Este artigo buscou apresentar as estratégias de atuação fonoaudiológica e os cuidados descritos na literatura para o manejo da disfagia em pacientes acometidos de COVID-19. Trata-se de uma revisão integrativa da literatura realizada em setembro de 2020 nas bases de dados PubMed/Medline, Lilacs, Scopus e Web of Science, utilizando os descritores “COVID-19”, “SARS-Cov-2”, “therapy”, “speech therapy” e “deglutition disorders”, combinados por meio dos operadores AND e OR. Foram inseridos estudos que abordavam a temática de interesse, pesquisas em humanos com COVID-19; publicações disponíveis em português, inglês ou espanhol, estudos do tipo artigos originais, caso controle, transversais e estudos de caso. Foi realizada leitura dos títulos e resumos, excluindo aqueles que não se encaixavam na pesquisa. Foram selecionados 3 artigos, cujo resultados mostraram relação do surgimento da disfagia com a COVID-19, em decorrência da intubação orotraqueal e dos danos neurológicos causados. Os cuidados com os atendimentos e a necessidade de acompanhamento também foram abordados. Verificou-se literatura escassa em relação aos procedimentos de avaliação e estratégias de intervenção fonoaudiológicas, mas sugerem atividades de estimulação sensorial e que promovam o fortalecimento dos músculos envolvidos na deglutição, além de estratégias de cuidado que visem proteger o paciente e os profissionais.

Las salas de urgencias son espacios con protocolos coordinados y actuaciones diagnósticas y terapéuticas secuenciales. Los profesionales de rehabilitación en urgencias aportan a la toma de decisiones sobre conductas de hospitalización o egreso, planes caseros, ayudas diagnósticas y remisión a especialistas. El propósito de este análisis de literatura fue identificar evidencia internacional, sobre el fonoaudiólogo en sala de urgencias y sus acciones profesionales con pacientes, familiares y equipos de salud. Materiales y métodos. Se revisó evidencia de las bases de datos Ebsco, PubMed, Scielo, Elsevier y Web of Science, entre enero de 2015 hasta septiembre de 2020, con términos de búsqueda: speech language pathology, speech therapy, emergency room en español e inglés. Se seleccionaron documentos que informaran sobre procesos de rehabilitación en salas de urgencias, que incluyeran acciones desde fonoaudiología. Se analizaron 51 documentos según criterios de inclusión. La evidencia muestra la incursión del trabajo fonoaudiológico en un contexto no tradicional, motivado por las necesidades en la toma de decisiones clínicas en diálogo interdisciplinar, principalmente en condiciones de disfagia y alteraciones deglutorias. Las acciones profesionales en este contexto incluyen la promoción y prevención de ingresos a sala de urgencias, hasta la atención en condiciones críticas, agudas y al final de la vida. Este contexto representa retos profesionales hacia habilidades de juicio clínico, argumentación científico profesional, uso de instrumentos de detección y trabajo interdisciplinar. que hacen la diferencia entre aspectos como el egreso y la hospitalización, un tratamiento o procedimiento y en algunos casos, entre la vida y la muerte.

Background: Dysphagia causes severe complications among people with a stroke. Physiotherapy allows the cure of this pathology, and among the tools it offers is neuromuscular electrical stimulation. However, this is a technique that has not been protocolized. Therefore, it was considered necessary to carry out a systematic review on the efficacy of the various parameters of application of the neuromuscular electrical stimulation in dysphagia generated after a stroke. Methods: A systematic search for publications was conducted in March 2020 in the Pubmed, Cinahl, Medline, Web of Science and Scopus databases, using as search terms: Electric stimulation therapy, Deglutition disorders and Stroke. Results: 21 articles were obtained in which the application of neuromuscular electrical stimulation was applied in isolation (n = 7) or in combination with other techniques such as strengthening exercises and manual therapy techniques (n = 14), with this second modality of treatment having greater benefits for patients. Conclusion: The greatest efficacy of this technique is reached when applied at 60-80 Hz, 700 μs of pulse duration, at the motor intensity threshold and in sessions of 20–30 min.

Amyotrophic Lateral Sclerosis (ALS) is a degenerative disease that occurs with the deterioration of motor neurons. The beginning of clinical impairment may be bulbar with the average life time, after the first symptoms, between 2 to 5 years, presenting serious swallowing, speech and breathing disorders. Deglutition disorders can lead to malnutrition, dehydration, aspiration, displeasure, and more serious complications such as aspiration pneumonia and death. With the evolution of the disease, the patient needs procedures that generate doubts in the professionals and family, as the correct moment of indication of the use of alternative long-term feeding routes called gastrostomies (GTT). The objective of this article is to analyze the impact of dysphagias and the most favorable moment for the placement of gastrostomies. Early GTT placement may help prevent the patient from clinically debilitating more quickly, respond better to multidisciplinary team therapeutics, and feel more comfortable.Key-words: dysphagia, enteral nutrition, gastrostomy, deglutition disorders, motor neuron disease, amyotrophic lateral sclerosis.

RESUMOObjetivo: evidenciar a importância do cuidado integrado de Fonoaudiologia e Enfermagem ao cliente disfágico. Método: trata-se de estudo bibliográfico, descritivo, tipo revisão integrativa, com recorte temporal de 2013-2018, realizada na Scielo, BVS e Pubmed/Medline, durante o mês de março de 2018. Agruparam-se os resultados de acordo com o ano de publicação, país, autor, periódico e tipo de estudo. Resultados: compôs-se a amostra de 14 estudos, que referem que os cuidados de fonoaudiologia baseiam-se em minimizar as complicações oriundas da disfagia e as de enfermagem, confirmam seu potencial no rastreio e gerenciamento da disfagia. Conclusão: considerou-se que a integração dos cuidados de fonoaudiologia e enfermagem favorece a identificação precoce da disfagia e pode resultar no desenvolvimento de diretrizes e protocolos de apoio a esses profissionais. A statistical review provides analysis of four years of speech therapy services of a suburban school system which can be used for comparison with other school system programs. Included are data on the percentages of the school population enrolled in therapy, the categories of disabilities and the number of children in each category, the sex and grade-level distribution of those in therapy, and shifts in case-load selection. Factors affecting changes in case-load profiles are identified and discussed.

Purpose The purpose of this study was to explore the ways in which adolescent students who stutter perceive their school experiences. Method This study used a qualitative, phenomenological research design. Semistructured interviews were conducted with 7 adolescent students who stutter (3 in middle school and 4 in high school). Participants were interviewed about their school experiences, including the effects of stuttering on academics, learning, teacher relationships, peer relationships, speech therapy experiences, and self-image. Data analysis consisted of transcribing interviews and analyzing them for emerging themes. Results Findings revealed that participants described a variety of experiences around the school setting. Participants reported less favorable middle school experiences. Middle school participants reflected more on teasing, bullying, and feelings of embarrassment, whereas high school participants revealed that teachers, staff, and peers were receptive and accepting of them and their stuttering. All participants reported that their speech therapy helped with classroom participation. Conclusions As a result of the participants' varied experiences, it is important to listen to and incorporate the voices of students who stutter into school, classroom, and therapy decision-making practices.

Purpose The purpose of this study was to determine the extent to which school-age children with speech sound disorder (SSD) exhibit concomitant reading difficulties and examine the extent to which phonological processing and speech production abilities are associated with increased likelihood of reading risks. Method Data were obtained from 120 kindergarten, first-grade, and second-grade children who were in receipt of school-based speech therapy services. Children were categorized as being “at risk” for reading difficulties if standardized scores on a word decoding measure were 1 SD or more from the mean. The selected predictors of reading risk included children's rapid automatized naming ability, phonological awareness (PA), and accuracy of speech sound production. Results Descriptive results indicated that just over 25% of children receiving school-based speech therapy for an SSD exhibited concomitant deficits in word decoding and that those exhibiting risk at the beginning of the school year were likely to continue to be at risk at the end of the school year. Results from a hierarchical logistic regression suggested that, after accounting for children's age, general language abilities, and socioeconomic status, both PA and speech sound production abilities were significantly associated with the likelihood of being classified as at risk. Conclusions School-age children with SSD are at increased risk for reading difficulties that are likely to persist throughout an academic year. The severity of phonological deficits, reflected by PA and speech output, may be important indicators of subsequent reading problems.

Purpose This study investigated the extent to which speech-language pathologists (SLPs) facilitate parents' completion of homework activities for children with speech sound disorder (SSD). In addition, this study explored factors related to more consistent communication about homework completion and strategies considered particularly effective for supporting this element of parental involvement. Method Licensed SLPs serving at least one child with SSD were invited to participate in an online survey. Questions relevant to this study gathered information regarding (a) frequency of communication about homework distribution and follow-up, (b) demographic and workplace characteristics, and (c) an open-ended question about the specific strategies used to support parental involvement and completion of homework activities. Results Descriptive results indicated considerable variability with respect to how frequently SLPs engaged in communication about homework completion, but that school-based SLPs were significantly less likely to engage in this type of follow-up. Strategies considered effective, however, were similar across therapy contexts. Conclusion These results suggest potentially important differences between school-based services and therapy in other contexts with respect to this particular aspect of service provision for children with SSD.

Abstract Providing adequate speech therapy services to Latinos who stutter can present challenges that are not obvious to the practicing clinician. This article addresses cultural, religious, and foreign language concerns to the therapeutic relationship between the Latino client and the clinician. Suggestions are made for building cross-cultural connections with clients and incorporating the family into a collaborative partnership with the service provider.
