THORACO-OMPHALOPAGUS CONJOINED TWINS (CASE REPORT)Petrenko O.O., Petrenko A.O., Harmatina E.O.Conjoined twins are formed during an impairment in embryo division process. There are some risk factors considered to increase conjoined twins’ formation rate. However, further investigation of genetic processes same as teratogenic agents involved in it is required. In this case an 18-year old patient with the first pregnancy, having no apparent teratogenic factors or family anamnesis with diagnosed conjoined twins is presented. An abortion was performed on the 13th-14th weeks of gestation. Multiple malformations of conjoined twins were studied and genetic counseling was prescribed to the patient during planning of the next pregnancy.Conjoined twins are formed during an impairment in embryo division process. Есть некоторые факторы риска, которые, согласно исследованиям, увеличивают частоту образования сиамских близнецов. Однако требуется дальнейшее исследование вовлечённых в процесс генетических механизмов, а также тератогенных агентов. В данном клиническом случае представлены данные 18-летней пациентки с первой беременностью, не имеющей никаких очевидных тератогенных факторов или семейного анамнеза, у которой диагностированы сиамские близнецы. Аборт производился на 13-14 неделе беременности. В даному клінічному випадку представлені дані 18-річної пацієнтки з першою вагітністю без будь-яких очевидних тератогенних факторів або сімейного анамнезу, у якій виявилися сіамські близнюки. Аборт проводився на 13-14 тижні вагітності. Були досліджені аномалії розвитку сіамських близнюків, пацієнту було рекомендовано генетичне консультування перед плануванням наступної вагітності.Ключові слова: сіамські близнюки, торакоомфалопагі, аномалії розвитку.

Thyroid cancer is the most frequent endocrine neoplasm in the general population. Common risk factors include gender, radiation exposure, and genetic backgrounds. The association of papillary thyroid cancer and celiac disease has frequently been reported in the literature; however, the association of papillary thyroid cancer and thalassemia trait is rare. Likewise, the association of thalassemia major and celiac disease is also rare. We hereby report a unique case of papillary thyroid cancer in a patient with celiac disease and thalassemia trait.

Abstract Background The new coronavirus disease 2019 pandemic has spread throughout most of the world. Cerebral venous thrombosis is a rare thromboembolic disease that can present as an extrapulmonary complication in coronavirus disease 2019 infection. Case presentation We report the case of a Hispanic woman with Down syndrome who has coronavirus disease 2019 and presents as a complication extensive cerebral venous thrombosis. Conclusions Cerebral venous thrombosis is a rare thromboembolic disease that can present as an extrapulmonary complication in coronavirus disease 2019 infection. In the absence of clinical and epidemiological data, it is important to carry out further investigation of the risk factors and pathophysiological causes related to the development of cerebrovascular thrombotic events in patients with Down syndrome with coronavirus disease 2019 infection.

Placental infection by SARS-CoV-2 with various pathologic alterations reported. Inflammatory findings, such as extensive perivillous fibrin deposition and intervillous histiocytosis, have been postulated as risk factors for fetal infection by SARS-CoV-2. We describe the placental findings in a case of a 31-year-old mother with SARS-CoV-2 infection who delivered a preterm female neonate who tested negative for SAR-CoV2 infection. Placental examination demonstrated a small for gestational age placenta with extensive intervillous histiocytosis, syncytiotrophoblast karyorrhexis, and diffuse intervillous fibrin deposition. Immunohistochemical staining demonstrated infection of the syncytiotrophoblasts by SARS-CoV-2 inversely related to the presence of intervillous histiocytes and fibrin deposition. Our case demonstrates that despite extensive placental pathology, no fetal transmission of SARS-CoV-2 occurred, as well as postulates a relationship between placental infection, inflammation, and fibrin deposition.

Abstract Background Trigeminal-cardiac reflex (TCR) is a brainstem vagus reflex that occurs when any center or peripheral branch of the trigeminal nerve was stimulated or operated on. The typical clinical manifestation is sudden bradycardia with or without blood pressure decline. The rhino-cardiac reflex which is one type of TCR is rare in clinical practice. As the rhino-cardiac reflex caused by disinfection of the nasal cavity is very rare, we report these two cases to remind other anesthesiologists to be vigilant to this situation. Case presentation This case report describes two cases of cardiac arrest caused by rhino-cardiac reflex while disinfecting nasal cavity before endoscopic transsphenoidal removal of pituitary adenomas. Their heart rate all dropped suddenly at the very moment of nasal stimulation and recovered quickly after stimulation was stopped and the administration of drugs or cardiac support. Conclusion Although the occurrence of rhino-cardiac reflex is rare, we should pay attention to it in clinical anesthesia. It is necessary to know the risk factors for preventing it. Once it occurs, we should take active and effective rescue measures to avoid serious complications.

AbstractThe case of a 31 year old male presenting as an emergency with a recurrent colonic volvulus is described. A chest X-ray on admission to hospital showed the presence of hepato-diaphragmatic interposition of the colon, Chilaiditi’s Sign, which is known to be a risk factor for colonic volvulus. This is only the fourth reported case of colonic volvulus in association with Chilaiditi’s Syndrome and the first with recurrent colonic volvulus. The optimal treatment for recurrent volvulus in patients with risk factors such as Chilaiditi’s Syndrome or megacolon is also discussed.
