The article presents description of our own observation of a clinical case of severe viral infection caused by SARS- CoV-2 complicated by bilateral polysegmental pneumonia, complete bronchocutaneous fistula in a patient born in 1961. Brief information about the features of pathogenesis, clinical presentation, differential diagnosis and surgical treatment of pulmonary complications of this pathology according to modern literary data is presented. The study of the given clinical case indicates that the most important component of differential diagnosis of lung lesions with SARS-CoV-2 virus is laboratory diagnosis and high-resolution multispiral computed tomography. Pneumothorax and pyopnevothorax are a severe complication of COVID-19. However, in persistent bronchopleural fistula, it is necessary to continue active surgical treatment when it is clinically possible, performing minimally traumatic interventions.

Introduction: Autoimmunity is a condition in which the body's own cells which are immunocompetent and the antibodies, acts against its own self-antigen which will result in structural and functional damage to the body. The diseases caused by this phenomenon are called Autoimmune diseases. Autoimmune diseases are more commonly seen in females. Hundred thousands of individuals in the Western countries are affected. The diagnosis is made from the clinical presentation of the patient with which a differential diagnosis is made, following which various tests both phenotyping methods and genotyping methods are carried out to conclude the nal diagnosis. Conclusion: The genotyping methods play the most important role in the laboratory diagnosis of systemic immune diseases.

A 7-year-old intact male Rottweiler dog was evaluated for recurrent dysphagia and regurgitation. Physical examination was unremarkable and routine blood works were within normal limits. Computed tomography revealed a defined lesion in the caudal mediastinum arising from the oesophagus. The lesion was excised using intercostal thoracotomy and the histological diagnosis was oesophageal duplication cyst. The dog recovered uneventfully and at a 3-year follow-up no clinical signs were reported. Although extremely rare, oesophageal duplication cysts should be considered in the differential diagnosis in cases of chronic regurgitation and dysphagia associated with evidence of an oesophageal lesion.

Aneurysmal bone cyst (ABC) and telangiectatic osteosarcoma (TOS) share a lot of similarities in terms of the initial clinical presentation, the radiological semiotics and even the morphological pattern, however, they are completely different nosological entities requiring absolutely different treatment approaches. This article reports a clinical case of TOS of the femur which developed 1.5 years after the treatment of ABC of the same location and presents an analysis of literature on the differential diagnosis of these diseases, the modern concept of the pathogenesis of ABC and the possibility of the transformation of ABC into TOS. The patient's parents gave consent to the use of their child's data, including photographs, for research purposes and in publications.

According to evidence in the literature occurrence of the Hippel Lindau disease changes from 1: 33000 - 1: 36000 (population frequency) to 1 in 39000 live births. Up to 20% of them have not family history. Hereditary predisposition of most patients with systemic angioreticulomatosis has patterns and attributes. Diversity of forms and infrequent cases of the Hippel Lindau disease complicate timely diagnosis. Untimely control of condition of patient with the Hippel Lindau disease increases the probability of complications in surgical treatment. It can be the reason of early disability too. The presented clinical case reflects the need for a multidisciplinary approach to the management of patients with von Hippel-Lindau syndrome.

Rapid evolution of pulmonary complications associated with severe COVID-19 pneumonia often pose a management challenge to clinicians especially in the critical care setting. Serial chest imaging enable clinicians to better monitor disease progression and identify potential complications early which may decrease the mortality and morbidity associated with COVID-19. We report a case of severe COVID-19 pneumonia in a 69-year-old man that presented to University Malaya Medical Centre in March 2020 with multiple pulmonary complications including lung cavitation, bronchopleural fistula, pneumothorax, pneumomediastinum, subcutaneous emphysema and acute pulmonary embolism which we highlight through serial chest radiographs (CXR) and computed tomography (CT). The patient unfortunately succumbed to his disease one month after admission. COVID-19 patients may develop pulmonary complications due to a combination of direct viral lung damage, hypoxemia and high stress ventilation. Awareness of COVID-19 complications can prompt early diagnosis and timely management to reduce morbidity and mortality. Keywords: COVID-19 case report, lung cavitation, bronchopleural fistula, pneumothorax, pneumomediastinum

Acute isolated sphenoid sinusitis is a rare and is seen in only 3% of all cases of all sinusitis. It is frequently misdiagnosed because of its vague clinical presentation. We report a case of 35year old female who presented to our emergency department with complaints of painless rapidly developing bilateral vision loss. Non contrast computed tomography of paranasal sinuses showed only mild opacification of sphenoid sinus. Endoscopic sphenoidectomy was performed within 4 hrs of presentation. After 48 hrs of surgery the patient vision returned to 6/6 bilaterally. Acute sphenoid sinusitis should be considered in the differential diagnosis of acute vision loss. Awareness, early diagnosis, astute clinical sense and emergent intervention can prevent permanent complication.DOI: http://dx.doi.org/10.3329/jom.v15i2.20700 J MEDICINE 2014; 15 : 146-148

Relevance. Pulmonary sequestrations are rare pathology and make from 0.15 to 6.4 % of all congenital abnormality. Thus intra-abdominal localization of this malformation is described only in 2.5 % of all pulmonary sequestrations. Objective. Reflect the difficulty of differential diagnosis of rare lung malformation in newborns. Materials and methods. 2 cases of the extralobar sequestration located under the diaphragm diagnosed for newborns in a children's hospital for the last 20 years are in details described. Results. The diagnosis of an intra-abdominal extralobular pulmonary sequestration was finalized only after surgical treatment, confirmed histologically. Questions of differential diagnostics of rare congenital pulmonary malformations are discussed with tumors of suprarenal localization at newborns, comparison of own results to data of literature. Conclusions. In the differential diagnosis of suprarenal lesions at newborns it is necessary to consider the possibility of intra-abdominal extralobar pulmonary sequestration, especially in the localization of pathological changes at the left side.

The development and wide use of instrumental investigational methods, including multispiral computed tomography, computed tomography with angiography, duplex scanning of blood vessels, subtraction angiography, enabled diagnosis of rare diseases, in particular, of the intrahepatic arterioportal fistulae (IAPF). Methods of surgical treatment of this pathology include both open surgery and endovascular treatment with the use of embolizing implants or coils. The article presents a clinical case of diagnosis and endovascular treatment of IAPF in a 33‑year‑old patient, hospitalized in the Center of Interventional Neuroradiology at Feofaniya Clinical Hospital. The choice of the surgical treatment was based on the account of indications and contraindications to the intervention. The endovascular method was used: exclusion of the intrahepatic arterioportal fistulae by means of implantation of micro‑coils Nester 20 мм, MReye 20 mm, Tornado 10–4 mm. Positive dynamics was observed in the long‑term postoperative period (after 3 months), the control was performed based on such criteria as complaints’ absence, normalization of laboratory parameters, positive dynamics of the data of control hepatic angiography. IAPF elimination promoted the reduction of portal hypertension, and improvement of functional liver state. Endovascular method of treatment of patients with arterioportal fistulae is the method of choice at this pathology. The advantages of endovascular method include the minimization of intraoperative and postoperative complications, the shorter postoperative rehabilitation period. In case of the necessity for other method of surgical treatment, one should follow the indications and contraindications to the surgical intervention.
