IntroductionMany individuals with type 2 diabetes mellitus (T2DM) experience “psychological insulin resistance”. Consequently, it could be expected that insulin therapy may have negative effects on psychological outcomes and well-being. Therefore, this study compared health status and psychosocial functioning of individuals with T2DM using only oral antihyperglycemic agents (OHA) and on insulin therapy (with or without OHA).Materials and MethodsIn this cross-sectional study, we used baseline data of a cluster randomized controlled trial conducted in 55 Dutch general practices in 2005. Health status was measured with the Short Form (SF)-36 (scale 0–100) and psychosocial functioning with the Diabetes Health Profile (DHP, scale 0-100). To handle missing data, we performed multiple imputation. We used linear mixed models with random intercepts per general practice to correct for clustering at practice level and to control for confounding.ResultsIn total, 2,794 participants were included in the analysis, their mean age was 65.8 years and 50.8% were women. Insulin-users (n = 212) had a longer duration of T2DM (11.0 versus 5.6 years) and more complications. After correcting for confounders and multiple comparisons, insulin-users reported significantly worse outcomes on vitality (SF-36, adjusted difference -5.7, p=0.033), general health (SF-36, adjusted difference -4.8, p=0.043), barriers to activity (DHP, adjusted difference -7.2, p<0.001), and psychological distress (DHP, adjusted difference -3.7, p=0.004), all on a 0-100 scale.DiscussionWhile previous studies showed similar or better health status in people with type 2 diabetes receiving insulin therapy, we found that vitality, general health and barriers to activity were worse in those on insulin therapy. Although the causality of this association cannot be established, our findings add to the discussion on the effects of insulin treatment on patient-reported outcomes in daily practice.

Purpose The purpose of this paper is to investigate certain social and psychological parameters and to compare them with basic demographic information, such as the gender and the education, of patients suffering from Type 2 diabetes mellitus (T2DM). Design/methodology/approach A cross-sectional study of Type 2 diabetic patients was conducted. In total, 200 patients with T2DM were enrolled in the study. Psychological factors were assessed with questionnaires, including the Symptom Checklist 90-R (SCL-90-R), the Short-Form Health Survey (SF-36), the Life Orientation Test-Revised (LOT-R) and the Cardiac Anxiety Questionnaire (CAQ). The associations of psychological with socio-demographic factors were assessed through logistic regression analyses. Findings Women patients had higher levels of heart-focused anxiety psychopathology than men, and therefore, women tend to avoid activities that burden the heart. Men patients had a significantly higher index of physical functionality than women. No differences were recorded by the SCL-90R questionnaire between men and women. For the patients with elementary education, lower levels of mental health (SF-36-MCS) were observed. Originality/value The chronicity of the disease aggravates the psychopathology of the patients thereby creating adverse impact not only on health but also on efforts for compliance. A personalized approach by the health professionals could contribute in addressing the psychological factors that accompany T2DM.

Abstract: Diabetes Mellitus is one of the most common degenerative diseases and which cannot be cured, that can controlled through proper management. Insulin therapy as the most effective pharmacological therapy, requires compliance to reduce the risk of Diabetes Mellitus. Health locus of control is a set of beliefs about what is good and bad that affects person's health status, this is why the health locus of control related to compliance. Purpose to find out the correlation between health locus of control and compliance insulin therapy in type 2 Diabetes Mellitus patients at Pancaran Kasih GMIM Hospital Manado. Method uses cross sectional study design. Sample consisted of 40 respondents with purposive sampling method. Results with Kolmogorov Smirnov test at significance level of 95%, obtained ρ value is 0.077 greater than the significant value 0.05. Conclusion there is no correlation between health locus of control and compliance insulin therapy in type 2 Diabetes Mellitus patients at Pancaran Kasih GMIM Hospital Manado.Keywords: Diabetes mellitus, Health Locus Of Control, ComplianceAbstrak: Diabetes Melitus merupakan salah satu penyakit degeneratif yang paling sering diderita dan yang belum dapat disembuhkan, hal yang dapat dilakukan adalah dengan mengontrol penyakit lewat pengelolaan Diabetes Melitus yang tepat. Tujuan untuk mengetahui hubungan antara health locus of control dengan kepatuhan terapi insulin pada pasien Diabetes Melitus tipe 2 di RSU GMIM Pancaran Kasih Manado. Metode menggunakan desain penelitian cross sectional study. Sampel terdiri dari 40 responden dengan metode pengambilan sampel purposive sampling. Hasil dengan menggunakan uji kolmozorov smirnov pada tingkat kemaknaan 95%, didapatkan nilai ρ value 0,077 lebih besar dari nilai signifikan 0,05. Kesimpulan tidak terdapat hubunganantara health locus of control dengan kepatuhan terapi insulin pada pasien Diabetes Melitus tipe 2 di RSU GMIM Pancaran Kasih Manado.Kata Kunci: Diabetes Melitus, Health Locus Of Control, Kepatuhan
