Abstract Introduction. Clinicians claim that the prevalence of hypodontia has skyrocketed in recent years. However, there is no clear evidence whether it is really a change in dentition in Homo Sapiens or a purely hypothetical observation, due to the advanced modern technology used in the diagnosis of dento-maxillary anomalies. The aim of this study was to establish the prevalence and distribution of nonsyndromic hypodontia in young patients from the South-East Transylvanian region of Romania. Material and methods. A number of 325 dental charts were selected. The patients who needed orthodontic treatment were treated in the Pedodontics Department and Orthodontic Department of UMFST “George Emil Palade“ Targu Mures and two private dental offices during 2017-2019. The age of the patients was between 12 and 25 at the moment when the clinical and paraclinical examination were performed. Results. From a total number of 275 cases investigated, 78 patients were diagnosed with hypodontia in permanent dentition, not taking into consideration the third molars. Hypodontia’s prevalence is 6.47% for patients which seek orthodontic treatment, without taking into consideration the third molars. This value is found in the range of 2.8%-11.3% reported in studies in the literature. The present study showed that hypodontia affects a greater proportion of females (6.83%) than males (5.74%), without statistically significant differences. Conclusions. The prevalence of non-syndromic hypodontia in permanent dentition, compared to the total number of patients who requested orthodontic treatment, is 6.47%, being higher than the data reported in the literature of our country, but falls within the range reported in the international literature in general.

Objective: Tooth agenesis is one of the most common congenital anomalies seen in humans. Although ab¬sence of one or more teeth is common, absence of multiple teeth is rare. Oligodontia is a rare developmental anomaly, involving agenesis of six or more permanent teeth, excluding the third molars. The reported preva¬lence of oligodontia in permanent dentition is 0.14%. Oligodontia can be presented as an isolated condition or as a part of a syndrome. Discussion: The present case report highlights a unique case of non-syndromic oligodontia, with agenesis of four permanent incisors, left permanent canine and right second premolar in the mandibular arch and its management with a novel fixed functional prosthetic appliance. Conclusion: Prosthetic rehabilitation is an urgent need for these kind of patients so that they do not suffer from mastica¬tory and esthetic problems which can eventually lower the self esteem of individuals.

Third molars are highly variable in their presence and form. This report focuses on a horizontally impacted third molar and analyzes the potential etiology of this situation. Upon a clinical and radiographic examination, it was noted that the patient had four third molars present. The patient’s third molars began erupting around the age of 19. Currently, they are asymptomatic with incipient caries on the occlusal surfaces. While three of the third molars erupted in a normal orientation, one of the third molars (mandibular left) erupted in an orientation that would be classified as horizontal and/or mesio-angular. No treatment has been undertaken at the moment, however surgical extraction can be recommended with the prognosis being very good. For the clinician that has to treat dental complications that arise from abnormal tooth eruption, as seen in numerous genetic and acquired disorders, knowledge about the basic molecular mechanisms involved may become extremely important.

The system suggests promising application prospect in future studies related to CBCT superimposition and evaluation for its feasibility and efficiency.Methods: Forty-nine maxillary third molars from 27 patients (mean age, 20.78 years) were included. CBCT images were obtained before and after orthodontic treatment with premolars extracted (mean treatment duration, 31.47 months). The changes in the position, angulation, and rotation of the third molars were evaluated with a space coordinate system using four landmarks: anterior nasal spine (ANS), posterior nasal spine (PNS), left and right orbitales.Results: After orthodontic treatment, the third molars moved forward (adjusted mean, 1.44 mm) (p < 0.001) and downward (adjusted mean, 2.87 mm) (p < 0.001) accompanied by outward rotation of the crowns (adjusted mean, 5.38°) (p = 0.001), while changes in angulation were insignificant.Conclusions: This was the first study to systematically investigate the spatial position change of maxillary third molars in adult patients who received orthodontic treatment with premolar extraction. During the process, maxillary third molars moved downward and forward accompanied by outward rotation of the crowns. Orthodontists should take tooth movement potential into consideration when making extraction plans.

ABSTRACT Objective: To evaluate the skill of orthodontists and oral/maxillofacial surgeons (OMFS) in providing a prognosis of mandibular third molars spontaneously erupted, through follow-up panoramic analysis. Methods: 22 orthodontic patients treated without extraction, presenting spontaneously erupted mandibular third molars (n = 44) were analyzed through panoramic serial radiographs. The first panoramic radiograph was obtained just after orthodontic treatment (PR1), in patients aging from 13 to 19 years. A second panoramic radiograph (PR2), was obtained in average two years later. The radiographs were randomly analyzed by 54 specialists, 27 orthodontists and 27 OMFS, to obtain the opinion about the approach to be adopted to these teeth in PR1. Then, another opinion was collected by adding a serial radiograph (PR1+2). Results: The concordance of the answers was moderate for OMFS (Kappa 0.44; p< 0.0001) and significant for orthodontists (Kappa 0.39; p< 0.0001). In the analysis of the first radiograph (PR1) of the spontaneously erupted molars, OMFS indicated extraction in 44.5% of cases, while orthodontists indicated in 42%, with no difference between groups (p= 0.22). In PR1+2 analysis, orthodontists maintained the same level of extraction indication (45.6%, p= 0.08), while surgeons indicated more extractions (63.2%, p< 0.0001). Conclusions: Orthodontists and OMFS were not able to predict the eruption of the third molars that have erupted spontaneously. Both indicated extractions around half of the third molars. A follow-up analysis, including one more radiograph, did not improve the accuracy of prognosis among orthodontists and worsened for OMFS.

Aims. To evaluate the role of third molars in the development of crowding or relapse after orthodontic treatment in the anterior segment of the dental arch.Methods. PubMed search of the literature was performed selecting all the articles relevant to the topic and limiting the studies to controlled trials on humans and written in English language. Systematic review was conducted according to the PRISMA (preferred reporting items for systematic reviews and meta-analyses) statement.Results. A total of 12 clinical studies were included in the review. A high risk of bias was found in most of the articles, either because the relative items assessed were inadequate or because they were unclearly described. The third molars were not correlated with more severe anterior tooth crowding in most of the studies. However, four of them described a different outcome.Conclusion. Definitive conclusions on the role of the third molars in the development of anterior tooth crowding cannot be drawn. A high risk of bias was found in most of the trials, and the outcomes were not consistent. However, most of the studies do not support a cause-and-effect relationship; therefore, third molar extraction to prevent anterior tooth crowding or postorthodontic relapse is not justified.

Oligodontia is a condition in which the patient has more than six ageneses, excluding the third molars. Whereas the absence of one tooth is quite common, oligodontia is a rare congenital condition that can happen with or without the syndrome. The condition happens due to disturbances during odontogenesis process. Management of oligodontia is a long-term process, and it involves prosthetic, restorative, and orthodontic treatment to support the oral function and esthetic of the patient. This article aims to report a rare case of agenesis of 13 permanent teeth (excluding third molars) in an 11-year-old girl and her treatment plan.

Orthodontic treatment, especially during the period of active growth, can significantly affect the development of the jaws and teeth. Treatment with extraction of molars is more complicated because the space that needs to be closed is larger than the premolar spaces and the time of treatment will be longer. However, in cases where we have a lack of space, teeth protrusion or dental asymmetry, and the extraction of permanent teeth is indicated, the damaged molars should be the first option for extraction when premolars are in better condition. The third molars in different ways are affected by orthodontic therapy. One of them is orthodontic treatment by applying the extraction of the third molars, this situation being widely explored. Extraction of third molar for orthodontic reasons is justified when it prevents the eruption of the second molar or otherwise affects the other teeth. The indications of extraction and selection of teeth to be removed depend on the specific aspects of malocclusion. Premolars are teeth that are usually extracted from orthodontic reasons, but this is not the rule and depends on individual treatment needs. The decision to correct the malocclusion with teeth extraction should be based on the ratio of the size of the dental arch with the size of the teeth, facial appearance and other factors arising from the diagnosis itself. Indications for extraction of the third molars during orthodontic treatment are: mandibular incisor crowding, if they in some way constitute a obstruction of orthodontic treatment as well as preparation for orthognathic surgery. The success of an orthodontic treatment involving the extraction of molars is directly related to the patient complaints and psychological profile, the absence of diseases and skeletal discrepancies, the professional experience as well as the fact whether the patient underwent previous orthodontic treatment with extraction. Patients with partially or completely non-erupted teeth are significantly more likely to have certain consequences: before, during or after the orthodontic treatment. Therefore, orthodontists need to raise patient awareness of these consequences in the context of orthodontic treatment.
