Basaloid follicular hamartoma (BFH) is a rare benign follicular tumor with varied clinical presentation. A 64-year-old male presented with a gradually enlarging plaque on the scalp of 10 years duration, the surface of which showed a unique cobblestone like appearance and dilated follicular openings discharging keratinous material. Histology was initially reported as trichofolliculoma. As the clinical picture was not suggestive of trichofolliculoma, the histopathology was reviewed. A final diagnosis of BFH was made based on the characteristic features such as distorted hair follicles with peripheral extensions of basaloid cells in a branching and anastomosing pattern. A periodic follow-up of such lesions is essential due to the risk of malignant transformation to basal cell carcinoma.

Background. We report a case of multiple keratocysts first diagnosed in an 8-year-old boy. Case report. The incidental radiographic finding of a cystic lesion in an 8-year-old boy led to the surgical enucleation and further diagnosis of a keratocyst associated with a tooth crown. In the course of dental maturation from deciduous to permanent teeth, the boy presented new lesions, always associated with the crowns of teeth. Gorlin-Goltz (nevoid basal-cell carcinoma) syndrome was suspected, and the genetic analysis detected a previously undescribed germline variant in the PTCH1 gene. Treatment. This included a surgical removal of the cystic lesions, as well as the affected teeth. Follow-up. Due to the high recurrence rate of the keratocysts, frequent radiological checks were performed over a 5-year period.

Skin cancer has emerged as a major problem for light-skinned people globally. Basal cell carcinoma (BCC) is one of the most common forms of skin cancer. BCC is associated with significant morbidity. There are multiple histological patterns of BCC, wherein the adenoid cystic variant is a rare form. A 75-year-old female with a history of breast carcinoma visited our centre for a routine follow-up. The patient was diagnosed with invasive breast carcinoma and underwent left breast conservation surgery in August 2018. At follow-up, the patient complained of itchiness, redness, and ulceration over a long-standing mole located at the right lumbar region. The lesion was excised and histopathologically diagnosed as the adenoid cystic variant of BCC. Adenoid cystic variant of BCC is an uncommon presentation. Identifying the mole in the lumbar region with clinical signs and symptoms was an incidental finding. In most cases, skin moles are benign. However, this case is of considerable interest as the patient presented with two primary cancers of different pathological characteristics within 3 months. The patient is currently doing well and is due for follow-up.

Abstract The characteristic features of central nervous system involvement in the nevoid basal cell carcinoma syndrome are reviewed. Emphasis is given to the frequent association of medulloblastoma with the syndrome.
