Abstract Purpose Polysomnography (PSG) is considered the best objective study to diagnose and quantify sleep disorders. However, PSG involves multiple electrodes and is usually performed in a sleep laboratory that in itself may change the physiology of sleep. One of the parameters that can change during PSG is the sleep position, leading to more supine sleep. The aim of this study was to quantify the amount of supine sleep during PSG and compare it to consecutive nights of a home sleep apnea test (HSAT) in the same patients. Methods This prospective study evaluated 22 consecutive patients undergoing PSG followed by HSAT. Sleep position was analyzed during PSG and subsequently on 2 to 6 nights (mean 3.7 nights) at home, and the amount of supine sleep was recorded during each night. Results Of 22 patients, there were 12 men (55%). The median age was 60.0 years for women and 45.5 years for men. Median proportion of supine sleep during PSG and HSAT was 61% and 26% (p < 0.001), respectively. Four “phenotypes” were identified according to their sleep position during PSG and HSAT, with 5 patients sleeping mainly supine during all nights, 7 patients sleeping mainly non-supine during all nights, 3 patients sleeping in different positions during each night, and 7 patients sleeping supine during PSG but non-supine at home, during HSAT. Conclusions There is a higher proportion of supine sleep during PSG compared to home sleep. We identified a subgroup of patients who slept mainly supine during PSG and mainly non-supine during HSAT. PSG may overestimate OSA severity in a specific phenotype of patients.

The article examines in detail the history of the relationship between A.P. Suslova and V.V. Rozanov in connection with the notion existing in the historical and literary science that Rozanovs marriage to Suslova was based on his deep interest in the work of F.M. Dostoevsky and his desire in such an unusual way to penetrate deeper into the secrets of the life and work of the author of The Idiot and The Brothers Karamazov. However, an appeal to various documentary evidence shows that Rozanovs marriage motives came from the warehouse of his nature and constituted a complex of rather complex reasons, among which the human, and not literary, research principle still dominates. The desire for a benevolently objective study of the life history of A.P. Suslova makes it possible to clarify at the modern scientific level the important facts of the biography of F.M. Dostoevsky and V.V. Rozanov, to free them from the stratifications of legends and myths.

Background : In many people the disease is mild and self-limiting, but in a considerable portion of patients the disease is severe and fatal. Determining which patients are at high risk of severe illness or mortality is an essential part of understanding this illness Aims and Objective : Study of correlation of prognostic risk score with mortality in patients admitted to RLJH and RC ICU with coronavirus disease 2019 (COVID-19) . Methodology : This was a Retrospective observational study in the patients tested RT-PCR positive for COVID 19 and admitted at SRI DEVARAJ URS MEDICAL COLLEGE Tamaka, Kolar during the two month duration i.e. June 2021 to July 2021 in the 50 patients .The statistical analysis was done by Graph pad prism 6 version from that we have calculated correlation co-efcient and Survival graph Result : In our study we have seen that The majority of the patients were in the age group of 61-70 were 28% , followed by 51-60 were 24%, 71-80 were 22%, 41-50 were 14% , 30-40 & >80 were 6%. The majority of the patients were Male i.e. 68% and Female were 32%. There was strong correlation between Prognostic score and mortality of the patients which was statistically highly signicant i.e. Spearman r= 0.8450, P value (two-tailed) was < 0.0001*** . survival rate for the Score 0, 2 was almost 100% and as the score increases i.e. 3,4,5,6 there survival percentage also decrease approximately to 25 %, 5% and 0%,0% respectively. Conclusion :It can be concluded from our study that there was signicantly higher coreltaion between the Prognostic score and mortality of the patients hence this score could be reliable tool for the assessment of the severe patients and prompt and aggressive treatment for the better outcome

The problems of preserving the dermatovenerological health of the population are extremely relevant for Ukraine, where diseases of the skin and subcutaneous tissue occupy the 5th place in the structure of morbidity. Objective — study of the structure of the dermatovenerological service in the context of reforming the healthcare system at this stage (end of 2021) and its characteristics. Materials and methods. The results of the analysis of the state of resources of medical institutions of communal ownership of dermatovenerological profile in Ukraine are presented. Results and discussion. Today, in Ukraine, the number of hospital beds for 24-hour hospitals for adults is 188 in the regions (Zaporizhzhia, Rivne, Kharkiv, Cherkasy) and 220 beds in Kyiv city, which is extremely insufficient. There are 24 beds for the pediatric patients in the regions (Luhansk, Rivne, Cherkasy) and in the city of Kyiv — 60. The number of day hospital beds for adults has significantly decreased — to 248 in the regions (Dnipropetrovs’k, Zaporizhzhia, Kyiv, Lviv, Poltava, Kherson, Cherkasy, Chernivtsi and Kramatorsk city), for pediatric patients — 61 (Dnipropetrovs’k, Kropyvnytskyi, Mykolaiv, Poltava regions). In 2021, 11 dermatovenerologic dispensaries were liquidated. Instead of them, dermatovenerologic centers and subdivisions subordinate to regional hospitals were formed. Thus, there is an unfavorable restructuring of the bed fund of institutions providing dermatovenerological care. Conclusions. A decrease in the number of state and municipal institutions providing free dermatovenerological care will lead to the worsening of the epidemic situation, the spread of sexually transmitted infections and reduction of the availability of specialized dermatovenerological care for the population in a difficult economic and social situation.
