Background: Obesity and decreased physical activity are the most important factors in the development of type 2 diabetes, which in recent decades has led to an increase in the number of people with this disease. The aim of this study was to investigate the impact of circuit resistance training (CRT) on serum glucose, insulin resistance and health related physical fitness in elderly men with type 2 diabetes. Material and methods: Twenty-two patients with type 2 diabetes (60.99 ±2.93 years) volunteered to participate in this study. They were divided randomly into two groups: training (n = 11) and control (n = 11). Participants in the training group performed a progressive CRT program for ten weeks. In addition, anthropometry variables, muscular strength and endurance were evaluated before and after ten weeks’ CRT. Also, 10 ml of the blood sample was taken from participants to measure fasting serum glucose, fasting serum insulin and insulin resistance. Results: After ten weeks of CRT, the body composition and glucose dropped significantly (P < 0.05) in the training group. Also, muscular endurance, upper and lower body strength in the post-test were significantly higher than the pre-test in the training group (P < 0.05). Conclusions: CRT led to a significant improvement in insulin resistance, fasting serum glucose, BMI, endurance and strength of elderly men with type 2 diabetes. Therefore, this type of resistance training can be useful for improvement in physical and physiological variables of elderly men with type 2 diabetes.

Aim. To evaluate how health-related quality of life (HRQOL) and traditional Chinese medicine (TCM) constitutions of Yin-Xu, Yang-Xu, and Stasis are related in type 2 diabetes patients. Method. Seven hundred and five subjects were recruited in 2010 for this study from a Diabetes Shared Care Network in Taiwan. Generic and disease-specific HRQOL were assessed by the short form 36 (SF-36) and the diabetes impact measurement scale (DIMS). Constitutions of Yin-Xu, Yang-Xu, and Stasis were then assessed by the body constitution questionnaire (BCQ), a questionnaire consisting of 44 items that evaluate the physiological state based on subjective symptoms and signs. Results. Estimated effects of the Ying-Xu and Stasis on all scales of the SF-36 were significantly negative, while estimated effects of the Yang-Xu on all scales (except for SF, RE, MH, and MCS) were significantly negative. For DIMS, the estimated effects of the Ying-Xu and Stasis on all scales were significantly negative except for Stasis on well-being, while Yang-Xu has a significantly negative effect only on symptoms. Conclusions. This study demonstrates that TCM constitutions of Yin-Xu, Yang-Xu, and Stasis are closely related to a reduction in HRQOL. These findings support the need for further research into the impact of intervention for TCM constitutions on HRQOL in patients with type 2 diabetes.

Type 2 diabetes (T2D) is a global public health priority, particularly for populations of black African-Caribbean ethnicity, who suffer disproportionately high rates of the disease. While the mechanisms underlying the development of T2D are well documented, there is growing evidence describing distinctions among black African-Caribbean populations. In the present paper, we review the evidence describing the impact of black African-Caribbean ethnicity on T2D pathophysiology. Ethnic differences were first recognised through evidence that metabolic syndrome diagnostic criteria fail to detect T2D risk in black populations due to less central obesity and dyslipidaemia. Subsequently more detailed investigations have recognised other mechanistic differences, particularly lower visceral and hepatic fat accumulation and a distinctly hyperinsulinaemic response to glucose stimulation. While epidemiological studies have reported exaggerated insulin resistance in black populations, more detailed and direct measures of insulin sensitivity have provided evidence that insulin sensitivity is not markedly different to other ethnic groups and does not explain the hyperinsulinaemia that is exhibited. These findings lead us to hypothesise that ectopic fat does not play a pivotal role in driving insulin resistance in black populations. Furthermore, we hypothesise that hyperinsulinaemia is driven by lower rates of hepatic insulin clearance rather than heightened insulin resistance and is a primary defect rather than occurring in compensation for insulin resistance. These hypotheses are being investigated in our ongoing South London Diabetes and Ethnicity Phenotyping study, which will enable a more detailed understanding of ethnic distinctions in the pathophysiology of T2D between men of black African and white European ethnicity.

Background: This study aimed to evaluate the effects of a combination of aerobic-resistance training (CARET) and broccoli supplementation on dectin-1 levels and insulin resistance in men with type 2 diabetes mellitus (T2D). Methods: Forty-four males with T2D were randomly allocated to four groups (n = 11 each group): CARET + broccoli supplement (TS), CARET + placebo (TP), control + broccoli supplement (S), and control + placebo (CP). CARET was performed three days per week for 12 weeks. TS and S groups received 10 g of broccoli supplement per day for 12 weeks. All variables were assessed at baseline and 12 weeks. Results: Plasma dectin-1 levels were decreased in TS and TP groups compared with the CP group (p < 0.05). Cardiometabolic risk factors showed significant reductions in TP and TS groups compared to S and CP groups (p < 0.05). Conclusion: The combination of CARET and broccoli supplementation produced the largest improvements in insulin resistance and dectin-1 and other complications of T2D.

The aim of this study was to compare the effects of 8 weeks of aerobic versus resistance training programs on serum fetuin-A, fetuin-B, and fibroblast growth factor-21 (FGF-21) levels in males with type 2 diabetes mellitus. Participants (n = 34) were randomly assigned to a resistance training group (RTG; n = 12), an aerobic training group (ATG; n = 11), or a control group (n = 11). The ATG completed 30–45 min of aerobic running training at 65%–75% of the maximum heart rate. The RTG completed three sets of 10 repetitions maximum of leg press, bench press, knee extension, seated cable row, knee flexion, military press, and calf rise. Blood samples were taken before and after the training period to assess dependent variables. After 8 weeks, both the ATG and the RTG reduced fetuin-A (p < 0.05) and fetuin-B (p < 0.05), but increased FGF-21 (p < 0.05). Moreover, the RTG showed greater decrease than the ATG in fetuin-A (−18.3% vs. −7.9%), fetuin-B (−29.2% vs. −11.45%), and a lower increase in FGF-21 (42.2% vs. 25.1%), respectively. Aerobic and resistance exercise training significantly decreased serum fetuin-A, and fetuin-B, and increased FGF-21 levels in males with type 2 diabetes mellitus. However, more significant alterations in serum factors were observed from resistance training. Thus, resistance training may be considered a more suitable training strategy.
