Summary Introduction: This article is part of the Focus Theme of Methods of Information in Medicine on the German Medical Informatics Initiative. HiGHmed brings together 24 partners from academia and industry, aiming at improvements in care provision, biomedical research and epidemiology. By establishing a shared information governance framework, data integration centers and an open platform architecture in cooperation with independent healthcare providers, the meaningful reuse of data will be facilitated. Complementary, HiGHmed integrates a total of seven Medical Informatics curricula to develop collaborative structures and processes to train medical informatics professionals, physicians and researchers in new forms of data analytics. Governance and Policies: We describe governance structures and policies that have proven effective during the conceptual phase. These were further adapted to take into account the specific needs of the development and networking phase, such as roll-out, carerelated aspects and our focus on curricula development in Medical Inform atics. Architectural Framework and Methodology: To address the challenges of organizational, technical and semantic interoperability, a concept for a scalable platform architecture, the HiGHmed Platform, was developed. We outline the basic principles and design goals of the open platform approach as well as the roles of standards and specifications such as IHE XDS, openEHR, SNOMED CT and HL7 FHIR. A shared governance framework provides the semantic artifacts which are needed to establish semantic interoperability. Use Cases: Three use cases in the fields of oncology, cardiology and infection control will demonstrate the capabilities of the HiGHmed approach. Each of the use cases entails diverse challenges in terms of data protection, privacy and security, including clinical use of genome sequencing data (oncology), continuous longitudinal monitoring of physical activity (cardiology) and cross-site analysis of patient movement data (infection control). Discussion: Besides the need for a shared governance framework and a technical infrastructure, backing from clinical leaders is a crucial factor. Moreover, firm and sustainable commitment by participating organizations to collaborate in further development of their information system architectures is needed. Other challenges including topics such as data quality, privacy regulations, and patient consent will be addressed throughout the project.

Summary Introduction: This article is part of the Focus Theme of Methods of Information in Medicine on the German Medical Informatics Initiative. Similar to other large international data sharing networks (e.g. OHDSI, PCORnet, eMerge, RD-Connect) MIRACUM is a consortium of academic and hospital partners as well as one industrial partner in eight German cities which have joined forces to create interoperable data integration centres (DIC) and make data within those DIC available for innovative new IT solutions in patient care and medical research. Objectives: Sharing data shall be supported by common interoperable tools and services, in order to leverage the power of such data for biomedical discovery and moving towards a learning health system. This paper aims at illustrating the major building blocks and concepts which MIRACUM will apply to achieve this goal. Governance and Policies: Besides establishing an efficient governance structure within the MIRACUM consortium (based on the steering board, a central administrative office, the general MIRACUM assembly, six working groups and the international scientific advisory board), defining DIC governance rules and data sharing policies, as well as establishing (at each MIRACUM DIC site, but also for MIRACUM in total) use and access committees are major building blocks for the success of such an endeavor. Architectural Framework and Methodology: The MIRACUM DIC architecture builds on a comprehensive ecosystem of reusable open source tools (MIRACOLIX), which are linkable and interoperable amongst each other, but also with the existing software environment of the MIRACUM hospitals. Efficient data protection measures, considering patient consent, data harmonization and a MIRACUM metadata repository as well as a common data model are major pillars of this framework. The methodological approach for shared data usage relies on a federated querying and analysis concept. Use Cases: MIRACUM aims at proving the value of their DIC with three use cases: IT support for patient recruitment into clinical trials, the development and routine care implementation of a clinico-molecular predictive knowledge tool, and molecular-guided therapy recommendations in molecular tumor boards. Results: Based on the MIRACUM DIC release in the nine months conceptual phase first large scale analysis for stroke and colorectal cancer cohorts have been pursued. Discussion: Beyond all technological challenges successfully applying the MIRACUM tools for the enrichment of our knowledge about diagnostic and therapeutic concepts, thus supporting the concept of a Learning Health System will be crucial for the acceptance and sustainability in the medical community and the MIRACUM university hospitals.

Objetivo: Analisar como a literatura científica descreve o enfermeiro, bem como seu papel no Programa de Gerenciamento do Uso de Antimicrobianos (PGUA). Método: Revisão integrativa da literatura, realizada mediante consulta às bases de dados Scientific Electronic Library Online (SciELO), Literatura Latinoamericana e do Caribe em Ciências da Saúde (LILACS) e Medical Literature Analysis and Retrieval System Online (MEDLINE). Resultados: Foram incluídos seis artigos que atenderam os critérios de inclusão previamente estabelecidos. Todos foram publicados na língua inglesa e divulgados na base de dados eletrônica MEDLINE. A metade dos artigos foi publicada em revistas científicas que tiveram fator de impacto avaliado acima de 2,0, destacando os periódicos Clinical Infectious Diseases (8,216), BMC Medical Informatics and Decision Making (2,288) e American Journal of Infection Control (2,209). Quanto aos temas abordados, todos os artigos reconheceram o enfermeiro como peça fundamental no PGUA. Conclusão: Embora os estudos na literatura sejam limitados, foi possível concluir que a participação do enfermeiro promove otimização terapêutica e uso racional dos antibióticos, contribuindo para redução da resistência antimicrobiana.

The chapter describes the approach for the South Tyrolean city of Meran in the creation of use cases and the implementation of an urban agenda (roadmap) for the development of the city from a smart city perspective, with the involvement of citizens, experts, and local administrators. A list of key services, based on a technical and economic pre-feasibility study and social impact assessment, has been developed and will be able to be provided through a digital platform. In particular, the example of the concrete development of a use case about public lighting is presented in detail: starting from the identification of the use case to the execution of the installation of 100 intelligent streetlights and 5 test-sites for the monitoring of water consumption up to the visualization of the collected data.

This article focuses on Makerere University and Kyambogo University to highlight stakeholders’ concerns pertaining to the shared governance framework for public universities in Uganda. It is anchored in the interpretive lens and the data was derived from three state-sponsored reports on the two public universities. The secondary data were analyzed using content analysis. The findings demonstrate that the size, composition, authority, and effectiveness of the university council and the senate and the mode through which leaders assume office, are the salient governance concerns in the two universities. The results further show that, stakeholders’ concerns regarding the current shared governance framework for public universities relate to the substance of the framework rather than its form or the framework itself. This suggests that the governance framework comprising the university council, the university senate and the administration is fit for purpose and in sync with the nature of the academy. Arguably, these governance organs (and the nature of their work) set a university apart from other organisations. Finally, without being prescriptive, the article sketches options for reform.Le présent article se concentre sur l’Université de Makerere et l’Université de Kyambogo pour mettre en lumière l’inquiétude des parties prenantes au sujet du modèle de gouvernance partagée dans les universités publiques en Ouganda. Il adopte un point de vue interprétativiste et s’appuie sur des données issues de trois rapports commandités par l’Etat sur ces deux universités publiques. Les données secondaires ont été traitées par une analyse de contenu. Les résultats démontrent que la taille, la composition, l’autorité et l’efficacité du conseil universitaire et du sénat de l’université, et la façon dont les dirigeants exercent leurs fonctions sont les principaux sujets d’inquiétude en ce qui concerne la gouvernance de ces deux universités. Les résultats démontrent également que les inquiétudes des parties prenantes au sujet du modèle actuel de gouvernance partagée des universités publiques porte sur le contenu de ce modèle, plus que sur sa forme ou sur le modèle lui-même. Cela montre que le modèle de gouvernance qui comprend le conseil universitaire, le sénat de l’université et l’administration répond aux attentes et est en cohérence avec l’environnement académique.On peut considérer que ces organes de gouvernance (et la nature de leur travail) différencient l’université d’autres types d’organisations. Enfin, sans aucune intention normative, cet article ébauche des possibilités de réforme.

Edge computing is a distributed computing paradigm such that client data are processed at the periphery of the network, as close as possible to the originating source. Since the 21st century has come to be known as the century of data due to the rapid increase in the quantity of exchanged data worldwide (especially in smart city applications such as autonomous vehicles), collecting and processing such data from sensors and Internet of Things devices operating in real time from remote locations and inhospitable operating environments almost anywhere in the world is a relevant emerging need. Indeed, edge computing is reshaping information technology and business computing. In this respect, the paper is aimed at providing a comprehensive overview of what edge computing is as well as the most relevant edge use cases, tradeoffs, and implementation considerations. In particular, this review article is focused on highlighting (i) the most recent trends relative to edge computing emerging in the research field and (ii) the main businesses that are taking operations at the edge as well as the most used edge computing platforms (both proprietary and open source). First, the paper summarizes the concept of edge computing and compares it with cloud computing. After that, we discuss the challenges of optimal server placement, data security in edge networks, hybrid edge-cloud computing, simulation platforms for edge computing, and state-of-the-art improved edge networks. Finally, we explain the edge computing applications to 5G/6G networks and industrial internet of things. Several studies review a set of attractive edge features, system architectures, and edge application platforms that impact different industry sectors. The experimental results achieved in the cited works are reported in order to prove how edge computing improves the efficiency of Internet of Things networks. On the other hand, the work highlights possible vulnerabilities and open issues emerging in the context of edge computing architectures, thus proposing future directions to be investigated.

SummaryThis article is part of the Focus Theme of Methods of Information in Medicine on the German Medical Informatics Initiative. The Medical Informatics Initiative (MII) was launched within the scope of the German Federal Ministry of Education and Research’s (BMBF) Medical Informatics Funding Scheme, with the goal of developing infrastructure for the integration of clinical data from patient care and medical research in Germany. Its work is to be performed over the course of a decade (2016–2025) across three funding phases, with the first two concentrating on university hospitals. During the conceptual phase (now concluded), a central supporting project ensured coordination – and laid the ground for standardised solutions for all the initiative’s sites and scientific consortia that will enable effective data use and exchange, both for health care as well as research. The conceptual phase focused on the following: a) interoperability, through the consistent use of international standards (from an early stage, i.e. primary IT systems in patient care); b) standardised templates for patient consent and harmonised data protection; and c) standard rules for data use and access (monitoring and safeguarding access to data). On this basis, the initiative aims in the long term to improve medical research (particularly health care research, using data from treatments), to accelerate the transfer of knowledge from research to patient care – and to provide important impetus for the digitalization of medicine in Germany.
