COVID 19 has caused a major disruption in social, economical and educational activities around the world . We are still not clear about the issues that the covid 19 has resulted in imparting post graduate training programs specially anesthesia .WE have seen some such disruptions in the past during SARS but this pandemic has taken the entire world by surprise . Training programs are still evaluating the impact of covid 19 .Some of the suggested changes in the methodology of our training and teaching involves obtaining active feed back of the anesthesia trainees , conducting on line classes for teaching preoperative assessments and clinical scenarios , clinical skills training by using high fidelity simulation and using assessment strategies which are somewhat different from the strategies that were used in the past . We also have to modify our criteria for graduating our trainees without affecting patient safety. It seems a difficult task but constant review of the situation, making changes as the situation evolves and reinforcement of training methodologies keeping the safety of trainee and the patient in mind will definitely help us in producing excellent future anesthesiologist. Abstract BackgroundContinuing professional education is essential for nurses to provide quality patient care and upgrade their professional skills and competence. The need for continuing medical education (CME) has become more apparent in the face of advances in medical science, the ever-changing healthcare system, and the important role nurses play in improving health care. It is therefore imperative to explore nurses’ experience of CME courses and the extent to which such programs are effective. The present qualitative study aimed to assess the lived experience of nurses on the effect of CE programs in promoting their knowledge, skill, and attitude toward non-communicable diseases.MethodsThis qualitative study was conducted in 2019-2020 at various hospitals affiliated to Shiraz University of Medical Sciences (Shiraz, Iran). The target population was nurses actively working in the chronic wards of these hospitals. The participants were selected using maximum variation sampling, including nursing managers, education and clinical supervisors, and staff nurses. Data were collected through individual, face-to-face, semi-structured interviews and analysed using the conventional content analysis method. Data trustworthiness was assessed according to the criteria proposed by Guba and Lincoln.ResultsAnalysis of the interview data resulted in 230 primary codes based on which 15 subcategories, 6 categories, and 3 themes were identified. The extracted themes were training to improve knowledge and attitude, training to improve professional skills, and the need for effective training programs. The associated categories weretraining to improve clinical knowledge, training to improve professional attitude, training in clinical skills related to holistic and person‐centred care, communication skills training, incorporate the fundamentals of effective training, and recognize common challenges and barriers to effective training.ConclusionProfessional competence and performance of nurses can be improved through intrinsic motivation stimulation, planning and implementation of training programs based on professional needs, and effective assessment of the teaching/learning process.

This paper presents methods and practices of improving heat rate through testing and, most importantly, through heat rate monitoring. This work was preformed at Portland General Electric’s 585 MWe Boardman Coal Plant, which used two very different Powder River Basin and Utah coals ranging from 8,100 to over 12,500 Btu/lbm. Such fuel variability, common now among coal-fired units was successfully addressed by Boardman’s on-line monitoring techniques. Monitoring has evolved over the past ten years from a Controllable Parameters approach (offering disconnected guidance), to a systems approach in which fuel chemistry and heating value are determined on-line, their results serving as a bases for Second Law analysis. At Boardman on-line monitoring was implemented through Exergetic System’s Input/Loss Method. Boardman was one of the first half-dozen plants to fully implement Input/Loss. This paper teaches through discussion of eight in-plant examples. These examples discuss heat rate improvements involving both operational configurations and plant components: from determining changes in coal chemistry and composite heating value on-line; to recognizing the impact of individual rows of burners and pulverizer configurations; to air leakage identifications; to examples of hour-by-hour heat rate improvements; comparison to effluent flows; etc. All of these cases have applicability to any coal-fired unit.

“No-one except you and the patient really know what happens when you take him for an interview. You learn from your own mistakes behind the closed door” (Adams & Cook, 1984).Clinical skills in psychiatry are not best acquired in unsupervised practice or in learning by apprenticeship. In the past, it was reported that during the course of their clinical training, students' interviewing skills actually deteriorated rather than improved (Helfer, 1970; Maguire & Rutter, 1976), although recent research suggests that improved training is now having an impact (Davis & Nicholaou, 1992). The methods described below may be employed at both undergraduate and postgraduate level. Traditionally, psychiatrists have been actively involved, along with general practitioners, in the provision of communication skills training to undergraduates. In the future, this training will become a continuous process carried out alongside and within other teaching in all specialities (Gushing, 1996). One of the biggest challenges facing medical schools is how to engage and train clinical teachers from a range of specialities in facilitating students in the acquisition of clinical interpersonal skills.

Background: Maternal mortality, which constitutes the extreme point of the existing inequality between women in poor and rich countries, remains very high in Africa south of the Sahara. The objective of this study was to introduce a new training approach in Emergency Obstetric and Neonatal Care (EmONC) entered in Senegal to strengthen the skills of healthcare providers.Methods: The approach was based on the skills training using the so-called "humanist" method and "lifesaving skills". Simulated practice took place in the classroom through thirteen clinical stations summarizing the clinical skills on EmONC. The evaluation was done in all phases and the results were recorded in a database to document the progress of each learner.Results: With this approach, 432 providers were trained in 10 months. The increase in technical achievements of each participant was documented through the database. The combination of training based on the model “learning by doing” has ensured learning and mastering all EmONC skills and reduced missed learning opportunities as observed in former EmONC trainings.Conclusions: The impact of training on EmONC indicators and the introduction of this learning modality in basic training are the two major challenges in terms of prospects.

IntroductionThe therapeutic alliance might be the most important part of beginning clinical relationship and may have an important impact in treatment adherence. In fact, many studies indicate that the therapeutic alliance is the best predictor of treatment outcome.ObjectivesTo assess clinical skills and attitudes in mental health professionals(MHP).AimsThis study explore the impact of clinical skills and socio-demographic factors related MHP may have on treatment adherence of patients with mental health disorders (MHD).MethodsIn this cross sectional study, we use a convenience sample of MHP working in the mental health departments of three general hospitals in Lisbon great area. Data is being collected through individual interviews. We used a optimism scale (ETOS), Medication Alliance Beliefs Questionnaire (MABQ), and socio-demographic and clinical questionnaire.ResultsA convenience sample composed of sixty-five mental health clinician working in a variety of settings is being collected. We don’t found statistically significant differences between the therapeutic optimism and the socio-demographic and clinical characteristics of MHP. The average values of optimism found in MHP with additional training in skills training it was higher (t test = 1,64). The results demonstrate that the most of clinicians (n = 42; agree 64,6%; strongly agree, n = 19; 29,2%) believe that have the capacity to positively influence outcomes for people with mental disorders.ConclusionThis topic, along with a detailed examination of the relationship between therapeutic alliance and treatment adherence, will be the subject of future research projects.Disclosure of interestThe authors have not supplied their declaration of competing interest.
