In Kerala, the disturbing trend is that the public health care system is getting alienated from the people since 1980’s. About 30% of the lower income families seeks medical service from the government hospitals. This is because of the fall in the quality of the services of the government hospitals. In the present situation, the rate of utilization of the private sector can be increased drastically pointing to the poor performance of the public health care system. The government hospital has some problems like poor physical or infrastructure facilities, ineffective leadership and unsatisfactory supply of drugs and medical supplies faculty of staffing procedure. These above stated problems do not exist in private hospitals. Therefore, the present study carried out to assess the healthcare expenditure of government and private hospitals patients in Kannur district. The study was conducted during 2015-16. The sample size of the survey contains a total of 120 respondents from Kannur district. The study analyses the interrelationship between health care expenditure and major socioeconomic factors such as monthly income, age, gender, marital status and occupation. The health care expenditure divided into two-direct and indirect health care cost. The direct health care cost includes- user fee charge, medical charge, diagnostic charge and surgical cost. The indirect health care cost comprises of transportation charge, food and bevarages charge and accommodation charges. The study found that the direct cost of health care is high in both private and public sector hospitals. Finally, the study suggests that an initiative along the role of government is requested to secure the health demands of poor as health care costs are growing over time.

AbstractThe purpose of this research paper was to explore how HIV/AIDS manipulate nurses’ perception of their workload and pay in the era of HIV/AIDS in Gaborone and the surrounding areas of Botswana. The health care sector in Botswana is overwhelmed by HIV/AIDS, and this takes a toll on the nurses because they are the ones at the forefront in the fight against this demanding and risky illness at the workplace. The focus in this study was on the workload and pay in the era of HIV/AIDS. The general picture that emerges from the current study is that nurses are dissatisfied with their pay and to some extent the workload and this supports some of the earlier studies which reinforce their importance in the workplace. The study used both primary and secondary sources of information. For the purpose of this study convenience sampling was used. A questionnaire was used for data collection. The study adapted Index of Organizational Reactions (IOR). The findings of the current study will help the government to design strategies that will increase the level of job satisfaction among the nurses in the public health care sector of Botswana.

One of the constitutional principles of the public health care system in Brazil is universality, which turns health into a fundamental right and ensures that all citizens shall have access to health service whenever required. The purpose of this study is to assess the positioning of ambulances in Duque de Caxias-RJ, and find new arrangements to maximize the covered population. The configuration of a network that provides such service is indeed significant since small deviations may lead to users’ death. Therefore, four scenarios were built in order to represent different network arrangements, according to the manager’s strategy or the budget limitations of the city.  An Integer Programming model for servers’ positioning was used in each scenario. Indicators such as percentage of coverage population and total cost were then used to compare and choose the best solution. Results have shown that the current coverage could be doubled by just relocating facilities that already exist, without adding any costs. It is important to notice that this solution is rather different from the current positioning.

Background:Voluntary doula care, as a complement to midwifery care, is emerging as support for women and their families throughout the childbearing period. Doula care is not an official part of the public health care system in Nordic countries.Aim:The aim was to describe and interpret women’s experiences of becoming voluntary unpaid doulas and to generate a model as part of a research program.Method:Latent qualitative content analysis was chosen using interviews with nine voluntary doulas in an NGO group in Finland.Findings:Becoming a voluntary doula seems to be related to two motives, personal and caring. Interest in and fascination with childbearing, doula care as rewarding and joyous but challenging work, and networking and affinity in the doula group are personal motives. The caring motives were seeing doula care as important for the childbearing woman and her partner, being humble about childbearing knowledge, understanding vulnerability and uniqueness in childbearing based on their own experiences, and seeing doula care as a complement to midwifery care.Conclusion:A model has been created suggesting that the balance between the two motives might be crucial for doula care. Ethical reflections on the motives for becoming and being a doula need to be supported in a caring culture where the primary focus is the best care for the woman and her partner in childbearing.
