Context A baseball-specific training load may influence strength or glenohumeral range of motion, which are related to baseball injuries. Glenohumeral reach tests and grip strength are clinical assessments of shoulder range of motion and upper extremity strength, respectively. Objective To examine changes in glenohumeral reach test performance and grip strength between dominant and nondominant limbs and high, moderate, and low baseball-specific training-load groups. Design Repeated-measures study. Setting University laboratory and satellite clinic. Patients or Other Participants Collegiate baseball athletes (n = 18, age = 20.1 ± 1.3 years, height = 185.0 ± 6.5 cm, mass = 90.9 ± 10.2 kg). Main Outcome Measure(s) Participants performed overhead reach tests (OHRTs), behind-the-back reach tests (BBRTs), and grip strength assessments using the dominant and nondominant limbs every 4 weeks for 16 weeks. Percentage change scores were calculated between testing times. After each training session, participants provided their duration of baseball activity, throw count, and body-specific and arm-specific ratings of perceived exertion. We classified them in the high, moderate, or low training-load group based on each training-load variable: body-specific acute:chronic workload ratio (ACWR), arm-specific ACWR, body-specific cumulative load, and arm-specific cumulative load. Mixed models were used to compare training-load groups and limbs. Results The arm-specific ACWR group demonstrated as main effect for OHRT (F = 7.70, P = .001), BBRT (F = 4.01, P = .029), and grip strength (F = 8.89, P < .001). For the OHRT, the moderate training-load group demonstrated a 10.8% greater increase than the high group (P = .004) and a 13.2% greater increase than the low group (P < .001). For the BBRT, the low training-load group had a 10.1% greater increase than the moderate group (P = .011). For grip strength, the low training-load group demonstrated a 12.1% greater increase than the high group (P = .006) and a 17.7% greater increase than the moderate group (P < .001). Conclusions Arm-specific ACWR was related to changes in clinical assessments of range of motion and strength. Clinicians may use arm-specific ACWR to indicate when a baseball athlete's physical health is changing.

Purpose: This study described and analyzed practices and perceptions of rhythmic gymnastics coaches, medical staff, and athletes on training-load management. Methods: Online surveys were distributed among professionals and gymnasts involved in rhythmic gymnastics training across the world. One hundred (50 coaches, 12 medical staff, and 38 gymnasts) participants from 25 different countries completed the surveys. Results: Respondents stated using coaches’ perception on a daily basis as a method of monitoring external (57%) and internal (58%) load, recovery/fatigue (52%), and performance (64%). Variables and methods (eg, wearable devices, athlete self-reported measures, session rating of perceived exertion), and metrics (eg, acute and chronic load) commonly reported in the training-load literature and other sports were not frequently used in rhythmic gymnastics. The majority of coaches (60.3% [17%]) perceived that maladaptation rarely or never occurred. Medical staff involvement in sharing and discussing training-load information was limited, and they perceived that the measurement of athletes’ recovery/fatigue was poor. Gymnasts noted good quality in relation to the measurement of performance. Most participants (≥85%) believed that a specific training-load management model for rhythmic gymnastics could be very or extremely effective. Conclusions: In conclusion, rhythmic gymnastics coaches’ perception is the most commonly used strategy to monitor load, recovery/fatigue, and performance; although, this could be a limited method to guarantee effective training-load management in this sport.

Abstract Purpose: This article addresses several intervention approaches that aim to improve life for individuals with severe aphasia. Because severe aphasia significantly compromises language, often for the long term, recommended approaches focus on additional domains that affect quality of life. Treatments are discussed that involve increasing participation in personally relevant life situations, enhancing environmental support for communication and participation, and improving communicative confidence. Methods: Interventions that have been suggested in the aphasia literature as particularly appropriate for people with severe aphasia include training in total communication, training of communication partners, and activity specific training. Conclusion: Several intervention approaches can be implemented to enhance life with severe aphasia.

Abstract: In a multicenter European approach, the efficacy of the AIXTENT computerized training programs for intensity aspects (alertness and vigilance) and selectivity aspects (selective and divided attention) of attention was studied in 33 patients with brain damage of vascular and traumatic etiology. Each patient received training in one of two most impaired of the four attention domains. Control tests were performed by means of a standardized computerized attention test battery (TAP) comprising tests for the four attention functions. Assessment was carried out at the beginning and at the end of a four week baseline period and after the training period of 14 one-hour sessions. At the end of the baseline phase, there was only slight but significant improvement for the most complex attention function, divided attention (number of omissions). After the training, there were significant specific training effects for both intensity aspects (alertness and vigilance) and also for the number of omissions in the divided attention task. The application of inferential single case procedures revealed a high number of significant improvements in individual cases after specific training of alertness and vigilance problems. On the other hand, a non specific training addressing selectivity aspects of attention lead either to improvement or deterioration of alertness and vigilance performance. The results corroborate the findings of former studies with the same training instrument but in patients with different lesion etiologies.

Background: Although intervention with suicidal patients is one of the hardest tasks in clinical practice, little is known about health professionals’ perceptions about the difficulties of working with suicidal patients. Aims: The aims of this study were to: (1) describe the difficulties of professionals facing a suicidal patient; (2) analyze the differences in difficulties according to the sociodemographic and professional characteristics of the health professionals; and (3) identify the health professionals’ perceived skills and thoughts on the need for training in suicide. Method: A self-report questionnaire developed for this purpose was filled out by 196 health professionals. Exploratory principal components analyses were used. Results: Four factors were found: technical difficulties; emotional difficulties; relational and communicational difficulties; and family-approaching and logistic difficulties. Differences were found between professionals who had or did not have training in suicide, between professional groups, and between the number of patient suicide attempts. Sixty percent of the participants reported a personal need for training and 85% thought it was fundamental to implement training plans targeted at health professionals. Conclusion: Specific training is fundamental. Experiential and active methodologies should be used and technical, relational, and emotional questions must be included in the training syllabus.
