Fasting glucose level is associated with nocturnal hypoglycemia in elderly male patients with type 2 diabetes

Objectives Alzheimer’s disease (AD), impaired fasting glucose (IFG), and Type 2 diabetes mellitus (T2DM) were reported associated with smaller brain volumes. Nevertheless, the association of hyperglycemia with brain volume changes in AD patients remains unclear. To investigate this issue, structural magnetic resonance imaging was used to compare brain volumes among AD patients with different fasting glucose levels. Methods Eighty-five AD patients were divided into three groups based on their fasting glucose level as suggested by the American Diabetes Association: normal fasting glucose group (AD_NFG, n = 45), AD_IFG group (n = 15), and AD_T2DM group (n = 25). Sagittal 3D T1-weighted images were obtained to calculate the brain volume. Brain parenchyma and 33 brain structures were automatically segmented. Each regional volume was analyzed among groups. For regions with statistical significance, partial correlation analysis was used to evaluate their relationships with fasting glucose level, corrected for Mini-Mental State Examination score, age, education level, cholesterol, triglyceride, and blood pressure. Results Compared with the AD_IFG and AD_NFG groups, the volume of pons in AD_T2DM group was significantly smaller. Fasting glucose was negatively correlated with pontine volume. Conclusions T2DM may exacerbate pontine atrophy in AD patients, and fasting glucose level is associated with pontine volume.

Abstract: Stroke is the number three killer right after heart diseases and cancer. Diabetes Mellitus is one of the modifiable risk factor of stroke. The goal of this research was to obtain the data profile of the random glucose level and the fasting glucose level in stroke patients diagnosed with type 2 DM. This research was a descriptive research with the subjects of the type 2 DM diagnosed patients with accountable random glucose level and fasting glucose level which were inpatient in Neurology Department of RSUP. Prof. R. D. Kandou Manado with a time period from January to December 2011. The sex, age, random glucose level and fasting glucose level profile were obtain from the data of medical records. There were 60 stroke patients who were also diagnosed with type 2 DM. Most of them were females (51.67%) with age of 40 to 59 years old (50%), and the most the fasting glucose level of ≥ 200mg/dl (53.33%) and the random glucose level of ≥ 126mg/dl (51.66%). Conclusions: The patients who were also diagnosed with type 2 DM were mostly females with the age of 40 to 59 years old. The data profile of the random glucose level and the fasting glucose level in stroke patients were mostly diagnosed with type 2 DM. This data will be able to help to determine the actions in order to treat and also to prevent stroke in type 2 DM patients. Salah satu faktor risiko stroke yang dapat dimodifikasi adalah diabetes mellitus. Tujuan penelitian ini adalah untuk memperoleh data profil gula darah sewaktu dan gula darah puasa pasien stroke dengan diabetes mellitus tipe 2. Penelitian ini menggunakan metode penelitian deskriptif, subyek penelitian adalah pasien stroke dengan diabetes mellitus tipe 2 yang mempunyai hasil pemeriksaan gula darah sewaktu dan gula darah puasa dan di rawat inap di Bagian Neurologi BLU RSUP Prof. Dr. R. D. Kandou Manado periode Januari – Desember 2011. Jenis kelamin, Usia, Kadar gula darah sewaktu dan gula darah puasa dicatat dari data rekam medik. Terdapat 60 pasien yang mengalami stroke dengan diabetes mellitus tipe 2, sebagian besar adalah perempuan (51,67%), umur 40 – 59 (50%), kadar gula darah sewaktu terbanyak ≥ 200mg/dl (53,33%), kadar gula darah puasa terbanyak ≥ 126 (51,66%). Simpulan : Pasien stroke dengan diabetes mellitus tipe 2 didapatkan lebih banyak pada perempuan dengan usia 40 – 59 tahun. Profil gula darah sewaktu (GDS) dan gula darah puasa (GDP) terbanyak pada kategori terdiagnosis diabetes mellitus. Data ini dapat membantu menentukan kebijakan dalam upaya pencegahan dan penatalaksanaan stroke dengan diabetes mellitus tipe 2. Objective. This study is aimed at analyzing the association between bone metabolism indices and diabetic retinopathy (DR) in elderly patients with type 2 diabetes mellitus. Methods. Data of 352 men and 284 postmenopausal women, aged more than 50 years, with type 2 diabetes mellitus were retrospectively analyzed. Patients were divided into three groups based on the degree of DR: nondiabetic retinopathy (NDR) group, background diabetic retinopathy (BDR) group, and proliferative diabetic retinopathy (PDR) group. Results. (1) The diabetic duration and urinary albumin to creatinine ratio (UACR) were significantly higher in the PDR and BDR groups than in the NDR group ( P < 0.05 ). The level of beta-C-terminal telopeptide (β-CTX) in male patients was lower in the PDR and BDR groups than in the NDR group ( P < 0.05 ). In addition, the level of procollagen 1 intact N-terminal (P1NP) in female patients was higher in the PDR and BDR groups than in the NDR group ( P < 0.05 ). (2) For men and postmenopausal women, the proportion of vitamin deficiency was higher in the PDR and BDR groups than in the NDR group ( P < 0.05 ). (3) The logistic regression analysis in men and postmenopausal women showed that the diabetic duration and lower levels of UACR and 25(OH)D were independent risk factors for DR ( P < 0.05 ). (4) The diabetic duration was also an independent risk factor for PDR ( P < 0.05 ); however, no independent correlation was found between the level of 25(OH)D and PDR ( P > 0.05 ). Conclusions. A close association was observed between 25(OH)D level and DR in the elderly male patients and postmenopausal women with type 2 diabetes mellitus. P1NP and β-CTX levels might be closely related to DR in elderly male patients and postmenopausal women with type 2 diabetes mellitus.
