Abstract: Patients with end-stage glioblastoma multiforme (GBM) that cannot be operated or treated with chemotheraphy and radiotheraphy have increased clinical complaints, thus affecting the patients’ quality of lifes (QoL). Hyperbaric oxygen therapy (HBOT) could alters hypoxic condition in tumor tissue with a different cascade from wound healing process. This study was aimed to assess whether the QoL of end-stage GBM patients could improve with the administration of HBOT assessed by decreased clinical complaints based on NANO score. The study was carried out at the Surgery Department of Prof. Dr. R. D. Kandou Hospital Manado, using pre and posttrial design. Each sample was assessed for the NANO score before HBOT, after one time, three times, five times, and 10 times of HBOT. The results showed that there was a change in the NANO score before and after 10 times of HBOT. The decrease in the NANO score occurred more quickly after the therapy. In the last two measurements, the NANO score according to the RVC model decreased from one to two units. The decreased NANO score occurred quite regularly with variations in the score that changed from time to time. In conclusion, HBOT can improve the QoL of patients with end-stage GBM and reduce the NANO score which is an assessment of clinical complaints of the patients.Keywords: glioblastoma multiforme; hyperbaric oxygen therapy; NANO score Abstrak: Penderita glioblastoma multiforme (GBM) stadium akhir yang tidak dapat dilakukan operasi, kemoterapi dan radioterapi memiliki keluhan klinis yang semakin meningkat sehingga memengaruhi kualitas hidup. Terapi oksigen hiperbarik (TOHB) dapat mengubah kondisi hipoksia pada jaringan tumor dengan kaskade yang berbeda dari proses penyembuhan luka. Penelitian ini bertujuan untuk menilai apakah kualitas hidup pasien GBM stadium akhir dapat membaik dengan pemberian TOHB, dinilai dengan menurunnya keluhan klinis berdasarkan NANO score. Penelitian dilakukan di Bagian Bedah RSUP Prof. Dr. R. D. Kandou Manado, dengan menggunakan pre and posttrial design. Pada setiap sampel dilakukan penilaian NANO score sebelum TOHB, setelah satu kali, tiga kali, lima kali, dan 10 kali TOHB. Hasil penelitian mendapatkan adanya perubahan NANO score sebelum TOHB dan setelah 10 kali TOHB. Penurunan NANO score berlangsung lebih cepat pasca terapi tersebut. Dalam dua pengukuran terakhir, NANO score menurut model RVC turun dari satu hingga dua satuan. Pada grafik, hasil penurunan NANO score terjadi cukup beraturan dengan variasi score yang berubah dari waktu ke waktu. Simpulan penelitian ini ialah TOHB dapat meningkatkan kualitas hidup penderita GBM stadium akhir dan menurunkan NANO score yang menjadi penilainan keluhan klinis penderita.Kata kunci: glioblastoma multiforme; terapi oksigen hiperbarik; NANO score

Abstract: Wound healing process consists of inflammation, proliferation, and remodelling phases with increasing inflammatory cells, angiogenesis, and epithelization. Mechanism of hyperbaric oxygen therapy is O2 pressure over 1 ATA will increase oxygen pressure in the tissue. The main outcome measure is wound healing. This study was aimed to obtain the influence of hyperbaric oxygen therapy to wound healing process of deep second degree burn wounds. This was an experimental study. Subject were 36 rabbits divided into 2 groups, each of 18 rabbits. Deep second degree burn wounds were performed on all rabbits. One group was treated with hyperbaric oxygen therapy 2.4 ATA for 6 days, meanwhile the other group as control. The result of Mann-Whitney U test showed significant differences in inflammatory cells (P = 0.025) and epithelization (P = 0.024); albeit, there was not significant difference in angiogenesis (P = 0.442) between the two groups. Conclusion: Hyperbaric oxygen therapy could influence the inflammatory cells and epithelization but not the angiogenesis.Keywords: second degree burn wound, healing process, hyperbaric oxygen therapyAbstrak: Proses penyembuhan luka terdiri dari: fase inflamasi, proliferasi, dan perupaan kembali/remodeling, yang tampak dengan meningkatnya sel-sel radang, angiogenesis serta epitelialisasi. Mekanisme kerja terapi oksigen hiperbarik (TOHB) ialah pemberian tekanan O2 yang melebihi 1 ATA akan menyebabkan peningkatan tekanan O2 dalam jaringan. Jenis penelitian ialah eksperimental. Subyek penelitian 36 ekor kelinci yang dibuat luka bakar derajat dua dalam, kemudian dibagi menjadi dua kelompok, masing-masing 18 ekor. Kelompok perlakuan diberikan TOHB dengan dosis 2,4 ATA selama 6 hari sedangkan kelompok lain sebagai kontrol. Hasil uji Mann-Whitney U menunjukkan terdapat perbedaan bermakna pada jumlah sel radang (P = 0,025) dan epitelialisasi (P = 0,024), tetapi tidak terdapat perbedaan bermakna pada angiogenesis (p=0,442), serta ada perbedaan bermakna pada pada kedua kelompok. Simpulan: Terapi oksigen hiperbarik berpengaruh terhadap jumlah sel radang dan epitelialisasi namun tidak terhadap angiogenesis.Kata kunci: penyembuhan luka bakar, oksigen hiperbarik, luka bakar derajat dua dalam

Abstract For many years now we have been observing a growing number of patients with amputations performed on lower extremities due to chronic wounds occurring as a result of atherosclerotic lesions in peripheral arteries, thromboembolism as well as due to chronic ischaemia in lower extremities. Modern physical medicine is systematically enhancing treatment possibilities for patients with chronic wounds by an introduction of innovative therapeutic devices into clinical practice, which often allow to prevent amputations, accelerate the healing process, and, most of all, alleviate or completely eliminate pain. The article presents the therapeutic mechanism and methodology of one of such methods - local hyperbaric oxygen therapy with the use of a device called LASEROBARIA - S, alongside a description of its therapeutic effects in the case of two patients.

Backgrounds and objectivesIdiopathic sudden sensorineural hearing loss (ISSNHL) is an entity whose diagnosis and treatment remain controversial to date. Various modalities of treatment have been tried with varying degrees of success. Hyperbaric oxygen therapy (HBOT) is a recent modality of treatment for this condition which acts by improving cochlear microcirculation. The objective of this study was to evaluate the effect of HBOT in improving hearing loss and secondary symptoms patients with ISSNHL.MethodsThis prospective study was undertaken over a 6 month period from the Department of Otorhinolaryngology in a tertiary referral hospital in eastern India. 48 consecutive newly diagnosed ISSNHL patients were included in this study. The patients were subjected to HBOT at 2 A.T.A (Atmosphere Absolute) for an hour over 45 days. Hearing status and secondary symptoms as assessed by clinical tests, pure tone audiogram were analysed before and after HBOT.ResultsMales comprised 62.5% (30/48) of our study population, mean age of the population was 49.3 ± 13.4 years. 45/48 patients (93.7%) had unilateral ISSNHL, while 3 (6.3%) had bilateral ISSNHL. Average hearing loss in the affected ear before and after treatment was 79.96dBHL and 62.27dBHL respectively. The most common secondary clinical feature was tinnitus (27/48, 56.3%), followed by vertigo (24/48, 50%), aural fullness (15/48, 31.25%) and nystagmus (5/48, 10.41%). After treatment tinnitus and vertigo were showed marked improvement; (22/27, 81.48%) and (18/24, 75%). 5 out of 15 (33.34%) patients showed reduced aural fullness. There was no improvement in patients suffering from nystagmus.

Chronic fatigue syndrome (CFS) is a poorly-understood respiratory condition that affects millions of individuals. Hyperbaric oxygen therapy (HBOT) is a treatment option being considered to address CFS as it is suggested to combat fatigue and increase oxygenation. HBOT provides two opportunities in advancing research of CFS: it may provide data on symptom amelioration and be utilized in the search for a biomarker. By either identifying biomarkers before using HBOT to compare epigenomes of patients before and after treatment or using HBOT to find epigenetic discrepancies between patients with and without treatment, matching epigenetic regulation with symptom amelioration may significantly advance the understanding of the etiology and treatment mechanism for CFS. EPAS1/HIF-2α is a leading candidate for an epigenetic biomarker as it responds differentially to hypoxic and normoxic conditions, which degrades more slowly in hypoxic conditions. Epigenetic regulation of EPAS1/HIF-2α in such differential conditions may be explored in HBOT experiments. In addition to HBOT as a promising treatment option for CFS symptoms, it may aid the identification of biomarkers in CFS. Further research into both outcomes is strongly encouraged.

ABSTRACT   Quality of life is the subjective perception of the individual to the physical, psychological, social, and environmental conditions experienced. The aim of the study was to analyze differences in the physical dimensions of the quality of life of diabetes mellitus patients between providing hyperbaric oxygen therapy 10 days and 5 days. This study used the quasy experiment method with the approach of pre post test with control group design. The population in the study was 882 people with a sample of 14 respondents divided into two groups. 7 respondents in the treatment group and 7 respondents in the control group. Sampling used was consecutive sampling technique. Data were analyzed using Paired t test and Independent t test. The results of the Paired t test statistic in the treatment group had differences in quality of life before and after 10 days of hyperbaric oxygen therapy, while the control group had no difference in quality of life before and after 5 days of hyperbaric oxygen therapy. The independent t test results were different after being given hyperbaric oxygen therapy 10 days and 5 days. It is expected that health workers can be used as a reference source and as a basis for determining nursing intervention in the management of diabetes, especially in the quality of life of diabetic patients.

Background: Snakebites are the main responsible for envenoming in dogs and the bothropic venom remains the most common in Brazil, which can induce a necrotic skin wound. Hyperbaric oxygen therapy (HBOT) use 100% oxygen under high pressure and used to treat different wounds in human patients. To the authors’ knowledge, no reports regarding to use the HBOT in skin wound caused by snakebite (Bothrops jararaca) are present in the literature. The present clinical case aimed to describe the use of HBOT for the treatment of an extensive necrotic wound caused by jararaca snakebite in a dog.Case: A neutered 8-year-old mixed-breed dog, weighing 12 kg, was admitted with a 7-day history of extensive necrotic wound was identified in the face and neck causing by a snakebite, and no sign of pain. The procedure of HBOT (single sessions of 1.5 ATM, 45 min, repeated every 48 h, up to 12 sessions) was decided, and the complete blood cells, alanine aminotransferase, creatinine, creatine kinase, prothrombin time, activated partial thromboplastin time, wound clinical evaluation were measured at the following time-points: 2nd, 5th, 10th, and 12th sessions. At the 5th session was identified leukopenia, neutropenia and lymphopenia. Wound re-epithelialization was initiated after the 5th session, and the complete epithelialization was identified at the 12th session of HBOT. During the HBOT no side effects were identified. Three months after the HBOT finished, the animal returned to the clinic and the clinical status evolved positively, and the wound was completed healed.Discussion: This report described the treatment of an extensive necrotic skin wound caused by snakebite (Bothrops jararaca) in an 8-year-old, neutered, mixed-breed dog using the HBOT. The wound healing was achieved after 12 sessions, similar to the literature, which reported a ranging from 1 to 12 sessions. The HBOT protocol used in this case was similar as reported for human patients with chronic wounds due to the lack of HBOT protocols for animals. No reports regarding the use of HBOT for treat necrotic wound caused by snakebite was described in the literature, and to the authors’ knowledge, this is the first report in Brazil describing the use of HBOT in dogs. On the other hand, dogs with surgically induced skin wounds and treated with daily session of HBOT using the treatment protocol of 1.7 ATM (30 min) and 2.0 ATM (40 min) up to 7th day of treatment did not show significant results on healing [9]. This fact was associated with the HBOT achievement in the proliferative and remodeling phases of the healing process. The high intensity of HBOT was between the 5th and 10th session since the wound showed a higher area decrease rate and consequently increase of wound contraction. This period was corresponding to the 10th and 20th day of the healing process, which can be identified angiogenic activity, re-epithelialization, and collagen maturation. The decrease in PVC has been associated with the anticoagulant and/or hemorrhagic activity caused by the venom, and leukopenia, neutropenia and lymphopenia was related with possible bone marrow exhaustion. Single sessions of HBOT (1.5 ATM, 45 min, and repeat each 48 h, up to 12 session) induces healing of necrotic wound caused by snakebite (Bothrops jararaca) in an 8-year-old, neutered, mixed-breed dog without any side effects.Keywords: dog, healing, hyperbaric chamber, skin wound, snake.
