propensity to damage their teeth than young female individuals. Maxillary central incisor is the tooth most commonly involved, and it often evolves to pulp necrosis after trauma.

The aim of this article is to present a clinical case report where a female patient, 62 years old, presented with element 14 poorly positioned and mobility. The initial planning was immediate implantation with immediate provisionalization. The case described was carried out with a minimally invasive extraction, implant placement of the 3.75 x 11.5 mm (Helix, GrandMorse, Acqua, Neodent®, Brazil), immediate loading with a universal healing abutment (3.3 x 2.5 mm, Neodent®, Brazil) and a provisional cylinder of the universal healing abutment click (3.3 x 6.0 mm, Neodent®, Brasil) to approach of capture from a denture teeth and thus, the cementation of provisional implant crown. For modification of the gingival area, a subepithelial connective tissue graft was performed through the tunneling technique to improve peri-implant tissue conditioning. A 6-month postoperative follow-up was performed, showing satisfactory clinical and radiographic conditions during the most important phase of healing, and finally, a final follow-up of 12 months with the definitive cemented prosthesis. According to the literature and clinical experience, rehabilitation with immediate implant placement is one of the most challenging procedures when associated with immediate loading, where many criteria must be evaluated before the surgical phase, it means, and excellent initial planning.

ABSTRACT Crown fractures of the anterior teeth are a common form of dental trauma mainly affecting children and adolescents. One of the options for managing crown fractures is the reattachment of the fractured fragment when the tooth fragment is available with minimal or no violation of the biological width. Reattachment of fractured fragments can provide good esthetics, as it maintains the tooth's original anatomic form, color, and surface texture. It also restores function and is a relatively simple procedure. This case report deals with the esthetic management of a crown-root fracture that was successfully treated with endodontic treatment followed by reattachment of fractured fragment with fiber post. Background: An attractive smile is often attributed to incisor alignment and proclined or retroclined incisors could contribute to a less aesthetic smile. Objectives: This study was aimed to evaluate the maxillary central incisor crown inclination, crown angulation and tooth size variation in aesthetic and unattractive smiles. Methods: This study was conducted among 100 females (equally divided into aesthetic and unaesthetic smile groups), aged between15 - 25 years, with Angle’s Class I normal occlusion and balanced profiles. Variations in maxillary central incisor crown inclination and angulation were measured on lateral cephalograms, photographs and the study models of the subjects. Variations in tooth morphology were also evaluated by measuring crown height and width in both groups. Mann-Whitney U test was performed to evaluate the difference between the aesthetic and unaesthetic smiles. P < 0.05 was considered as significant. Results: This study showed that incisor angulation was significantly higher in the unaesthetic group compared to the aesthetic group (P < 0.001). However, there was no statistically significant difference for the incisor inclination relative to the Palatal Plane between the aesthetic and unaesthetic smile groups (P = 0.086). In addition, there were significant differences between the aesthetic and the non-aesthetic groups regarding the central incisor height (P < 0.05). Conclusions: The findings of this study indicate that the possible variations in aesthetics are related to the ethnic elements that are associated with differences in crown angulations, as well as tooth size.

Management of traumatic injuries to the teeth is a challenge to the practicing dentist. It has no prescriptive method for occurring, possesses no significant predictable pattern of intensity or extensiveness and occurring at times when dentists are least prepared for it. Ninety-two percent of traumatic injuries results in fracture of maxillary permanent central incisors because of its protrusive and anterior positioning. The young permanent maxillary central incisor root canal chamber is large and tapered. This case report describes the management of Ellis Class III fractured tooth in a young permanent maxillary central incisor by an innovative clinical procedure.
