Today, brain attack disorders are one of the most life-threatening areas in the medical era, which the mankind is facing nowadays. Globally, more than 10, 00,000people are subjected to brain attack disorders like hemiplegia and tremor, every year, where two-thirds of them survive. Among the survival community, more than 80 per cent of them are subjected to long-term impairment of their upper extremity. In order to treat the impairment, the survival group is subjected to medications and rehabilitation in order to improve their daily living. But the facilities are very limited in fast- developing countries like India when compared to western standards. The rehabilitation given in corresponding with medications during the treatment period in hospitals does not give a complete recovery from disability. People from rural background could not meet their rehabilitation requirements even in the hospital during treatment and also when they are discharged to home after treatment from hospitals due to financial constraints and reachability. In order to motivate the survival group to fulfill their daily living and improve their lifestyle, this paper is focused on intelligent home-based rehabilitation system at low cost, reliability, and affordability. One major movement disorder namely Upper Arm Hemiplegia was taken into account and visited few major hospitals around Coimbatore and Chennai for literature and case study. The facilities available in various hospitals and their drawbacks were analyzed. Based on the studies conducted at hospitals and taking advice from therapists, an innovative low-cost home- based rehabilitation device using Electro-Mechanical systems has been developed to support samples who were used to impaired living even after treatments. To support Upper Arm Hemiplegia patients, the devices which were developed and experimented with Fugl Meyer Assessment to analyse the improvements in different functionalities are discussed in this paper.

This paper describes a wearable upper limb orthosis that could be integrated within a home-based rehabilitation system for stroke patients. This orthosis consists of links and the installation of springs. First, the advantages of this design are with no actuating motors, with simple structure, and with lightweight, but with similar benefit as other therapeutic robots. Second, through the installation of springs to provide with spring force, when patients wearing this upper limb orthosis will feel the resistance, to benefit for more effective muscle rehabilitation exercise. At last, the advantage of this design is that due to the installation of springs and springy clamp one, users wearing the upper do rehabilitation training according with human skeleton movement trajectory, which makes users comfortable and that contributes to biceps, triceps, and deltoid muscles on the upper arm and shoulder training.

We aimed to understand the relationships between care recipients’ profiles and home- and community-based services (HCBS use patterns. Data were from the 2010 to 2013 Long-Term Care Service Management System in Taiwan ( N = 78,205). We used latent class analysis and multinomial logistic regression analyses. Three HCBS use patterns were found. Care recipients who lived alone, lived in less urbanized areas, and had instrumental activities of daily living disabilities were more likely to be in the home-based personal care group. Those in the home-based personal and medical care group were more likely than others to have a primary caregiver. Care recipients who had poorer abilities at basic activities of daily living and cognitive function, better household income, and lived in a more urbanized area were more likely to be in the non-personal care multiple services group. The findings suggest that policymakers alleviate barriers to accessing various patterns of HCBS should be encouraged.

Rehabilitation of the upper limb is an important aspect of the therapy for people affected by neuromotor diseases for the recovery of the capability to perform activities of daily living (ADLs). Nonetheless, the costs associated with the administration of rehabilitation therapy and the increasing number of patients highlight the need for new solutions. Technology-based solutions and, in particular, telerehabilitation could strongly impact in this field. In this paper, a new system based on radiofrequency (RF) technology is presented which is able to effectively provide home-based telerehabilitation and extract meaningful information on the therapy execution performance. The technology has been tuned to the needs of the rehabilitation system, optimizing the hardware, the communication protocol and the software control. A methodology for extracting the execution time of the rehabilitation tasks, the distance covered by the patient’s hand in each subtask and the velocity profile is presented. The results show that a highly usable system for the rehabilitation of the upper limb has been developed using the RF technology and that performance metrics can be reliably extracted by the acquired signals.

Abstract Objective Chronic obstructive pulmonary disease (COPD) can be prevented and treated, although presenting with persistent airflow restriction; the airflow restriction caused by COPD is mostly progressive. In recent years, more attention has been paid to the home-based pulmonary rehabilitation (PR) and its influence on COPD. Exercise training is the basic constituent of PR. However, it is not clear which exercise trainings are the ideal ways to deliver home-based PR. Methods In this review, we focus on the effect of home-based exercise training on patients with COPD. We searched literature, which was necessarily required to be randomized controlled trails (RCTs) from the establishment of the four respective databases (Medline, PubMed, Web of Science, and China National Knowledge Infrastructure) from January 2008 to January 2018. We used the Cochrane collaborative “risk of bias” tool to assess the quality of evidence. A total of 21 trials (1694 participants) were included. Through the analysis of the literature, we find that a simple, low-cost, and low-intensity family-based lung-rehabilitation plan to adapt to the real life may lead to the improvement of the ability to exercise, the reduction of the difficulty in breathing, and the improvement of carrying out daily activities. Results In the exercise training of home-based PR, lower limb exercise (LLE) training demonstrated a more perceptible effect in improving the quality of life of patients with COPD. At the same time, the combination of LLE training, breathing training, and upper limb exercise training is more obvious than the simple LLE training. In addition, home-based low-intensity aerobic training may sometimes be no less than the outpatient or center intervention to improve dyspnea, health status, and exercise tolerance. In conclusion, the simple and easy home-based PR exercise program is useful. Long-term home-based PR may require an enhanced need for maintenance. Conclusions A simple, low-cost, and low-intensity high blood pressure response (HBPR) plan to adapt to the real life may lead to an augmentation in the ability to exercise, a reduction of the difficulty in breathing, and an improvement in carrying out day-to-day activities. HBPR strategies can benefit patients (elderly patients with COPD at home) in the long term.

The COVID-19 pandemic has highlighted the need for advancing the development and implementation of novel means for home-based telerehabilitation in order to enable remote assessment and training for individuals with disabling conditions in need of therapy. While somatosensory input is essential for motor function, to date, most telerehabilitation therapies and technologies focus on assessing and training motor impairments, while the somatosensorial aspect is largely neglected. The integration of tactile devices into home-based rehabilitation practice has the potential to enhance the recovery of sensorimotor impairments and to promote functional gains through practice in an enriched environment with augmented tactile feedback and haptic interactions. In the current review, we outline the clinical approaches for stimulating somatosensation in home-based telerehabilitation and review the existing technologies for conveying mechanical tactile feedback (i.e., vibration, stretch, pressure, and mid-air stimulations). We focus on tactile feedback technologies that can be integrated into home-based practice due to their relatively low cost, compact size, and lightweight. The advantages and opportunities, as well as the long-term challenges and gaps with regards to implementing these technologies into home-based telerehabilitation, are discussed.
