The associations between motor ability, walking activity and heart rate and heart rate variability parameters among children with cerebral palsy and typically developed controls

Abstract Cerebral palsy (CP) is the most prevalent severe motor disability among children. The aim of this work was to assess autonomic dysfunction in children with cerebral palsy clinically and electrophysiologically .The study was carried out on forty children with cerebral palsy their age ranged from 4-12 years and twenty healthy children with matched age and sex as control group. CP children were subjected to questionnaire for autonomic dysfunction symptoms. Both CP children and Control group were assessed for Sympathetic Skin Response and Heart rate variability. Most of children had quadriplegic spastic cerebral palsy (82.5%). Based on Gross motor function classification system (GMFCS) classification the majority of children were in levels 4 and 5. The prevalence of autonomic dysfunction symptoms was 80% for thermoregulatory abnormalities (cold extremities), chronic constipation 65%, sleep disturbance 52.5%, loss of appetite 47.5%, sweating abnormalities 40% , recurrent nausea and/or vomiting 25%, increased sensitivity to light or dark 22.5% and bloating 15%. The percentage of unelicited Sympathetic skin response in CP children was 47.5% and 60% in upper limbs and lower limbs respectively, all of them were in level 4 and 5 of GMFCS. 20% of CP children had postural hypotension. Mean Heart rate of CP children was significantly increased more than healthy children upon head tilt test. Sympathetic Skin Response and Heart rate variability were proven to be simple and non invasive procedures in investigating autonomic dysfunction in CP children.

Background: We aimed to compare and examine the effect of aquatic interventions, Watsu® vs Immersion, on the autonomic nervous system and the range of motion in children with cerebral palsy, due to common belief that use of Watsu is beneficial for the special needs. Material and methods: Twenty-three children (age 7.5±2.8) were randomized to receive Watsu® therapy and Immersion interventions in the cross-over, age-stratified study. Each therapy session lasted 30 minutes twice a week for a total of 10 weeks in two non-consecutive periods. Short-term heart rate variability parameters by using a Polar H7 heart rate sensor with a signal processing software and the passive range of motion by using a universal goniometer was measured at baseline and post-treatment. Results: Watsu® therapy significantly improved the heart rate variability parameter (pNN50, t = 2.312, p = 0.031) and lower flexibility (t = 6.012, p = 0.000) in comparison to immersion. Conclusions: In comparison to immersion, Watsu® therapy was shown to be safe and effective for the autonomic modulation and flexibility of children with cerebral palsy. Therefore, it is recommended as a complementary tool for physical therapy on land.

Cardiac autonomic dysfunction has been reported in patients with cerebral palsy (CP). The aim of this study was to assess the existing literature on heart rate variability (HRV) in pediatric patients with CP and a special attention was paid to the compliance of the studies with the current HRV assessment and interpretation guidelines. A systematic review was performed in PubMed, Web of Science, and Cumulative Index to Nursing and Allied Health Literature (CINAHL) databases searched for English language publications from 1996 to 2019 using Medical Subject Headings (MeSH) terms “heart rate variability” and “cerebral palsy” in conjunction with additional inclusion criteria: studies limited to humans in the age range of 0–18 years and empirical investigations. Out of 47 studies, 12 were included in the review. Pediatric patients with CP presented a significantly higher resting heart rate and reduced HRV, different autonomic responses to movement stimuli compared to children with normal development, but also reduced HRV parameters in the children dependent on adult assistance for mobility compared to those generally independent. None of the included studies contained the necessary details concerning RR intervals acquisition and HRV measurements as recommended by the guidelines. Authors of HRV studies should follow the methodological guidelines and recommendations on HRV measurement, because such an approach may allow a direct comparison of their results.
