According to the national railway network construction plan, Investment in railways has increased due to the need for environmentally friendly transportation, and the rail network is expanding throughout South Korea. Railway projects should be evaluated using strategic environmental impact assessments. In the “Guidelines for the Construction of Environment-friendly Railways,” seven priority headings that must be considered for railway projects are described. This guide notes that qualitative evaluation must be conducted during the survey process to reasonably predict impacts on the environment. However, quantitative evaluation with specific indicator values may also be necessary. In this study, independence analysis and logistic regression analysis were used to quantitatively evaluate railway environmental and ecological indicators. The results were used to develop a regression model reflecting seven indicators; biodiversity class, ecosystem type, vegetation conservation class, tree age class, ecological naturalness, presence of river ecosystems, and fragmented patch size. The fitness regression model showed 90.3% classification accuracy and the receiver operating curve (ROC) model fit was 88.6%. An environmental quality assessment map was prepared by classifying areas of environmental quality according to five grades. This is the first model for environmental and ecological evaluation of railway projects. Evaluation using the map showed that the railroad passes through areas with lower protection values compared to the results obtained using the national environmental evaluation map. Kappa analysis showed a low level of agreement between the two maps (kappa coefficient = 0.212). The results of this study can be applied to railway development project sites and may help to identify the best sites for the development of an environmentally friendly railway system.

Background: Due to the lack of a gold standard diagnostic test, reference centres with experienced personnel and costly procedures are needed for primary ciliary dyskinesia (PCD) diagnostics. Diagnostic flowcharts always start with clinical symptoms. Therefore, the aim of this work is to define differential clinical criteria so that only patients clinically compatible with PCD are referred to reference centres. Materials and methods: 18 variables from 476 Mediterranean patients with clinically suspicious PCD were collected. After analysing cilia function and ultrastructure, 89 individuals were diagnosed with PCD and 387 had a negative diagnosis. Simple logistic regression analysis, considering PCD as a dependent variable and the others as independent variables, was done. In order to define the variables that best explain PCD, a step-wise logistic regression model was defined. Aiming to classify individuals as PCD or PCD-like patients, based on variables included in the study, a classification and regression tree (CART) was designed. Results and conclusions: Simple logistic regression analysis shows statistically significant association between age at the beginning of their symptomatology, periodicity, fertility, situs inversus, recurrent otitis, atelectasis, bronchiectasis, chronic productive cough, rhinorrea, rhinusinusitis and recurrent pneumonias, and PCD. The step-wise logistic regression model selected situs inversus, atelectasis, rhinorrea, chronic productive cough, bronchiectasis, recurrent pneumonias, and otitis as PCD predictive variables (82% sensitivity, 88% specificity, and 0.92 Area Under the Curve (AUC)). A decision tree was designed in order to classify new individuals based on pansinusitis, situs inversus, periodicity, rhinorrea, bronchiectasis, and chronic wet cough.

Failure of women to undergo a successful first child delivery is becoming one of the most challenging problem and a major concern to most of our healthcare providers. In this paper, we apply the binary logistic regression analysis to investigate whether age of women at first birth have a relationship with the outcome of their delivery (Success or failure). The data was collected from Gombe Town Maternity and was subjected to analysis. From the result of the analysis, we observed that most of the women at tender age (12-17) are classified to fail (69%) during their first child delivery while most of the women at higher age (19 and above) have a better chance of succeeding during their first parturition. Also, the result shows that the average age at which women ought to conceive successfully is 19 years. The Wald statistics result also shows that the logistic regression model fits the data very well.

Background and purpose: The discrimination of vertebral artery (VA) occlusive dissection from other etiologies is critical for acute stroke management but sometimes difficult. We aimed to assess the factors associated with VA occlusive dissection and develop the discrimination score. Methods: We examined consecutive patients with acute posterior ischemic stroke due to unilateral VA occlusion from our prospective stroke registry between 2012 and 2019. Unilateral VA occlusion was confirmed by magnetic resonance angiography and cervical ultrasonography. The diagnosis of VA dissection was based on the magnetic resonance imaging or digital subtraction angiography. Dissection score was developed from associated factors to discriminate VA occlusive dissection by logistic regression analysis. Discriminative performance was analyzed by receiver operating curve (ROC) analysis. Results: Consecutive 84 patients (70±13 years; male, 77%) involved 16 (19%) with VA occlusive dissection. On logistic regression analysis, each of younger age ( ≤ 70 years), absence of hypertension, absence of dyslipidemia, head or neck pain, medullary infarction and non-dominance side VA occlusion were significantly associated with VA occlusive dissection (Table). Dissection score was created with these factors by assigning respective points based on the corresponding regression coefficients, and the score were ranged from 0 to 9 (Table). High discriminative performance for VA occlusive dissection was observed (area under the curve: 0.91) and optimal cut-off value was 5 or more (accuracy, 79%; sensitivity, 94%; specificity, 75%). Conclusions: In patients with acute posterior ischemic stroke due to unilateral VA occlusion, dissection score had high discriminative performance for diagnosing VA dissection.

PurposeThe paper sought to make contribution to youth entrepreneurship research field. This is because whilst youth entrepreneurship presents enormous socio-economic benefits, including economic growth, diversification, innovation and poverty elimination earlier research have paid more attention to adults' entrepreneurship with less attention been paid to youth entrepreneurship resulting in a compelling research gap following the present huge youth unemployment across the world. Consequently, the motivation of the paper is to guide public policy and practice on the promotion of youth entrepreneurship, especially in developing countries.Design/methodology/approachThe study employs the survey research design based on logistic regression analysis as the key analytical technique to examine data. The choice of the logistic regression model is due to the fact that the main research question that informs the study is a dichotomous one. Hence it was found appropriate to select the logit regression model based on similar works in the field.FindingsOver all, the results show that lack of financial support from one's family background, early entry into formal employment, as well as being born into entrepreneurial dominated families significantly predict the probability of a youth considering entrepreneurship as an attractive life venture.Research limitations/implicationsThe study is based on non-probability sampling method and so readers must bear that in mind when they are interpreting the results.Practical implicationsFollowing from the findings, one of the practical implications is that youth entrepreneurs must align the external influences to the internal capacity of the businesses to initiate and/or start sustainable entrepreneurial ventures.Originality/valueThe study sheds light from an under-explored and new geographical context to advance existing knowledge in the field.

Antenatal care is a service provided to pregnant women to monitor, support maternal health and detect mothers whether normal or problematic pregnant women. According to the WHO, globally more than 70% of maternal deaths are caused by complications of pregnancy and childbirth such as hemorrhage, hypertension, sepsis, and abortion. Based on data obtained from the profile of the North Sumatra provincial health office in 2017, in the city of Padangsidimpuan in 2017 the coverage of ANC visits reached (76.58%) and had not reached the target in accordance with the 2017 Provincial Health Office strategy plan (95%). This type of research was an observational analytic study with a cross sectional design. The population in this study were independent practice midwives who were in the Padangsidimpuan, the sample in this study amounted to 102 respondents. The technique of collecting data used questionnaires and data analysis used univariate, bivariate and multivariate analysis with logistic regression analysis. Based on bivariate analysis showed that there was a relationship between facilities, knowledge and attitudes of independent midwives with compliance with the standards of antenatal care services with a value of p <0.05. The results of the study with multivariate logistic regression analysis showed that the factors associated with the compliance of independent midwives in carrying out antenatal care service standards were attitudes with values (p = 0.026).

Abstract Background Postanesthetic shivering is an unpleasant adverse event in surgical patients. A nonsteroidal anti-inflammatory drug has been reported to be useful in preventing postanesthetic shivering in several previous studies. The aim of this study was to evaluate the efficacy of flurbiprofen axetil being a prodrug of a nonsteroidal anti-inflammatory drug for preventing postanesthetic shivering in patients undergoing gynecologic laparotomy surgeries. Method This study is a retrospective observational study. I collected data from patients undergoing gynecologic laparotomy surgeries performed between October 1, 2019, and September 30, 2020, at Kumamoto City Hospital. All the patients were managed with general anesthesia with or without epidural analgesia. The administration of intravenous 50 mg flurbiprofen axetil for postoperative pain control at the end of the surgery was left to the individual anesthesiologist. The patients were divided into two groups: those who had received intravenous flurbiprofen axetil (flurbiprofen group) and those who had not received intravenous flurbiprofen axetil (non-flurbiprofen group), and I compared the frequency of postanesthetic shivering between the two groups. Additionally, the factors presumably associated with postanesthetic shivering were collected from the medical charts. Intergroup differences were assessed with the χ2 test with Yates’ correlation for continuity category variables. The Student’s t test was used to test for differences in continuous variables. Furthermore, a multivariate logistic regression analysis was performed to elucidate the relationship between the administration of flurbiprofen axetil and the incidence of PAS. Results I retrospectively examined the cases of 141 patients aged 49 ± 13 (range 21-84) years old. The overall postanesthetic shivering rate was 21.3% (30 of the 141 patients). The frequency of postanesthetic shivering in the flurbiprofen group (n = 31) was 6.5%, which was significantly lower than that in the non-flurbiprofen group (n = 110), 25.5% (p value = 0.022). A multivariate logistic regression analysis showed that administration of flurbiprofen axetil was independently associated with a reduced incidence of postanesthetic shivering (odds ratio 0.12; 95% confidence interval, 0.02-0.66, p value = 0.015). Conclusions My result suggests that intraoperative 50 mg flurbiprofen axetil administration for postoperative pain control is useful to prevent postanesthetic shivering in patients undergoing gynecologic laparotomy surgeries.

Introduction: Hip fractures are a significant economic burden to our healthcare system. As there have been efforts made to create an alternative payment model for hip fracture care, it will be imperative to risk-stratify reimbursement for these medically comorbid patients. We hypothesized that patients readmitted to the hospital within 90 days would be more likely to have a recent previous hospital admission, prior to their injury. Patients with a recent prior admission could therefore be considered higher risk for readmission and increased cost. Methods: A retrospective chart review identified 598 patients who underwent surgical fixation of a hip or femur fracture. Data on readmissions within 90 days of surgical procedure and previous admissions in the year prior to injury resulting in surgical procedure were collected. Logistic regression analysis was used to determine if recent prior admission had increased risk of 90-day readmission. A subgroup analysis of geriatric hip fractures and of readmitted patients were also performed. Results: Having a prior admission within one year was significantly associated (p < 0.0001) for 90-day readmission. Specifically, logistic regression analysis revealed that a prior admission was significantly associated with 90-day readmission with an odds ratio of 7.2 (95% CI: 4.8-10.9). Discussion: This patient population has a high rate of prior hospital admissions, and these prior admissions were predictive of 90-day readmission. Alternative payment models that include penalties for readmissions or fail to apply robust risk stratification may unjustly penalize hospital systems which care for more medically complex patients. Conclusions: Hip fracture patients with a recent prior admission to the hospital are at an increased risk for 90-day readmission. This information should be considered as alternative payment models are developed for hip fracture care.
