Diabetes mellitus continues to be one of the mode widespread diseases around the world. More prevalently, type II, or non insulin dependent diabetes, is associated with a more sedentary lifestyle, lack of daily exercise, and chronic hyperglycemia related to poor diet. Of all the body systems and organs affected by diabetes, the foot is perhaps one the most brutally affected and relates the most devastating consequences. Diabetes Mellitus continues to be the number one cause of non traumatic lower limb amputations , which leads to mortality rates higher than several types of cancer. This article will review the epidemiological data and review the clinical warning signs of the high risk diabetic foot.

Background : The number of DM victims in the world increased year by year. According to Health Elementary Research in 2013  the highest number was in DIY, it was 2,6%. Then, according to Godean I Local Government Clinic data, DM was the second rank of Non-Communicable Disease (NCD) after hypertension. The increasing of DM victims number was connected by the overweight case that was influenced by unhealthy diet and lack ofphysic activities so it caused accumulation of fat tissues. In the next time it can cause complication for the victims. There are three pillars of DM management namely, diet, psychological intervention, and exercises. The first that should be done is exercises. One of them is physic exercises namely DM gymnastic. Its functions are to control blood sugar, lose weight and body fat so cardiovascular disease risk factor can be prevented and fixed. In this research the DM gymnastic was done three times in a week for six weeks in duration time of 30-60 minutes. Objective : Purpose of the research to know the influence of DM gymnastic to the change of fat body percentage of DM type II victims in Godean I Local Government Clinic. Method : Research method this research used quasi-experiment method, the research plan used pre-post design. The population was all of DM type II victims in Godean I Local Government Clinic with the subject number were 24 people. The data analysis used statistic test Paired Sample t Test with the credibility 95%. Result : Result of the research the statistic test Paired Sample t Ttest showed that there were not differences before and after DM gymnastic treatment for six weeks. According to the average of fat body percentage after DM gymnastic treatment was higher than before (32,52% > 32,86%). Conclusion : There was no influence of DM gymnastic to the body fat percentage of DM type II victims in GodeanI Local Government Clinic p = 0,59 or p = >0,05.   Keywords: Gymnastic, Diabetes Mellitus, Fat Body Percentage

Diabetes mellitus is one of cronical degenerative disease. The prevalence of sufferers continues to increase every year, one of the non-pharmacological therapies is exercise such as gymnastics prolanis DM and walking exercises to decrease the glucose level by the body muscles during the physical body. Objective this research is to find out the effectiveness of Prolanis and walking exercises in reducing blood sugar levels in DM patients type II. Method this study uses the Quasi experiment with pre and post with two group design. The study population was 92 DM patients type II with a sample of 82 people. Glucometer (Auto check blood glucose monitor) is used to collect the data. Wiloxon Test is used to analyze the statistical data. Results the research shows that the value of Prolanis Exercises (DM) is p=0.002 which means a significant influence on decrease of the sugar level of DM patients type II. Meanwhile the value of walking exercises is p=0.001. From the statistic test of WIlconxon Signed Rank Test, It obtained Z correlation = 7.886. It means that there is a significant correlation between Prolanis and walking exercises towards the decrease of blood sugar level of Diabetes Mellitus patients type II. Physical activities done regularly, measurably, and correctly are suggested to be considered as non-pharmacological therapies for Diabetes Mellitus patients type II to decrease blood sugar level. Objects: The study's purpose was to see if there was a link between netrin.1 and type II diabetes patients. Methodology: The present study carried out on (45) patients affected with diabetes mellitus from (120) persons were examined in the education of Diwaniyah hospital from 6th September to the 4th December 2020. The study includes measurement of the body mass index, fasting blood glucose level, HbA1c, lipid profile and level of netrin-1. Results: Our findings were revealed the netrin-1 level was significantly lower in the diabetes (1205.36±753.09) compared to control group (1477.79±700.26; P < 0.01). Our observations were appeared Significantly higher levels of blood cholesterol, TG, LDL, and VLDL-C and In diabetes individuals, there was a significant drop There was a 0.05 difference in HDL-C levels when compared to healthy controls. Conclusion: These findings indicate that low netrin1 concentration in serum are strongly linked to the occurrence of type II diabetes.

Diabetes mellitus (DM) is not one disorder; it represents a series of metabolic conditions related to hyperglycemia and caused by defects in hormone secretion and hormone action. Exposure to chronic hyperglycemia may result in microvascular complications in the retina (diabetic retinopathy), kidney (diabetic nephropathy), neuron (diabetic-neuropathy), skin, foot, and cardiac complications (stroke, hypertension…etc.). International Diabetes Federation estimates that 1.1 million children and adolescents aged 14–19 years have type one DM. Without interventions to halt the increase in diabetes, there will be at least 629 million people living with diabetes by 2045. In the body, white adipose tissue is the leading site for the storage of excess energy produced from the food intake in large quantities, of the development of insulin resistance (IR) and type 2 DM by the over intake of fatty acid in the body. It results in the accumulation of fatty acyl co-A (FA-CoA) within the myocytes. It leads to improper signaling of the insulin and reduces the level in the myocytes and pancreases beta cells. It combines with genetically reduces the expression of peroxisome proliferator-activated receptor-gamma (PPAR-γ) coactivator-1, initiates the inflammation process by the activation of the tumor necrotic factor alpha and protein kinase C. These alterations lead to further increase the intramyocellular FA-CoA and triglycerides. The sequence of events may develop mitochondrial dysfunction in the sarcolemma outer layers. Finally improves IR also with increasing intramyocellular lipids. This concept might be helpful to those who are pursuing endocrinology specialization, nursing staff, pharmacists, and other medical departments.
