Background: Uterine involution assessments are critical for the prevention of postpartum hemorrhage. Various methods have been used worldwide. Methods: The PUUS (Postpartum Uterine Ultrasonographic Scale) method evaluates, by transabdominal ultrasonography, the length of the endometrium of the uterine cavity occupied by blood or debris, from grade 0 (no blood) to grade 4 (over three-quarters of the endometrial length occupied by blood/debris). A total of 131 consecutive patients admitted for delivery in the Elena Doamna Obstetrics and Gynecology University Hospital in Iasi, Romania, were prospectively evaluated using the PUUS method. The mean age was 27.72 years old, and they were examined during the first 24–48 h after vaginal delivery, or in the first 48–72 h after cesarean delivery. For patients with a PUUS grade greater than 1, re-examination was preformed daily in the following days, until the PUUS grade decreased to 1 or 0. Results: By standardizing uterine involution in a numerical fashion, we precisely demonstrate that uterine involution varied with the method of delivery (vaginal/cesarean) and with the number of vials of oxytocin received intrapartum, but not with the number of vials of ergometrine maleate received, and not with the origin of the parturient (rural/urban).

Postpartum hemorrhage is a leading cause of maternal mortality. Various methods can be used to evaluate the postpartum uterine cavity volume. This work aims to introduce a simple method for uterine postpartum cavity volume evaluation, called Postpartum Uterine Ultrasonographic Scale (PUUS), which could be used routinely. In this prospective study, 131 consecutive Caucasian patients were evaluated by using the PUUS method. The mean age was 27.72 years (ranging from 15 to 42). Patients were examined in the same time intervals: within the first 24-48 hours after delivery in case of vaginal delivery, and within the first 48-72 hours, in case of cesarean delivery. Patients with PUUS grades 2, 3, or 4 were reexamined daily until the PUUS grade declined to 1 or 0. The PUUS method evaluated the length of the endometrium of the uterine cavity occupied by blood or debris, from grade 0 (no blood) to grade 4 (over three-quarters of the endometrial length occupied by blood/debris). The PUUS grade of uterine involution varied with the day of examination, gestation, and parity. In this article, a novel method of evaluating uterine postpartum involution titled PUUS is introduced. This method standardized uterine cavity involution in a numerical fashion. We hope that the PUUS scale could further be used to decrease the morbidity and mortality of women due to postpartum hemorrhage.

Background: The objective of this study is to assess the efficacy and safety of Fahmy's four quadrant suture technique (FFQS) in controlling blood loss during cesarean delivery for placenta previa (PP).Methods: The study was carried out in a tertiary University Hospital between January 2017 to December 2017 involved 12 women with heavy bleeding occurred after removal of the placenta from the lower section during cesarean delivery for PP. The FFQS technique consisted of two sutures to ligate the uterine branches on both sides and two sutures on the anterior and posterior wall of the lower uterine segment. Details regarding the management and maternal outcomes were recorded.Results: The mean age of the study participants was 29.58±5.29 years and the mean parity was 2.25±1.14. The mean gestational age at termination of pregnancy was 36.91±1.38 weeks.  The mean duration of the whole surgery was 78.75±43.28 minutes, while the mean duration of FFQS technique was only 10±2.09 minutes. The technique was exclusively effective in 8 out of 12 cases (66.67%) while 2 cases needed bilateral internal iliac artery ligation and 2 cases needed hysterectomy. The mean amount of blood loss in all cases was 2433.33±833.76 ml. the mean amount of transfused packed RBCs was 3.92±1.68 units and fresh frozen plasma (FFP) was 3.42±1.44 units. All cases had uneventful postoperative course and no mortality cases in present series.Conclusions: The new technique; FFQS represents a rapid, effective, and inexpensive opportunity for women with bleeding from the lower segment of uterus due to PP. This simple procedure should be attempted before other complex measures to achieve good hemostasis.

Introduction: Uterine involution, a physiological process in which the uterus reverts to prepregnant size, occurs during puerperium. The latter is a time of substantial maternal risk. Ultrasound is considered the most appropriate tool in monitoring the progression of uterine involution because it is cheap, convenient and repeatable with no radiation risk. Aim: To sonographically evaluate the involution of the uterus and uterine cavity in postpartum women in Enugu, Nigeria. Materials and Methods: This was a prospective longitudinal cohort study on 400 postpartum women at the University of Nigeria Teaching Hospital, Enugu, Nigeria. Transabdominal ultrasonography to measure uterine dimensions was done on day 1, day 3 and day 42 of puerperium using a 3.5-5 MHz curvilinear probe of a portable ALOKA ultrasound machine. Statistical analysis was done with the Statistical Package for Social Sciences (SPSS) software version 20.0. The uterine dimensions were expressed as mean±standard deviation and student's t-test analysis was used to compare the values for day 1, day 3 and day 42 of puerperium. Regression analysis was carried out to measure the relationship between uterine measurements on days 1, 3 and 42. A p-value of <0.05 was considered statistically significant. Results: The mean longitudinal, anteroposterior and transverse diameters of the uterus at day 1 were 14.51 cm, 8.54 cm and 10.97 cm, respectively. The corresponding values for day 42 were 8.27 cm, 4.78 cm, and 6.22 cm. The uterine diameters decreased significantly as puerperium advanced (p<0.001). The mean uterine cavity diameters also decreased significantly as puerperium advanced (p<0.001). There was significant regression between longitudinal diameters of both uterus and uterine cavity on days 1 and 3 but not between days 3 and 42. Conclusion: This study has established a normal range of diameters for the uterus and uterine cavity on day 1, day 3 and day 42 of puerperium. It can serve as reference data for further studies in the study area.

Objectives: To determine the frequency of microalbuminuria in patients withessential hypertension for postpartum hemorrhage. Study Design: Cross sectional study.Setting: Department of Medicine at Liaquat University Hospital Hyderabad. Period: Six months(from January 2015 to June 2015). Patients and Methods: All the patients ≥35 years of age bothgender had essential hypertension were enrolled and explored for microalbuminuria by urineexamination while the data was analyzed in SPSS 16. Results: Total one hundred subjects withessential hypertension were studied and explored for microalbuminuria. The mean age ±SDof for overall population was 52.82±7.85 and it was 51.23±8.21 and 53.32±6.52 in male andfemale gender respectively. The overall mean ± SD for systolic blood pressure was 170±12.62whereas it was 160.82±10.82 and 165±11.92 in male and female gender respectively. Themean ±SD for blood pressure (diastolic) was 105±71 while it was 95.01±7.21 and 100.82±6.42in male and female sex respectively. The duration of hypertension as mean ±SD was 3.72±2.52in overall population. The microalbuminuria was identified in 59 (59%) patients with statisticalsignificance in relation to age (p=0.00), gender (p=0.00), duration of hypertension (p=0.01),treatment status (p= 0.04), hypertension (p=0.002), BMI (p=0.05) and residence (p=0.00).Conclusion: In present series the prevalence for microalbuminuria in essential hypertensionwas detected as 59% and correlate with age, gender and duration and treatment status of thepatients along with raised systolic and diastolic blood pressure

Purpose: The study aims to investigate the effectiveness of Hayman’s suture in the management of Atonic postpartum hemorrhage. Subjects and Method: The study includes 37 patients who developed severe atonic PPH not responsive to pharmacological measures at the Department of Obstetrics and Gynecology at Aswan University hospital, in the period between Mars 2018 and December 2019. This is a case series study of 37 patients upon whom Hayman suture is applied when failed to respond to medical management, step-wise devascularization of the uterus is attempted when the suture cannot control the bleeding alone, reserving hysterectomy as a last resort when all measures are failed. Results: Results of the research None of the 37 patients of our study required hysterectomy to control the bleeding, Hayman suture succeeded solely to arrest the hemorrhage in 30 patients (81.1%), while 7 patients(18.9%) required step-wise devascularization of the uterus with Hayman suture to stop the bleeding. It is not uncommon for complications to be described with the use of uterine compression sutures. In our study, we did not report any complications. Conclusion: The application of Hayman’s suture is a fast, efficient, non-complex, lifesaving and fertility-sparing technique to control atonic postpartum hemorrhage.

Objective: To report a case of Ectopic pregnancy with implant receptorsMaterials and Methods: This article describes the case of a 41-year-old woman, diagnosed with an Ectopic Pregnancy on the gravid G5P4A0H4 11-12 weeks. Patients are using implant contraception, which has been used since 2009-2018 (3 installations). The patient came to the obstetrics and gynecology clinic Dr. M. Djamil Padang. The ultrasound gives the impression of Ectopic pregnancy in the infindibule tube. Analysis Ectopic pregnancy can also occur due to the influence of hormonal contraceptive use (progesterone).Results: Patient gets intervention performed laparotomy After the peritoneum is opened it appears fresh red blood amounting to ± 500 cc filling the abdominal cavity. Blood evacuation and exploration are performed, it appears that the source of bleeding originates from the left tubal rupture (Infundibulum) Impression of the left tubal rupture (Infundibulum) ecectic ectopic pregnancy is impaired. Sinistal salpingectomy is performed.Conclusion: Ectopic pregnancy Ectopic pregnancy is all pregnancies where the ovum fertilized by spermatozoa implant and grows outside the uterine cavity endometrial.

This research is aimed to expose the narrative structure of the novel Ca Bau Kan by using semiotical theory. The source of the data is the novel Ca Bau kan written by Remy Silado and published by KPG, eight edition, 2004. The data is collected by doing the library research. The teory applied in this research is the emiotical theory, especially the literary analysis of Subur Laksono Wardoyo that the analysis of the text of prose can be applied by using three fases; the analysis of the basic scheme narrative, the analysis of mean signifier, and the analysis of syntagmatics and pragmatics. The result of this research showed that the narrative structure in the novel CBK that (1) the life of Tinung before being a ca bau kan, (2) the life of Tinung as a ca bau kan, and (3) the life of Tinung after not being a ca bau kan anymore. Based on the narrative structure, it was found that “ Love is only one. No measurement is needed” is the mean signifier and able to be clarified by the analysis of syntagmatics-paradigmatics based on the biner oposition of weak x strong.AbstrakPenelitian ini bertujuan mengungkapkan stuktur narasi dalam novel Ca Bau Kan (CBK) dengan menggunakan teori semiotika. Penelitian ini menggunakan sumber data novel CBK karya Remy Silado yang diterbitkan oleh KPG, cetakan kedelapan tahun 2004. Pengumpulan data dilaksanakan dengan teknik kepustakaan. Teori yang digunakan dalam penelitian ini adalah teori semiotika, khususnya analisis sastra menurut Subur Laksono Wardoyo bahwa analisis teks prosa dapat dilakukan melalui tiga tahap, yaitu: analisis skema naratif dasar, analisis signifier utama, dan analisis sintagmatik-paradigmatik. Hasil penelitian menggambarkan bahwa struktur narasi pada novel CBK adalah sebagai berikut: 1) kehidupan Tinung sebelum menjadi ca bau kan, 2) kehidupan Tinung sebagai ca bau kan, dan 3) kehidupan Tinung setelah tidak menjadi ca bau kan. Berdasarkan struktur narasi, maka didapatkan bahwa “Cinta cuma satu, kagak perlu takaran” merupakan penanda utama dan dapat diperjelas melalui analisis sintagmatik-paradigmatik yang didasarkan atas sebuah oposisi biner lemah x kuat.

Background Single-port laparoscopic surgery (SPLS) was increasingly used on several surgical diseases. The aim of this study is evaluation of the results of the suture of the perforation by SPLS for the perforated duodenal ulcer treatement. Methods From January 2012 to July 2014, 35 patients with perforated duodenal ulcers underwent simple suture of the perforations by SPLS at Hue University Hospital and Hue Central Hospital. Results The mean age was 45.9 ± 14.4 years. The sex ratio (male/female) was 16.5 and the mean of BMI was 19.2 ± 2.3. There was one patient (2.9%) with previous history of laparoscopic repair of perforated duodenal ulcer. The duration of the symptoms was 9.9 ± 12.3 hours. 97.1% of patients had the perforations of the anterior wall of the duodenum. The mean size of the perforation was 4.7 ± 3.4 mm (2 – 22mm). 2.9% of patients had the perforations of the posterior wall of the duodenum. The rate of the conversion to the open surgery was 2.9%. The mean operative time was 75.8 ± 33.7min, and the mean hospital stay was 5.8 ± 1.4 days. The mean of the analgesic requirement time was 2.9 ± 0.8 days. The wound length was 1.9 ± 0.1 cm. There was 5.9% of the patients had wound infection. There was no operation-related mortality. Conclusions Simple suture of the perforation by single-port laparoscopic surgery is a feasible and safe procedure, and it may be a scarless surgical technique for perforated duodenal ulcers treatement. Introduction: Since Longo First described it in 1998, Stapled Hemorrhoidectomy has been emerging as the procedure of choice for symtomatic hemorrhoid. Several studies have shown it to be a safe, effective and relative complication free procedure. The aim of this study was to determine the suitability of (SH) as a day cas procedure at Hue University Hospital. Methods: From Decembre 2009 to April 2012, 384 patients with third- degree and fourth-degree hemorrhoids who underwent Stapled Hemorrhoidectomy were included in this study. Parameters recorded included postoperative complications, analegic requirements, duration of hospital stay and patient satisfaction. Follow-up was performed at 1 month and 3 months post-operative. Results: Of the 384 patients that underwent a Stapled Hemorrhoidectomy 252 (65,7%) were male and 132 (34,3%) were female. The mean age was 47,5 years (range 17-76 years. Duration of hospital stay: The mean day was 2,82 ± 1,15 days (range 1-6 days). There were no perioperative complications. There was one case postoperative complication: hemorrhage; Follow-up after surgery: 286 (74,4%) patients had less anal pain, 78 (20,3%) patients had moderate anal pain, 3 (0,8%) patients had urinary retention; Follow-up after one month: good for 325 (84,6%) patients, average for 59 (15,4%) patients; Follow-up after three months: good for 362 (94,3%) patients, average for 22 (5,7%) patients. Conclusion: Our present study shows that Stapled Hemorrhoidectomy is a safe, reduced postoperative pain, shorter hospital stay and a faster return to unrestricted daily activity

Background: Conventional three–port laparoscopic appendectomy is becoming popular for the treatment of acute appendicitis. In this report, we present the early results of a new technique of laparoscopic appendectomy conducted through a single-port. Patients and Methods: From March 2011 to October 2013, we have performed 86 operations Single Port Laparoscopic Appendectomy at Hue University Hospital and Hue Central Hospital. SILS Port (Covidien) is used, it can be performed with basic laparoscopic instruments. Results: In this study, 86 patients underwent Single-Port laparoscopic appendectomy, among them 52.33% were femele, 47.67% were male, female/male ratio was 1.09. The mean age was 33.09. An orther trocar insertion was required in 2 patients (2.33%). The mean operation time was 42.03 minutes and mean postoperative hospital stay 3.37 days. Postoperative complication occurred in 2 case (2.33%) was of omphalitis. During 2 weeks follow up, 2 case (2.33%) was of omphalitis. Conclusions: Single - port intracorporeal appendectomy is a safe, minimal invasive procedure with excellent cosmetic results. Key words: Single Port Laparoscopic Appendectomy, appendectomy
