Introduction: One of the health service facilities is a hospital. Hospitals are required to provide the best services for patient satisfaction. Satisfaction will be fulfilled if the service provided in a hospital is felt to be in accordance with patient expectations, otherwise the patient will not feel satisfied. Disatisfaction with services will encourage patients to use the services of other competitors and will result in decreased profits for the hospital. The level of satisfaction is closely related to quality services so that the services provided are in accordance with patient expectation. The purpose of this study was to determine the level of patient satisfaction, patient assessment of the quality of health services and the relationship between the level of satisfaction with the quality of health services in an Out Patient Department (OPD) of Bandar Lampung Adventist Hospital. Methods: The method used in this study is a descriptive correlation involving 160 patients in OPD of Bandar Lampung Adventist Hospital. The data gathering was done by purposive sampling, and used a questionnaire instrument. The questionnaire used in the form of a checklist consist of two parts were those related to the patient satisfaction levels (23 questions) and related to the quality of OPD services (45 questions), so the total of all questions was 68. Then the data were analyzed using the McClave percentage formula, Benson, and the correlation formula according to Sugiyono (2007: 122). Results: The results of this study found the percentage level of patient satisfaction in the Out Patient Department of Bandar Lampung Adventist Hospital is 76.35%. This states that the level of patient satisfaction is included in the high category. The percentage of patients' assessment of service quality is 75.96%, this value is also included in the high category. Through these two percentages, the correlation value is 0.852. Patients might be willing to pay more to obtain better quality medical services when they recognize that high-level hospitals have better quality. However, published papers have not found solid empirical evidence to support this possibility. Therefore, the purpose of this study is to empirically investigate patients’ willingness to pay (WTP) for an outpatient copayment. The study aims to analyze the difference between the two WTP values: to implement a hierarchy of medical care and to improve the quality of medical services. This study administered a questionnaire using the contingent valuation method with a quasi-bidding game for patients’ WTP and the SERVQUAL scale for medical service quality. The Wilcoxon signed-rank test was employed to test the difference between the two WTP values, notably to implement a hierarchy of medical care and to improve the quality of medical services. Both of the WTP values are higher than the academic medical centre’s current copayment NT$420 (approximately US$14); the percentage of respondents willing to pay a higher copayment declined when the outpatient copayment was increased, and the patients’ WTP to have better medical service quality was significantly higher than that to implement a hierarchy of medical care. Patients’ desire to receive better medical services from higher-level hospitals might be stronger than their desire to implement hierarchical medical care. This study reported the relationship between the respondents’ perceived medical service quality and WTP for having better service quality by using regression models. The respondents’ perceptions of medical service quality, especially for “reliability” and “assurance,” would positively affect their WTP. Policy makers should focus on improving the quality of medical services.

Abstract Introduction. Family medicine has a growing responsibility for the health of an individual, and therefore of society as a whole. Establishing an efficient primary healthcare system requires devising an appropriate management strategy. Surveying the quality assessment of medical services is an important element that influences the future shape of the development and management strategy in communes due to the dynamically changing demographic and economic trends, which are leaving their mark on the functioning of local communities. Aim. Evaluation of quality of medical services in primary healthcare centres in communes of Lubelskie Voivodeship. Material and methods. The anonymous survey was conducted among 60 patients benefitting from the medical services provided in healthcare centres in Niemce (Niemce Commune, Lublin District). An original questionnaire in the form of a poll was used to assess the quality of services provided in primary healthcare centres. Results. The results of surveys carried out among the patients of medical centres providing services in the field of primary healthcare (PHC) demonstrated the high level of satisfaction with the received services among the respondents. However, the accessibility of primary healthcare services outside the business hours of PHC centres and the functioning of prevention and health-education programmes need improvement. Conclusions. Based on the survey results we can conclude that the quality assessment of medical services provided by PHC centres, carried out through the ongoing monitoring of patients’ satisfaction, should be the most important indicator of changes in the management of primary healthcare.
