Sleep is very important for the health. Analyzing the polysomnography (PSG) helps us get valuable information to assess the quality of sleep. In this work, we develop a program to automatically detect the transition from wakefulness to sleep in adults. The accurate detection of the point of sleep onset occurs in the first time is useful for assessing the micro-structure of sleep. The proposed method is analyzed polysomnography of 30 healthy volunteers, using data of one channel Electroencephalography, Electrooculography and chin Electromyography. The algorithm automatically analyzes every second according to American Academy of Sleep Medicine (AASM) standards with the latest version. The results obtained under two levels: identify and list the epoch occurred the transition, and exact the time of the shift occurred. With more than 85% in accuracy, the study shows the feasibility to provide timely warning. This approach opens up developing a system in real-time warning: doze off in student, drowsiness, sleepiness when driving or working. It helps us to examine the brain's response to external stimuli to reduce the time of sleep latency.

Insomnia is an important medical problem; its treatment requires both nonpharmacological methods (education and psychotherapy) and the use of hypnotic agents. The benzodiazepine derivatives may be used as hypnotic agents but their use is substantially limited. The alter­native treatment includes so-called z-drugs, which means nonbenzodiazepine hypnotic agents. Their mechanism of pharmacological action is a GABA-receptor agonism. In general, these drugs improve the quality of sleep (sleep latency, wake after sleep onset, number and duration of awakenings, total sleep time). Contrary to the benzo­diazepines they do not possess anti-anxiety, myorelaxant, and anti-seizure properties, and are better tolerated. Eszopiclone for the treatment of insomnia was not available in Poland. It is not only effective in the treatment of insomnia in comparison to placebo but is also well tolerated. It may be used for a longer time than the benzodiazepines – not only a couple of weeks but per several months. Eszopiclone shortens the sleep latency, decreases the number of wakes after sleep onset and increases total sleep time. It improves the subjective evaluation of sleep by the patients, the quality of sleep and functioning during the day. Its efficacy in the treatment of insomnia in the elderly has also been proved. It is quite well-tolerated and the most frequent side-effect of eszopiclone in the unpleasant taste.

Sleep disorders are a common and poorly treated disease state. This double blind, four arm placebo-controlled, randomized trial compared (1) low dose trazodone, (2) Sentra PM, a neurotransmitter based medical food, (3) the joint administration of trazodone and the medical food Sentra PM and (4) placebo. There were 111 subjects studied in 12 independent sites. Subjects underwent baseline screening, informed consent and an initial sleep questionnaire. After 14 days subjects underwent a second evaluation by questionnaire. At baseline and Day 14 the subjects underwent 24 hour ECG recordings that were analyzed in the frequency domain of heart rate variability. The specific high frequency parasympathetic autonomic nervous system activity was analyzed. The primary endpoints were sleep latency and parasympathetic autonomic nervous system improvement in sleeping hours. The results showed improvement in sleep latency for the Sentra PM and combination of Sentra PM and trazodone (−41 and −56 minutes P < 0.001). There was an improvement in quality of sleep for the amino acid formulation Sentra PM and the combination (3.86 and 6.48 Likert units on a 10 point scale P < 0.001). There was an activation of circadian activity percent at night in the medical food and combination groups while there was no change in parasympathetic activity in either the placebo or trazodone group. These data indicate that Sentra PM can improve the quality of sleep, the response to trazodone as a sleep medication and parasympathetic autonomic nervous system activity.

Introduction: According to the Brazilian Association of Sleep Medicine, the activities of the present day have hampered the quality and time of sleep, causing damage to the Central Nervous System and releasing substances directly related to stress, thus the metabolism changes in the absence of a few hours of sleep by night they were equated with that of an old man. Objective: To verify the influence of classic (relaxing) body massage associated with atherapy in sleep quality and stress in university teachers. Methodology: An exploratory and qualitative study was carried out, with a sample of 14 university teachers, through questionnaires and massage practices with association of the essential oils of Olíbano and Cedro, controlled the results with oximeter and Power Lab. Results: Although there was no difference it is worth noting that cedar oil reduced in a manner considered the Heart Rate of the volunteers. All volunteers reported that there was improvement in sleep quality regarding the night of the day of receiving the massage, such as decreased pain, constant sleep at night, leading us to confirm the great effect that relaxing massage exerts on the individual. Conclusion: Although the inhalation results of essential oils have not given statistical differences, we can conclude that this procedure is effective in improving stress and gradually improving the quality of sleep.

Abstract Death of a significant other is consistently found to have a detrimental effect on cardiovascular functioning, and such relationship may be stronger when loss is accompanied by low-quality sleep. Using data from the Biomarker project of Midlife in the United States study (n=1,310), we examined whether quality-of-sleep has an additive effect on the relationship between loss and heart rate variability (HRV). Loss was measured as losing someone close within a year of data collection, and was categorized based on the respondents’ relationship with the deceased. Relationship was categorized as: immediate family, relative, and friend. Quality-of-sleep was measured using the Pittsburgh Sleep Quality scale. Results showed that the associations among loss, sleep, and HRV differed by gender. For women, losing an immediate family was associated with worse HRV and this did not differ by quality-of-sleep. For men, death of an immediate family was associated with worse HRV only among those with poorer quality sleep. These results suggest that low-quality sleep may indicate psychophysical vulnerability for men who experienced loss, which may relate to their lower capacity for physiological adaptation.

Background The pathology underlying exploding head syndrome, a parasomnia causing a loud sound/sense of explosion, is not well understood. Kappa rhythm is a type of electroencephalogram alpha band activity with maximum potential between contralateral temporal electrodes We report a case of preceding kappa activity before exploding head syndrome attacks. Case report A 57-year-old woman complained of explosive sounds for 2 months; a loud sound would transpire every day before sleep onset. She was diagnosed with exploding head syndrome. During polysomnography and the multiple sleep latency test, the exploding head syndrome attacks occurred six times. A kappa wave with activity disappearing a few seconds before most exploding head syndrome attacks was observed. The alpha band power in T3-T4 derivation gradually waxed followed by termination around the attacks. Conclusion This case demonstrated that the dynamics of kappa activity precede exploding head syndrome attacks. Finding ways to modulate electroencephalogram oscillation could elucidate their causality and lead to therapeutic intervention.

From this health tracking system, the fitbit application gathers a huge amount of data and allows us to analyze the fitness data collected by the application. By employing techniques like data exploration, modelling, deploying and integrating, we will be able to arrive with very useful insights.

The hypnotic effects of Zopiclone, a novel cyclopyrrolone derivative, were compared with placebo in a double-blind randomized crossover study in insomniac patients. Subjective morning assessments by the patients showed that Zopiclone 7·5 mg improved the quality of sleep, with a reduction in the sleep onset latency and the number of nocturnal awakenings. Zopiclone was judged by the physicians to be superior to placebo and was preferred by the patients. Subjective residual effects and adverse reactions were minimal.

Abstract Beyond broad recognition that ‘staff influence quality’, little is known about the care home workforce and its relationship to quality. Our study examines this relationship for the first time in the UK. Quality is a complex, contested and dynamic concept: we have operationalised this concept using data collected at national and organisational levels to measure quality, as well as considering the views of quality of different stakeholders. We will present interim findings from this study’s work packages (due to complete in July 2020), including: (1) our review which has developed theoretical explanations of how, why and in what circumstances staffing promotes quality for residents and relatives; and (2) our observational study using routine administrative data sets at national and provider level. Key findings include the importance of experience and stability of care hoe staff, as well as how care is delivered (individual commitment, team reciprocity and organisational mandate).

Noninvasive ventilation (NIV) use was initially reported in cystic fibrosis (CF) in 1991 as a bridge to lung transplantation, and over the decades the use of NIV has increased in the CF population. Individuals with CF are prone to various physiologic changes as lung function worsens, and they may benefit from NIV for advanced lung disease. As life expectancy in CF has been increasing due to advances such as highly effective modulator therapy, people with CF are now subject to the same co-morbidities that may benefit from NIV as their age matched cohorts. NIV can improve gas exchange, quality of sleep, exercise tolerance and augment airway clearance in CF, and it is important that CF providers are comfortable with this therapeutic modality. In this review, we will summarize the physiologic basis for NIV use in CF, describe indications for initiation of NIV, and postulate a practical approach for CF clinicians to take fin monitoring patients on NIV. We will discuss aspects unique to people with CF and the use of NIV. We hope that this serves as a resource for CF providers, especially those of us who do not have dedicated training in sleep medicine, as we continue to care for our patient population.
