Spatial neglect has been shown to occur in 17–65% of patients after acute left-hemispheric stroke. One reason for this varying incidence values might be that left-hemispheric stroke is often accompanied by aphasia, which raises difficulties in assessing attention deficits with conventional neuropsychological tests entailing verbal instructions. Video-oculography during free visual exploration (FVE) requires only little understanding of simple non-verbal instruction and has been shown to be a sensitive and reliable tool to detect spatial neglect in patients with right-hemispheric stroke. In the present study, we aimed to investigate the feasibility of FVE to detect neglect in 10 left-hemispheric stroke patients with mild to severe aphasia as assessed by means of the Token Test, Boston Naming Test and Aachener Aphasie Test. The patient’s individual deviation between eye movement calibration and validation was recorded and compared to 20 age-matched healthy controls. Furthermore, typical FVE parameters such as the landing point of the first fixation, the mean gaze position (in ° of visual angle), the number and duration of visual fixations and the mean visual exploration area were compared between groups. In addition, to evaluate for neglect, the Bells cancellation test was performed and neglect severity in daily living was measured by means of the Catherine Bergego Scale (CBS). Our results showed that the deviation between calibration and validation did not differ between aphasia patients and healthy controls highlighting its feasibility. Furthermore, FVE revealed the typical neglect pattern with a significant leftward shift in visual exploration bahaviour, which highly correlated with neglect severity as assessed with CBS. The present study provides evidence that FVE has the potential to be used as a neglect screening tool in left-hemispheric stroke patients with aphasia in which compliance with verbal test instructions may be compromised by language deficits.

Abstract Objectives To examine the validity and reliability of the Mandarin version of the Treatment Burden Questionnaire (TBQ) among stroke patients. Background Stroke patients need long-term management of symptoms and life situation, and treatment burden has recently emerged as a new concept that can influence the health outcomes during the rehabilitation process. Methods The convenience sampling method was used to recruit 187 cases of stroke patients in a tertiary grade hospital in Tianjin for a formal investigation. Item analysis, reliability and validity tests were carried out. The reliability test included internal consistency and test–retest reliability. And as well as content, structure and convergent validity were performed for the validity test. Results Of the 187 completed questionnaires, only 180 (96.3%) were suitable for analysis. According to the experts’ evaluation, the I-CVI of each item was from 0.833 to 1.000, and the S-CVI was 0.967. The exploratory factor analysis yielded three-factor components with a cumulative variation of 53.054%. Convergent validity was demonstrated using measures of Morisky’s Medication Adherence Scale 8 (r = –0.450, P < 0.01). All correlations between items and global scores ranged from 0.403 to 0.638. Internal consistency reliability and test–retest reliability were found to be acceptable, as indicated by a Cronbach’s α of 0.824 and an intraclass correlation coefficient of 0.846, respectively. Conclusions The Mandarin TBQ had acceptable validity and reliability. The use of TBQ in the assessment of treatment burden of stroke survivor may benefit health resources allocation and provide tailor therapeutic interventions to construct minimally disruptive care.

Objective. To provide empirical justification for selecting motor scales for stroke patients, the authors compared the psychometric properties (validity, responsiveness, test-retest reliability, and smallest real difference [SRD]) of the Fugl-Meyer Motor Scale (FM), the simplified FM (S-FM), the Stroke Rehabilitation Assessment of Movement instrument (STREAM), and the simplified STREAM (S-STREAM). Methods. For the validity and responsiveness study, 50 inpatients were assessed with the FM and the STREAM at admission and discharge to a rehabilitation department. The scores of the S-FM and the S-STREAM were retrieved from their corresponding scales. For the test-retest reliability study, a therapist administered both scales on a different sample of 60 chronic patients on 2 occasions. Results. Only the S-STREAM had no notable floor or ceiling effects at admission and discharge. The 4 motor scales had good concurrent validity (rho ≥ .91) and satisfactory predictive validity (rho = .72-.77). The scales showed responsiveness (effect size d ≥ 0.34; standardized response mean ≥ 0.95; P < .0001), with the S-STREAM most responsive. The test-retest agreements of the scales were excellent (intraclass correlation coefficients ≥ .96). The SRD of the 4 scales was 10% of their corresponding highest score, indicating acceptable level of measurement error. The upper extremity and the lower extremity subscales of the 4 showed similar results. Conclusions. The 4 motor scales showed acceptable levels of reliability, validity, and responsiveness in stroke patients. The S-STREAM is recommended because it is short, responsive to change, and able to discriminate patients with severe or mild stroke.

Abstract Introduction: A proper assessment of gait pattern is a significant aspect in planning the process of teaching gait in hemiparetic post-stroke patients. The Wisconsin Gait Scale (WGS) is an observational tool for assessing post-stroke patients’ gait. The aim of the study was to assess test-retest reliability and internal consistency of the WGS and examine correlations between gait assessment made with the WGS and gait speed, Brunnström scale, Ashworth’s scale and the Barthel Index. Material and methods: The research included 36 post-stroke patients. The patients’ gait was assessed with the use of the Wisconsin Gait Scale, gait speed with the use of walk test, the level of motor control in a paretic lower limb – according to Brunnström recovery stages, muscle tone in a paretic lower limb – according to modified Ashworth’s scale and functional independence was assessed using the Barthel Index. Gait was assessed with the use of the WGS twice, with a 7-day interval, by three experienced physiotherapists. Results: The analysis of internal consistency of the WGS revealed that the Cronbach’s α coefficient was high in the case of all the three raters and ranged from 0.85 to 0.88. It was noted that the coefficient of variation for all the comparisons was below 10%. When assessing the repeatability of the results, it was revealed that correlations between both measurements made by particular raters were very strong and highly significant. The WGS results significantly correlated with Brunnström scale, Ashworth’s scale and gait speed. Conclusions: It was concluded that the WGS has a high internal consistency and test-retest reliability. Also, significant correlations were found between gait assessment made with the use of the WGS and gait speed, level of motor control and muscle tone of a paretic lower limb. The WGS constitutes a promising tool for a qualitative, observational analysis of gait in post-stroke patients and allows for proper planning, monitoring and assessing rehabilitation results.

Abstract Background: The Box and Block Test (BBT) has been widely used to assess gross upper extremity (UE) motor function. We designed a haptic-combined virtual reality (VR) system, named VBBT, to improve BBT with kinematic parameters for more specific assessments.Methods: According to the block-transfer task in BBT, we designed a VBBT system with a haptic device to provide a sense of the grasping force and block's gravity during task performance for UE function assessment. Besides the number of transferred blocks (N-TB), some kinematic parameters were also collected in VBBT for more specific assessment, including the number of zero-crossings of moving acceleration (NZC-ACC), the number of zero-crossings of derivative of releasing force (NZC-DRF), the ratio of path length and linear length (PLR) and the distance between barrier partition and drop position (DDP). 16 post-stroke patients and 113 healthy subjects were recruited to examine quantitative performances, concurrent validity, test-retest reliability and subjective preference between the BBT and VBBT.Results: Based on the measurements of 95% healthy subjects, the normative ranges of quantitative performances were established for BBT and VBBT. Deficiencies in patients’ UE function could be identified when their measurements fell outsides. A moderate correlation was found in the N-TBs between the VBBT and BBT (r = 0.42). The measurements in VBBT presented a stronger age-related correlation than that in BBT (R2 = 0.57 and R2 = 0.16). The N-TBs in both BBT and VBBT were strongly correlated to the Action Research Arm Test (ARAT) (|r| = 0.84 and 0.83), and the NZC-ACC and NZC-DRF in VBBT also showed significant correlations. (|r| = 0.76 and 0.79). The N-TB, NZC-ACC and NZC-DRF in VBBT showed a good test-retest reliability (ICC = 0.75, 0.78 and 0.80) while a moderate reliability was found in BBT (ICC = 0.62). For the patient preference, VBBT was given a higher score than the BBT (p < 0.05) for its enjoyment and completion effort performing.Conclusion: The VBBT improved a gross manual assessment in BBT, which can provide clinically validated, reliable and motivative assessment with kinematic parameters for specific UE motor functions of post-stroke patients.

This study investigated the reliability and validity of the Korean version of the Utrecht Scale for Evaluation of Rehabilitation-Participation (K-USER-P) in patients with stroke. Stroke patients participated in this study. The Utrecht Scale for Evaluation of Rehabilitation-Participation was translated from English into Korean. A total of 120 questionnaires involving the K-USER-P were distributed to rehabilitation hospitals and centers by mail. Of those, 100 questionnaires were returned and 67 were included in the final analysis after exclusion of questionnaires with insufficient responses. We analyzed the questionnaires for internal consistency, test-retest reliability, and construct validity. The results indicated that internal consistency coefficients of the frequency, restriction, and satisfaction domains were 0.69, 0.66, and 0.67, respectively. Test-retest reliability was 0.63, 0.45, and 0.71 for the three domains, respectively. Intercorrelations between the SF-12 and the London Handicap Scale were generally moderate to good. The Korean version of the Utrecht Scale for Evaluation of Rehabilitation-Participation can be used as a measure of the participation level of stroke patients in clinical practice and the local community.
