Objective: To evaluate the relationship of baseline Hemoglobin A1c (HbA1c) with visual, anatomical outcomes and frequency of cost effective bevacizumab at 12 months in eyes of diabetic macular edema (DME) in a developing country . Study Design: Retrospective cross sectional comparative data. Place and Duration of Study: Prevention of blindness (POB), Karachi, Pakistan between October 2018 and sept 2019. Methodology: It is a comparative cross sectional retrospective study. A total of 280 patients with DME presenting to the eye OPD of POB who did not receive any treatment before in the absence of concurrent retinal disease and treated with at least 3 intravitreal injections of bevacizumab and who completed the follow-up of 12 months included in the study. Results: There were 280 patients , male were 149 (53.2%) and female 131 (46.8%), 95% patients age 50 years. Patients divided into 3 groups n= 20 (7.1%) , 187 (66.8%) and 73 (26.1%) patients with Hba1c <7.0 , 7.1-8.0 and > 8.0 respectively. Central macular thicknesss decreased significantly from baseline in all 3 groups with vision improved the most in group 1 <7.0 and group 2 ( 7.1-8.0 HbA1c). The no of antivegf injections increased in all 3 groups but statistically not significant. Significant inverse correlation of baseline HbA1c with vision at 12 month ( r = - 0.40, p<0.01) and positive correlation with central macular thickness. (r=0.53 p<0.01). Conclusion: Initial baseline Hba1c is strongly related with visual and anatomic outcome at 12 months . Number of injections were increased according to HbA1c in all 3 groups but statistically not significant may be due to the use of economical bevacizumab injection. Diabetic macular edema (DME) is a pathological accumulation of fluid in the subretinal or intraretinal spaces of the macular area in patients with diabetic retinopathy and leads to a significant decrease in Central vision. Various types of technical equipment for retinal imaging have led to significant advances in the study of diabetic macular edema. In particular, optical coherence tomography (OCT) can provide linear and structural analysis of macular layers in great detail. The important role of inflammation in the development of changes in the macular region, based on OCT classification, associated with characteristic changes in the retina, has been studied. In this review, we will describe the pathogenesis of diabetic macular edema and discuss its types based on the results of OCT and identify the relationship of various types of (DME) with inflammation. Inflammation plays an important role in the pathogenesis of diabetic macular edema, but its role differs depending on the type of changes detected on OCT.

Autism is still a nightmare for most parents. Parents with autism can be very stressful when dealing with a hyperactive child's behavior, aggressive and passive. Stress experienced by parents of children with autism will affect the ability of parents in the parenting role, especially in relation to coping strategies have in dealing with problems of children. The participation of parents is crucial the success of socializing with children with autism in the general population. This study aims to determine the relationship of coping strategies parents of autistic children and parenting parents. This type of research is an analytic correlation with cross sectional approach. The population in this study were all parents of autistic children in SLB Muhammadiyah Mojokerto numbering 15 people. Samples in this study were all parents of autistic children in SLB Muhammadiyah Mojokerto which totaled 15 people by using total sampling technique. Collecting data using questionnaires. Data analized use cross tabulation, presented in a frequency distribution. On cross-tabulation obtained results tend to use maladaptive coping strategies permissive parenting that is 8 (53.3%), there are also respondents who use adaptive coping strategies using authoritarian parenting as much as one person (16.7%), and adaptive coping strategies tend using democratic parenting style as much as 5 people (33.3%). Expected parents still seeking information to broaden their parents on coping strategies of parents of autistic children and parenting parents as well as parents to give special attention for children with autism to the development and advancement of their lives because they have the same rights as any other normal child.

Objective: As diabetes technology use in youth increases worldwide, inequalities in access may exacerbate disparities in hemoglobin A1c (HbA1c). We hypothesized an increasing gap in diabetes technology use by socioeconomic status (SES) would be associated with increased HbA1c disparities.

Research Design and Methods: Participants aged <18 years with diabetes duration ≥1 year in the Type 1 Diabetes Exchange (T1DX, US, n=16,457) and Diabetes Prospective Follow-up (DPV, Germany, n=39,836) registries were categorized into lowest (Q1) to highest (Q5) SES quintiles. Multiple regression analyses compared the relationship of SES quintiles with diabetes technology use and HbA1c from 2010-2012 and 2016-2018.

Results: HbA1c was higher in participants with lower SES (in 2010-2012 & 2016-2018, respectively: 8.0% & 7.8% in Q1 and 7.6% & 7.5% in Q5 for DPV; and 9.0% & 9.3% in Q1 and 7.8% & 8.0% in Q5 for T1DX). For DPV, the association between SES and HbA1c did not change between the two time periods, whereas for T1DX, disparities in HbA1c by SES increased significantly (p<0.001). After adjusting for technology use, results for DPV did not change whereas the increase in T1DX was no longer significant.

Conclusions: Although causal conclusions cannot be drawn, diabetes technology use is lowest and HbA1c is highest in those of the lowest SES quintile in the T1DX and this difference for HbA1c broadened in the last decade. Associations of SES with technology use and HbA1c were weaker in the DPV registry.

Positive influences of family members have been associated with a high probability of children’s daily breakfast consumption. Therefore, the aim of this study was to scrutinize the association of breakfast routines between mothers and their children. The baseline data of the Feel4Diabetes-study was obtained in 9760 children (49.05% boys)–mother pairs in six European countries. A parental self-reported questionnaire gauging the frequency of breakfast consumption and of breakfast´ foods and beverages consumption was used. Agreement in routines of mothers and their children’s breakfast consumption was analyzed in sex-specific crosstabs. The relationship of breakfast routine and food groups’ consumption between mothers and their children was assessed with analysis of covariance. The highest proportion of children who always consumed breakfast were those whose mothers always consumed it. Children consuming breakfast regularly had a higher intake of milk or unsweetened dairy products and all kind of cereal products (low fiber and whole-grain) than occasional breakfast consumers (p < 0.05). The strong similarity between mothers and children suggests a transfer of breakfast routine from mothers to their children, as a high proportion of children who usually consume breakfast were from mothers also consuming breakfast. All breakfast foods and beverages consumption frequencies were similar between children and their mothers.
