BACKGROUND: Uterovaginal prolapse is a very common morbidity among Indian women. Vaginal pessaries still have role in the management of uterovaginal prolapse. Long forgotten pessaries get incarcerated in the vagina and their removal poses problems. A 65 year old postmenopausal lady presented with pain lower abdomen and thin vaginal discharge. Speculum examination revealed embedded metallic bangle in the vagina. Attempts at removal by gentle traction resulted in pain. Metallic bangle was removed under general anaesthesia and biopsy from the overlying tissue revealed no evidence of malignancy. Vaginal pessary gives symptomatic relief and does not cause any discomfort, women tend to forget it. This case report is unique as here ordinary metallic bangle was used as a vaginal pessary and in spite of long duration it did not cause much discomfort or any serious complication. Background: Stress incontinence is one of the most common but debilitating health issue among women. It has a detrimental effect on overall health and quality of life of women. Trans obturator tape (TOT) has emerged as a promising treatment modality. The aim of present study was to assess the usefulness of TOT in terms of change in quality of life of stress incontinence patients.Methods: The study was performed at Department of Obstetrics and Gynecology, Acharya Shri Chandler of Medical Sciences (ASCOMS), Jammu. A total of 50 symptomatic women were enrolled in the study and underwent TOT procedure using outside-in technique. The quality of life of women was assessed at enrolment and 12 months after the procedure using King’s Health Questionnaire (KHQ). Change in QOL was assessed using paired ‘t’-test.Results: After 12 months follow-up, a total of 27 (54%) patients were entirely symptom free. As compared to pre-treatment QOL assessment on KHQ for general health/incontinence impact, quality of life and symptom scores a % decline of 73.2%, 79.8% and 78.4% was observed. For all the three parts, the change in scores was significant statistically. No other complications and side effects were reported.Conclusions: TOT is a useful procedure which provided symptomatic relief as well as QOL enhancement.

Intrauterine contraceptive devices are extremely effective and long lasting. They were intended for long term use and were retained for year’s altogether. Menopausal ladies often present with Lippes Loop in situ either deliberately or forgotten. We report a case of 80 years old patient who came with complaints of pain in lower abdomen and vaginal discharge and had Lippes Loop insitu, which was inserted 45 years back. The inserted Lippes Loop thread was not seen in clinical examination but visualized on Transvaginal Ultrasonography along with pyometra. It was removed under hysteroscopic guidance.

Vaginitis in postmenopausal women is termed as atrophic vaginitis, characterised by vaginal bleeding or spotting, vaginal discharge and discomfort or dryness in vulva. As the symptoms are not usually disclosed by women it may become chronic and can adversely affect the quality of life. In cervical intraepithelial neoplasia the squamous epithelium of cervix is replaced by the cells of varying degree of atypia. Even though CIN1 and CIN2 regress spontaneously, untreated CIN1 and CIN2 lesions may progress to invasive carcinoma. Ayurvedic management for the above conditions focuses on symptomatic relief and improvement of general health and immunity. Oral medication and Sthanika Chikitsa like Kshara Karma and Varthi play vital role in this. A 70 year old lady presented with complaints of burning sensation and itching over genital area. On examination haemorrhagic spots were observed on vaginal walls. She was sent to Early Cancer Detection Centre for cytological screening. The reports showed low grade sqamous intraepithelial lesion. She was managed with oral medications and Sthanika chikitsa. During follow up she was relieved from the symptoms like burning sensation and itching over genital area and vaginal discharge. Bleeding spots on vaginal walls were absent and cytology revealed a negative smear for intraepithelial lesion or malignancy. From this case report it is evidenced that Ayurvedic treatment modality is effective and successful for the management of Atrophic vaginitis with CIN.

 Occupational health hazards are the very important unavoidable problems of different type of jobs. All occupational jobs contain several tasks and workers are compelled to adopt some harmful and awkward postures during performing those tasks. These types of repetitive jobs may be related to occupational health hazards like musculo skeletal disorder (MSD). In the present methodology different MSD related problems of the workers can be assessed by the questionnaire techniques considering the issues of community health development. In this concern a simple and easily applicable questionnaire technique has been established for the different occupations during performing tasks. It was concluded that MSD among the workers might be related to the stressful work posture, long duration jobs, nature of jobs and use of ill-fitted hand tools. Abnormal vaginal discharge (AVD) caused by a variety of reproductive tract infections is a widespread syndrome among women in India and in other developing countries. The purpose of this study was to determine whether a polyherbal formulation, Praneem, can be used for the regression of the syndrome. A phase ΙΙ randomized controlled study was carried out with Praneem polyherbal tablets and Betadine vaginal pessary in 99 women with AVD. The authors found that 92% of women using Praneem were relieved of their symptoms of AVD as against 81.6% women using Betadine. Significant reduction was also seen with both treatments in lower abdominal pain, vaginal itching, and dysuria. Thus, the study indicates the efficacy of Praneem for the treatment of AVD and provides a rationale for planning a further Phase III study on a larger sample size for definitive conclusions.

Objective Diabetes mellitus leads to damage, dysfunction and failure of various organs especially eyes, kidneys, nerves and heart. The latency of occurence of hyperglycemia and diagnosis may be of long duration. This study was aimed to find out the mode of presetation of diabetes mellitus in diabetic patients attending out patients clinic of B P Koirala Institute of Health Sciences. Associated complications and comorbid condition present at the time of presenation were also studied.Methods The diabetic patients attending the Diabetic Out Patient Clinic of B P Koirala Institute of Health Sciences during June 2006 to June 2007 were included in this study.The patients details were collected from the predefined Proforma for diabetes patient from the database. This included demographic data, biochemical parameters and diabetic complications. For the purpose of study a total of 775 patients were randomly selected.The Data collected were entered and analysed using excel and SPSS(version 11.5)Results Out of 775 cases 436 (56.3%) were male and 339(43.7%) were female. Majority of patients 81.55% (n=632) had osmotic sympmtoms or symptoms related to complication of diabetes at the time of presentation to the clinic. Asymptomatic patient constituted 18.45% (n=143). The most common presenting complaints were polyuria (44.58%), followed by polydypsia (39.62%) and polyphagia (24.88%). About 54.97% (n=426) had symptoms of complications related to diabetes. Among them most common complication was neurological (39.67%), followed by renal (10.8%) metabolic (4.93%), cardiac(4.46%), autonomic neuropathy (4.93%) and peripheral vascular disease (3.99%).Conclusion Majority of the patients presenting in our OPD had osmotic symptoms or symptoms related to complication of diabetes. Access to diabetes care and lack of awareness of the disease and its complication might had contributed to this. Community awareness, program for early detection and managemnet may help proper diabetes care and prevention of complications.   Journal of College of Medical Sciences-Nepal, 2013, Vol-9, No-3, 25-30

TThis case report concerns a 70-year woman with a complaint of abnormal vaginal bleeding and foul smelling vaginal discharge of a short duration. Speculum examination revealed a huge polyp occupying the cervix and extending further downwards. On operation, it turned out to be an endometrial polyp arising from the fundus. Histologically the polyp showed classical features of alveolar rhabdomyosarcoma; confirmed immunochemically.

Women are always prone to several forms of infection in their genital tract.It ranges from simple form of infectious discharges to malignant conditions in the genital tract. The infectious types of discharge though appear to be a simple form it can be the start of bigger problems in future if left untreated.The tract infection is also important for the fact that it is a most useful part in process of progeny continuation. So, to find out an effective remedy in Ayurveda Palashadi Yoga was selected.15 patients were selected randomly for present clinical trial study. Highly significant results were observed in discharge per vagina and pain in lower abdomen.
