PORTABLE TELEMEDICAL COMPLEX USAGE PERSPECTIVES FOR MEDICAL CARE OF ELDERLY AND SENILE PATIENTS WITH CEREBROVASCULAR PATHOLOGY

В статье отражены перспективы оказания медицинской помощи пациентам пожилого и старческого возраста с цереброваскулярной патологией с использованием портативного телемедицинского комплекса. Проведен анализ телемедицинского взаимодействия врач-консультант-пациент для оказания медицинской помощи, обсуждены критерии выбора медицинских приборов в составе телемедицинского комплекса, организационно-правовые аспекты передачи, хранения и доступа к медицинской информации, приведены важные клинические особенности цереброваскулярной патологии у пациентов старших возрастных групп, определяющие необходимость привлечения высококвалифицированных специалистов, а также возможности использования разрабатываемого портативного комплекса. The article discusses perspectives of medical care in elderly and senile patients with cerebrovascular pathology with portable telemedical complex use. Analysis of telemedical cooperation physician-consultant-patient and discussion of medical equipment criteria for telemedical complex selection and organizational together with legal issues of medical data transfer, storage and access were performed. The important clinical features of cerebrovascular pathology in elderly patients were presented, which determined necessity of highly qualified medical specialist involvement in management of such patients. The opportunities of portable medical complex usage were discussed.

It discusses a pilot project being carried out at ‘Policlinico Gemelll’ hospital of Rome, which enables patients to achieve the maximum level of independence and a satisfactory quality of life. Urgency of the research. The principles of telemedicine need to be put into practice of emergency medical care and family medicine, so there is a need for diagnostic medical equipment with the ability of data transfer and remote consulting with subject matter experts. Target setting. Currently, ECG testing consumes the time to prepare the device for work, needs for the personal computer as the additional equipment and requires highly skilled staff. The development of portable electrocardiographic equipment with built-in means of communication is a task of current importance for biomedical engineering. Actual scientific researches and issues analysis. The current researches in the development of portable cardiograph for primary preclinical diagnostics are focused on the choice of optimal design parameters of device and the incensement of noise immunity of medical equipment. Uninvestigated parts of general matters defining. This article focuses on the design development of the portable electrocardiograph with built-in GSM module for operation in telemedicine systems. The research objective is the design of the portable electrocardiograph for telemedicine that has a convenient interface for device control, is equipped with means of communication with cardiac care centers, has universal ports for information output to external drives and connection of additional devices, is as ergonomic as possible, meets safety, reliability and energy efficiency requirements. The statement of basic materials. The main design requirements to portable electrocardiographic equipment are formulated. The basic functional units of the device are chosen and its electronic block diagram is synthesized. The computer simulation of processes and parameters of the circuitry of the instrumentation amplifier functionality at given operating temperatures proves the validity of proposed circuit design. The constructive implementation of the schematic diagram is designed. The device constructive parameters that meet the requirements of telemedicine most closely are defined. Conclusions. The key features of the designed portable electrocardiograph with GSM module are: the embodiment in folding form-factor, the availability of GSM module, the constant connection of ECG electrodes via integrated ECG cable. The embodiment in folding form-factor optimizes ergonomic characteristics of the device. The replacement of non-integral ECG cable with integrated one reduces the intensity of internal noise and reduces time to prepare the device for work. Builtin GSM module provides affordable communication with specialized medical facilities, thus reducing the duration of the diagnostics and increasing the efficiency of the medical care in non-ambulatory out-hospital an outpatient setting.

In the context of multidisciplinary care of elderly patients, this work will want to consider the presence of osteo-articular and muscular pain, mostly chronic, of these subjects. The treatment has made use of oxygen-ozone therapy, given the absence of side effects of this minimally invasive technique, and the possibility of its use simultaneously with an already established poly-drug therapy, as typically found in the geriatric patient.

Objective: To evaluate the improvement in the care of elderly patients hospitalized due to pertrochanteric hip fractures. Methods: A comparative study of two cohorts of patients admitted due to pertrochanteric hip fractu re before (2010) and after the application of in hospital management protocols (2018). The intervention consisted in the implementation of multidisciplinary measures during hospitalization based on current scientific evidence. An evaluation of the clinical results was performed, as well as the health care impact. Results: The characteristics of patients admitted for hip fracture in 2010 (216 patients) and 2018 (205 patients) were similar in age, sex, Barthel index and the Charlson abbreviated index. In 2018 patients had more comorbidity. A significant reduction of preoperative stay and overall stay in the cohort of 2018 was achieved. Detection of delirium, malnutrition and anaemia was higher in 2018, and a reduced incidence of infection and a better function al efficiency was achieved in this period. Conclusion: The introduction of measures for the improvement of the pertrochanteric hip fracture management reduces hospitalization with consequent cost reduction. Unification of criteria among professionals may b e an opportunity for better clinical results and reduction of complications.

The article describes the activities of the Pokrovsky monastery-hospital in Kiev, created by the efforts of the Grand Duchess Alexandra Petrovna (1838-1900), who took the monastic vows in 1889 under the name Anastasia. The monastery became widely known in the late XIX - early XX century for the highly skilled medical aid provided to the population since its inception up to the events of 1917. It was a unique complex of medical settings located in the same area and equipped with modern medical equipment comparable with the many World’s clinics of that time. In this monastery-hospital, pilgrims with different diseases who came at Pokrovsky monastery in Kiev were provided the required qualified, affordable medical care. Thanks to the Grand Duchess Alexandra Petrovna, her attentive attitude to all the latest innovations, novel treatment methods were developed and introduced to the work of monastery medical settings. In addition, she was able to create a special atmosphere and goodwill towards the sick. But, being engaged in devising new methods of providing medical care to patients, she was able to create a special and inviting atmosphere with respect to the patient. With the ever-increasing circle of her organizational and economic activities, being in charge of the monastery-hospital problems of great importance, Grand Duchess reserved the right to nurse her patients. She worked by vocation, at the behest of the soul. Her example of selfless labor in the monastery medical settings where monastic sisters held the medical staff responsibilities for the patients care was important for all of the staff. Her assistants - Sisters of Charity - she fostered by her own example of selfless aid to a sick person. This favorable work atmosphere was very effective helping cure the sick.
