Discharge planning is a complex process aiming to prepare patients during a transition period in a hospital until they are home. One of general hospitals (X) in Jakarta has implemented discharge planning, but it is not optimally conducted. This research aims to analyze the implementation of optimizing discharge planning in X General Hospital in Jakarta. This research employed pilot study initiated by conducting assessment through questionnaire, observation, and interview. The next stages were root cause analysis by employing fishbone analysis, Plan of Action (POA) arrangement, Plan Do Check Action (PDCA) implementation, and evaluation by employing questionnaire. One of the problems of implementing discharge planning is was implementation method in which the guidance of discharge planning was not available. Therefore, standard operating procedures (SOP) and format of discharge planning were necessarily revised. After arranging the guidance draft, SOP, and format of discharge planning, the researcher socialized and tested the SOP and format of discharge planning in two inpatient rooms. The results reveal that 80% of respondents state that the arranged format of discharge planning is easily understandable, easily applicable, less complicated, not rambling, and practical. Therefore, the management of X hospital Jakarta necessarily conducts the follow-up to develop and legalize the guidance draft, SOP, and format of discharge planning. Furthermore, the management necessarily socializes them to duty nurses before the hospital applies them.

Rekomendasi yang di usulkan yaitu berupa perbaikan format discharge planning, revisi SPO, pembuatan panduan dan pelaksanaan pelatihan discharge planning.ABSTRACTDischarge planning is a complex process that aims to prepare patients in the transition period in the hospital until the patient returns to his home, where the implementation must be made from the beginning the patient comes to the health service. However, discharge planning was done before the patient returns, or before the patient left the service unit in Hospital. In the ICU room with critical patient conditions, nurses prioritize paying attention to the critical condition of the patient, thus forgetting the implementation of this discharge planning. For this reason, this paper aimed to analyze the implementation of discharge planning in the ICU of Jakarta X Hospital, literature study to identify the root of the problem, so that recommendations can be proposed to resolve the problem. The method was used an analysis using fishbone diagram with an assessment conducted in the observation form. Interview and secondary data collection from Jakarta X Hospital. Based on the analysis results, the root cause of the problem is knowledge of nurses who are less about discharge planning and less optimal systems that support the implementation of discharge planning such as format, SPO and guidelines. For this reason, recommendations for improvements were made in the form of redesigning the format of discharge planning, revision of SPO, making guidelines and implementing discharge planning training

Root cause analysis techniques are an excellent choice for identifying the root causes of cost deviation in Iraqi construction projects. Many root cause analysis tools, such as the Fishbone diagram (FD), Pareto diagram (PD), and 5-Why analysis, have started to emerge from the literature as standard guidelines for identifying root causes. This study identified eighteen causes of cost deviation in construction projects, which they classified into three major groups (planning causes, designing causes, and execution causes). Pareto study indicates that twelve causes out of eighteen represent the most significant causes of cost deviation. After that, these eleven causes were filtered by 5-Why analysis, which concluded that insufficient project information and the implementation method is not appropriate with the project type are root causes for the planning group, while unclear owner requirements and changes in design are root causes for the designing causes group, Finally, changes in orders is a root cause for execution group.

Evaluation of Compliance to Standard Operating Procedures for Pain Management in Patients with Burns in Dr. Hasan Sadikin General Hospital BandungPain is defined as an unpleasant sensory and emotional experience related to actual or potential tissue damage. Pain management for burns is an integral part of burn therapy. Pain in burns is an acute pain and poor management will lead to health complications including chronic pain. Dr. Hasan Sadikin General Hospital Bandung has made a standard operating procedure (SOP) for pain management to improve compliance to pain management standard. This study aimed to evaluate the compliance to the standards in assessment techniques, follow-up, and re-evaluation of pain in patients with burn according to the applicable pain management SOP. This was a retrospective descriptive observational study on 99 medical records of burn patients who met the inclusion criteria in Dr. Hasan Sadikin General Hospital Bandung in 2018. The results of the study revealed that the pain assessment for these patient was carried out according to the SOP which refers to the use of a numeric rating scale or Wong Baker face pain scale in 99 patients (100%). In the follow-up, 71 were performed according to the SOP (72%) while the re-evaluation was performed in compliance with the SOP in 93 patients (94%). In conclusion, pain assessment in Dr. Hasan Sadikin General Hospital Bandung is performed in accordance with SOP on pain management but not all patients receive follow-up and re-evaluation of burn pain in accordance with the SOP on pain management.

The intention of this paper is to demonstrate the advantages and the economic benefits of vibration root cause analysis and diagnosis, proper treatment of rotors during service outage, and follow up after commissioning of the units versus the general industry approach of field balance all vibration problems. During the course of the paper several diagnostic tools and repair procedures will be introduced and discussed. Special enfaces is made in the identification and treatment of rotor’s bows, misalignments, and eccentricities since they represent the most common and less recognized vibration root cause. A case study will be used as a guide to facilitate the comprehension and to illustrate real industry results that can be achieved by systematically following the proposed methodology.
