Objective: To observe the results of syndromic management in women living in IDPs camps complaining of chronic vaginal discharge. Design: Descriptive study. Setting: Medical Camps at Larkana set by Chandka Medical College Hospital for Internally Displaced Persons (IDPs) due to floods. Period: 1st September 2010 to 31st December 2010. Material and Methods: Total 200 symptomatic patients aged from 20 to 50 years suffering from chronic vaginal discharge having history of more than 6 months duration were included in the study. Asymptomatic as well as pregnant women and patients with abnormal cervix and having abnormal growth on cervix were excluded from the study. A detailed history and examination (including speculum and vaginal) was done and a proforma was filled. All these patients were given empirical treatment recommended by WHO as syndromic management consisting of stat doses of antifungal along with antibiotics, where no laboratory tests are required before treatment. Results: Next to vaginal discharge which was main symptom in all patients, the other symptoms like dusparunia, dysuria, itching ,lower abdomen pain and low backache was reported 9%, 16%, 20%, 24% and 31% respectively. Also 8% patients reported post coital bleeding. All patients were married and the mean age of the patients was 28+0.2 years and 15% of them were over 40 years. Mean parity was 4±1.Vaginal infection improved in 65% of the patients excellently with a first line single course of antibiotic and percentage raised up to 88% with second course. 19(9.5%) patients couldn’t be followed as they left that camp and 5(2.5%) patients who did not improve with two courses of antibiotics had big cervical erosions, referred to nearby tertiary care hospital for further management. Conclusions: IDPs live in poor conditions in camps without basic facilities and where it is difficult to perform bedside tests like microscopy, Potassium Hydroxide, wet mount films and tests for Sexually transmitted diseases like Chlamydia and gonorrhea are not available, syndromic management there is a rational way of treating cases of chronic vaginal discharge to get quicker response in such desperate women.

Malaria is a public health problem in 90 countries around the world affecting 300 million people and responsible for about 1 million deaths annually. Bangladesh is considered as one of the malaria endemic countries in Asia. Every year large number of people suffered for malaria. But there is little studies about clinico-epidemiology of malaria. Aim: To study the epidemiological and clinical aspects of malaria. Methods: This is a Prospective observational study that was conducted in all medicine unit of Chittagong Medical College Hospital, Chittagong, Bangladesh during August 2017 to June 2018. Total 55 patients were included in the study having malaria diagnosed by blood slide examination or rapid diagnostic test. Patients were enrolled in this study after getting written informed consent from the patient or attendant. Detail demographic and clinical data were recorded in structured case report form. Patients were regularly followed up and outcome recorded. Results: Results showed males (65.5%) of 25±15.109 years of age were the main sufferer. Majority (45%) came from low socio-economic condition (<5000 taka/month). 65.5% patients denied any history of recent travel to malarias’ area. 69.1% cases give history of using mosquito net, but only 25.5% have insecticide treated mosquito net. Majority (89.1%) were diagnosed as severe malaria and only few (10.9%) as uncomplicated malaria. Most of the diagnosis done at Chittagong Medical College Hospital, mainly presenting with coma or altered consciousness and convulsion. At field level diagnostic test done in 67.27% cases, of which RDT in 14.5%, BSE in 45.5% and both in 7.3% cases. 78.2% cases outcome were good with improvement and death occurred in 21.8% cases, mainly due to acute renal failure. Conclusion: Though we are making significant effort to control malaria, still we have to improve in controlling malaria based on both preventing the infection and on prompt effective treatment of the infection and illness when it does occur.

Background: Skin diseases account for a higher proportion of outpatient department in developing countries including Bangladesh.Objective: The purpose of the present study was to evaluate the pattern of skin diseases in patients attending OPD of Dermatology and venereology in a tertiary care hospital.Methodology: This cross-sectional study was conducted in the outpatient department of medical college hospital of Kishoreganj in Bangladesh from 2013 to 2016 for a period of four (4) years. All the patients at any age and sex who were attended in the OPD of the hospital selected as study population. Only newly diagnosed cases were included in the study. The socio-demographic profiles and diseases pattern were recorded in a data sheet. Diagnosis was made on clinical basis by expert dermatologist. Lab investigations were restricted to the cases where it carried diagnostic importance.Result: A total number of 119228 patients (near about 82/day) were recruited for this study. Out of them 57825 (48.5%) cases were male and 61403 (51.5%) cases were female. Infective diseases were more common than non-infective diseases which were (65409; 54.9%) and (53819; 45.1%) cases respectively. In cases of infective diseases; parasitic, fungal, bacterial, viral, sexually transmitted infection and other nonspecific infections were 20408(31.2%), 17464(26.7%), 12166(18.6%), 8503(13.0%), 3597(5.5%) and 3271(5.0%) respectively. Out of 20408 parasitic diseases of which 20061(98.3%) cases were scabies and 1347(1.7%) cases were pediculosis. Out of 17464 fungal diseases tinea capitis, tinea corporis, tinea cruris, candidiasis, onychomycosis and pityriasis versicolor were found in 2655(15.2%) cases, 3737(21.4%) cases, 3423(19.6%) cases, 3127(17.9%) cases, 1031(5.9%) cases and 3492(20.0%) cases respectively. Out of 12166 bacterial diseases Impetigo was found in 4745(39.0%) cases, pyoderma in 4282(35.2%) cases, folliculitis in 3103(25.5%) cases, leprosy in 24(0.2%) cases and skin TB in 12(0.1%) cases. Among the viral diseases verruca was found in 4056(47.7%) cases. In case of non-infective diseases, eczema, papulo-squamous disease, acne, urticarial, drug reaction, vitiligo, chronic arsenicosis, neoplastic skin disorder, genodermatoses and others were 16953(31.5%), 9903( 18.4%),7696( 14.3%), 8019( 14.9%), 4682( 8.7%), 2099( 3.9%), 269(0.5%), 161(0.3%), 431(0.8%) and 3606( 6.7%) respectively.Conclusion: In conclusion, we found that a huge burden of skin diseases are present in the OPD. Infective skin diseases mostly bacterial and parasitic still predominant in Bangladesh though it is less frequent in developed countries.Journal of National Institute of Neurosciences Bangladesh, 2018;4(2): 116-122

Background: Pleural effusion is a problem commonly encountered by chest physicians. Objective: This study was an attempt to know the clinical presentation in order to avoid delay in diagnosis that may influence treatment and outcome. Methods: This cross-sectional study of thirty (30) admitted cases with pleural effusion were confirmed by chest radiography and aspiration of pleural fluid from one (1) year to twelve (12) years age of either sex the patientdwere selected purposively, was studied from July 2009 to Feb 2010 in the Department of Pediatrics, Rajshahi Medical College Hospital . All information were recorded in pre tested semi structured questionnaire. Results: Positivity was higher in male children, (66.7%). Completely immunized were 56.7%. One third of cases were severely malnourished. History of respiratory distress & fever was present in 96.7%, cough in 90%. All cases had diminished chest movement, sub costal recession, and diminished breath sound on the affected side. Lobar consolidation was observed in 33.3% cases, patchy opacities in 53.3% cases. Fluid levels were observed in 76.7% cases. Color of pleural fluid was straw in 56.7%, clear in 30.0%, blood stained in 13.3%. Background: Vaginal discharge is a common gynaecological problem worldwide. It is a common presentation of different gynaecological diseases. Objective: The aim of the study was to find out the aetiological factors responsible for vaginal discharge among the women presented with cervical pathology. Methodology: This cross sectional study was carried out in patients complaining of vaginal discharge attending at Gynaecology department (GOPD) in Dhaka Medical College Hospital (DMCH), Dhaka from March'2006 to October'2006. Required tests were carried out in the Department of Microbiology and Department of Pathology at Dhaka Medical College, Dhaka. Data were obtained by history taking, physical examination and relevant investigations. Results: Cervical pathology was associated in 64.0% of patients complaining of vaginal discharge. Out of these, most common are cervicitis (48.0%) carcinoma of cervix (8.0%) cervical erosion (4.0%) endocervical polyp (2.0%) and old cervical tear (2.0%). Other associated aetiologies of abnormal vaginal discharge were bacterial vaginosis (12.0%) candidiasis (10.0%) trichomoniasis (10.0%). Conclusion: Vaginal discharge is a manifestation of many cervical pathology including carcinoma. Therefore, proper evaluation is needed in all patients complaining of vaginal discharge before treatment. Background: Children suffering from cancer are more vulnerable than others. This study performed to outline the clinical characteristics and outcome of children with cancer with severe acute respiratory syndrome corona virus 2 (SARS-CoV-2) infection in Dhaka Medical College Hospital, Dhaka. Methods: Clinical data and epidemiologic history of 24 children with cancer with laboratoryconfirmed COVID-19 infection by real-time reverse transcription-PCR (RNA-PCR) were admitted in the Dhaka Medical College Hospital, Dhaka from May, 2020 to June, 2020. Clinical data and epidemiologic history of these patients were restrospectively collected and analyzed. Results: Among the 24 cases, 16 (67%)were males and 8 (33%) were females. The median age was 5 years (range 0.11–12 years). The most common symptoms were fever (11 cases, 45%) and runny nose (8 cases, 33%), cough (8 cases, 33%), 6 (24%) were asymptomatic. Acute Lymphoblastic Leukemia were the most common (50%) and second most were Wilms tumor (17%). Among the 24 cases, on admission, 17 (71%) had normal white blood cell counts, while only 2 (8%) more than 10x109/L and 5 (21%) less than 4x109/L, respectively. 16 cases (67 %) had normal neutrophil count and 7 cases (29%) had neutropenia. Lymphocyte counts were normal in 50% cases, lymphocytopenia in 10 (42%). C Reactive protein and serum ferritin raised in 19(79%) cases, D-dimer raised, prothrombin time, activated partial thromboplastin time raised in 8(33%) cases. X-ray chest was abnormal in 6 cases (25%). 16 cases (66%) received intravenous antibiotics, 5 cases (21%) needed oxygen therapy and 4 cases (17%) needed corticosteroid. Ninteen (79%) patients were discharged from hospital, 3cases (13%) were discharged on request and death was 2 cases (8%). Conclusions: Children at all ages appeared susceptible to COVID-19 and there was significant gender difference. Clinical manifestations of children’s COVID-19 cases were generally less severe than those of adult patients. Death rate is more in children with cancer than others. Background: Patient's satisfaction over the health care services is very important issue in Bangladesh.Objective: The purpose of present study was to find out the level of satisfaction of women attending in the OPD of Obstetrics and Gynecology department at a tertiary care hospital in Dhaka.Methodology: This descriptive type of cross sectional study was carried out in the OPD of Obstetrics and Gynecology department at Dhaka Medical College from September 2011 to October 2011 for a period of one month. All the women attending in Obstetrics and Gynecology department in outdoor service at Dhaka Medical College Hospital, Dhaka with the age group of 22 to 40 years were selected as study population. A purposive sampling method was used to collect the data.Results: A total number of 200 cases were taken for this study. Highly satisfied over the services delivered by the doctors was reported in 135 (65.8%) cases and service of nurses was satisfied 111(55.8%) of cases. Satisfactory waiting time (two hours) was 143(7l.9%) of cases and the satisfactory hospital environment was 127 (63.8%) of cases.Conclusion: In conclusion most of the women are satisfied to the treatment delivered by the doctors at the OPD of a tertiary care hospital.J Shaheed Suhrawardy Med Coll, 2014; 6(2):79-81

Background: Vesico-vaginal fistula can occur in different women with varied socio-economic condition. Objectives: The purpose of the present study was to see the socio-demographic characteristics of vesico-vaginal fistula (VVF) patients attended at a tertiary care hospital in Bangladesh. Methodology: This cross-sectional study was carried out from July 2013 to December 2013 for a period of 6 months in the National Fistula Centre in the Department of Obstetrics & Gynaecology at Dhaka Medical College Hospital (DMCH), Dhaka, Bangladesh. All patients who underwent surgical repair for iatrogenic VVF in National Fistula Centre of the department of Obstetrics and Gynaecology of Dhaka Medical College Hospital were included in this study. Patients who got themselves admitted to Obstetrics & Gynaecology department of DMCH with the complaints of fistula. The entire selected patients were interviewed for detailed socio-demographic characteristics. Result: A total number of 51 cases of VVF were recruited for this study. The mean age was 46.02 (±SD 6.104) years. Most of the respondents were illiterate (55.0%) and one-third patients had primary level education. The number of highly educated patients was scarce (12.0%). It was evident that average age at marriage of the patients was 15.8(±4.74) years. Some females were forced to accept marriage at the age of 10 years. The mean interval between initiation of menstruation and the marriage was only 4.72 years. Mothers were on an average 17.48 years old at the time of first delivery. Conclusion: In conclusion middle age illiterate women are most commonly suffering from VVF. Background: Cough variant asthma (CVA) is a subset of asthma where the only symptom is chronic persistent cough. Many cases go unrecognized due to lack of proper evaluation. Response to asthma medication with features supportive of airway hypersensitivity helps in management of this disease. Objective: To find out the proportion of cough variant asthma among the patients attending medicine outpatient department of Enam Medical College, Savar, Dhaka. Materials and Methods: This cross sectional study was conducted in Enam Medical College Hospital, Savar, Dhaka over a period of two years from July 2009 to July 2011. Cough variant asthma was diagnosed mainly on clinical ground as chronic cough without wheezing, fever, weight loss, shortness of breath or sputum or any other apparent cause that persisted for more than eight weeks with absolutely normal physical examination of chest, normal chest radiography and blood count except raised eosinophil count and IgE level. Patients who met these criteria were given 2 weeks course of inhaler beclomethasone propionate and were assessed for improvement. Those who improved after steroid inhalation were categorised as having cough variant asthma. Results: Out of purposively selected 148 patients complaining only of chronic dry cough for more than eight weeks, 92 patients met the primary selection criteria for cough variant asthma. These 92 patients were given 2 weeks trial of 250 ìgm beclomethasone inhalation twice daily. Seventy nine patients reported almost complete recovery from chronic cough after 2 weeks and were categorized as having CVA. Thirteen patients did not improve and were not categorized as CVA. Conclusion: These findings suggest that cough variant asthma is the most common among the patients with chronic cough not due to any apparent cause. The efficacy of inhaled corticosteroid suggests that early intervention is effective in the treatment of this disease. Dengue is a rapidly spreading mosquito-borne viral disease worldwide. The endemicity in Bangladesh is also increasing gradually. The study was a prospective observational one, documented the presenting features and outcome of management. It was carried out in Department of Medicine in Holy Family Red Crescent Medical College Hospital, Dhaka from June 2013 to December 2013. Total 100 admitted cases of both sera positive and sera-negative were included in this study. Detailed history was taken, clinical examination and relevant investigations were done. Out of 100 patients 54 (54%) were male and 46 (46%) were female. So male to female ratio was 1.7:1. The age of the patients ranged from 12-75 years. Among them 20-40 years age group was highest 63%. Patients of higher socio-economic group were (69%) more affected. Out of 100 cases 54 had classical dengue fever (DF), 46 had dengue hemorrhagic fever (DHF). Antibody was positive in 81% cases. All patients presented with high fever, headache in 90%, retro-orbital pain in 45%, body ache in 56%, and backache in 48%. Leucopenia found in 60, platelet count <100x1091L was in 57, HCT- normal 17, <20% rise in 48%, ?20% rise in 28%. With proper management all patients were recovered.
