Background: Increased high blood pressure in the elderly can be caused by various factors, one of which is stress. Stress can trigger the onset of hypertension through activation of the sympathetic nervous system which causes an increase in blood pressure intermittently. Hypertension patients need to be helped to reduce stress so that blood pressure does not increase. One of the non-pharmacological therapies to reduce stress is SEFT therapy. The purpose of this activity are to increase the elderly's knowledge about hypertension and increase awareness to control blood pressure and lowering blood pressure with SEFT therapy. Methods: Method this study is used blood pressure measurement, health education, and SEFT therapy. Results: Of this activity are health education can increase knowledge and awareness in controlling blood pressure, and SEFT therapy can reduce blood pressure in elderly hypertension. Conclusion:  SEFT therapy can be an intervention alternative in reducing blood pressure.

Poor sleep quality can change the stress hormone cortisol and the sympathetic nervous system so that it can increase blood pressure in the elderly. The purpose of this study was to determine the relationship of sleep quality with blood pressure in the elderly at PosyanduLansia RW II Kedungkandang Health Center, Malang. The study design used a correlational design with a cross sectional approach. The population of the study was 158 people, the determination using purposive sampling so that the research sample was 32 respondents. To determine the correlation of data collection techniques using a questionnaire. Data analysis method used is the spearmen rank test obtained p value = 0.001, <0.050 so that Ho is rejected, meaning that there is a relationship between sleep quality and blood pressure in the elderly in the Posyandu Elderly RW II Kedungkandang Health Center Malang. Poor sleep quality can change the stress hormone cortisol and the sympathetic nervous system, resulting in an increase in blood pressure. The results of the analysis found that most 18 (56.2%) respondents experienced poor sleep quality in the elderly and most 19 (59.4%) respondents had hypertension blood pressure between 140/90 mmHg to <160/90 mmHg in the elderly. From this study it is expected that the elderly need to get enough sleep for  6 hours to reduce blood pressure. The next researcher is expected to be able to relate the factors that affect sleep quality with blood pressure and the importance of the elderly's participation in Posyandu. Keywords: sleep quality, elderly, blood pressure Abstrak : Kualitas tidur yang buruk dapat mengubah hormone stress kortisol dan system saraf simpatik sehingga bias meningkatkan tekanan darah pada lansia. Tujuan penelitian untuk mengetahui hubungan kualitas tidur dengan tekanan darah pada lansia di Posyandu Lansia RW II Puskesmas Kedungkandang Kota Malang. Desain penelitian mengunakan desain korelasional dengan pendekatan cross sectional. Populasi penelitian sebanyak 158 orang, penentuan menggunakan purposive sampling sehingga sampel penelitian sebanyak 32 responden.Untuk mengetahui korelasi teknik pengumpulan data menggunakan kuesioner. Metode analisa data yang di gunakan yaitu uji spearmen rank didapatkan nilai p = 0,001 < 0,050 sehingga Ho ditolak, artinya ada hubungan antara kualitas tidur dengan tekanan darah pada lansia di Posyandu Lansia RW II Puskesmas Kedung kandang Kota Malang. Kualitas tidur yang buruk dapat mengubah hormon stress kortisol dan system syaraf simpatik, sehingga terjadi peningkatan tekanan darah. Hasil analisis didapatkan sebagian besar 18 (56,2%) responden mengalami kualitas tidur buruk pada lansia dan sebagian besar 19 (59,4%) responden memiliki tekanan darah hipertensi antara140/90 mmHg sampai< 160/90 mmHg pada lansia.  Dari penelitian ini diharapkan lansia perlu mencukupi tidur malam > 6 jam guna menurunkan tekanan darah. Untuk peneliti selanjutnya diharapkan dapat menghubungkan dengan faktor-faktor yang mempengaruhi kualitas tidur dengan tekanan darah serta pentingnya keikutsertaan lansia keposyandu. Kata Kunci: Kualitas Tidur, Lansia, Tekanan Darah.

The aim of the study was to determine the effectiveness of giving hydrotherapi foot soak in hypertensive patients. The study was conducted with one group pretestposttest design without a control group by measuring blood pressure (pretest) before being given an intervention in the form of foot soak hydrotherm against 15 people with hypertension in Banjar Sri Mandala, Dauhwaru Village, Jembrana Subregency. Then do the blood pressure measurement again (posttest) after finishing the intervention. After the study was completed, the results showed that before the hydrotherapi foot bath intervention, there were 0% of patients classified as normal, 13.32% in prehypertension category, 60.08% in hypertension category I and 26.60% in hypertension category II. After the intervention was given, there were 13.32% of the patients classified as normal, 66.68% in the prehypertension category, 20.00% in the first category of hypertension and no patients belonging to the second grade hypertension category. There is a decrease of 20-30 mmHg for systolic blood pressure and 0-10 mmHg for diastolic blood pressure after intervention. Based on the results of the study it can be concluded that the hydrotherapi foot bath is effectively used to reduce blood pressure in hypertensive patients in the Banjar Sri Mandala, Dauhwaru Village, Jembrana District.

Plasma NEFA (non-esterified fatty acid) concentrations are elevated in patients with obesity. In the present study we first aimed to provide an integral haemodynamic profile of elevated plasma NEFAs by the simultaneous assessment of blood pressure, pulse wave velocity, FBF (forearm blood flow) and sympathetic nervous system activity during acute elevation of NEFAs. Secondly, we hypothesized that NEFA-induced vasodilation is mediated by adenosine receptor stimulation. In a randomized cross-over trial in healthy subjects, Intralipid® was infused for 2 h to elevate plasma NEFAs. Glycerol was administered as the Control infusion. We assessed blood pressure, pulse wave velocity, FBF (using venous occlusion plethysmography) and sympathetic nervous system activity by measurement of noradrenaline and adrenaline. During the last 15 min of Intralipid®/Control infusion, the adenosine receptor antagonist caffeine (90 μg·min−1·dl−1) was administered into the brachial artery of the non-dominant arm. Compared with Control infusion, Intralipid® increased pulse wave velocity, SBP (systolic blood pressure) and pulse pressure, as well as FBF (from 1.8±0.2 to 2.7±0.6 and from 2.3±0.2 to 2.7±0.6 ml·min−1·dl−1 for Intralipid® compared with Control infusion; P<0.05, n=9). Although in a positive control study caffeine attenuated adenosine-induced forearm vasodilation (P<0.01, n=6), caffeine had no effect on Intralipid®-induced vasodilation (P=0.5). In conclusion, elevation of plasma NEFA levels increased pulse wave velocity, SBP and pulse pressure. FBF was also increased, either by baroreflex-mediated inhibition of the sympathetic nervous system or by a direct vasodilating effect of NEFAs. As the adenosine receptor antagonist caffeine could not antagonize the vasodilator response, this response is not mediated by adenosine receptor stimulation.

The renin-angiotensin system plays an important role in the regulation of arterial blood pressure and in the development of some forms of clinical and experimental hypertension. It is an important blood pressure control system in its own right but also interacts extensively with other blood pressure control systems, including the sympathetic nervous system and the baroreceptor reflexes. Angiotensin (ANG) II exerts several actions on the sympathetic nervous system. These include a central action to increase sympathetic outflow, stimulatory effects on sympathetic ganglia and the adrenal medulla, and actions at sympathetic nerve endings that serve to facilitate sympathetic neurotransmission. ANG II also interacts with baroreceptor reflexes. For example, it acts centrally to modulate the baroreflex control of heart rate, and this accounts for its ability to increase blood pressure without causing a reflex bradycardia. The physiological significance of these actions of ANG II is not fully understood. Most evidence indicates that the actions of ANG to enhance sympathetic activity do not contribute significantly to the pressor response to exogenous ANG II. On the other hand, there is considerable evidence that the actions of endogenous ANG II on the sympathetic nervous system enhance the cardiovascular responses elicited by activation of the sympathetic nervous system.

Periode lansia merupakan periode kehidupan yang rentan terhadap penyakit akibat mengalami perubahan fungsi tubuh dan berdampak pada terjadinya penyakitdegeneratif seperti Hipertensi. Hipertensi merupakan penyakit kronis yang jika tidak dikelola dengan tepat dapat menyebabkan komplikasi penyakit lainnya. Hipertensi membutuhkan penanganan yang komprehensif dalam mengontrol tekanan darah baik secara farmakologis dan nonfarmakologis, salah satunya adalah pendidikan kesehatan dan terapi tertawa.Kegiatan pengabdian masyarakat ini bertujuan untuk memperluas wawasan tentang pentingnya kesehatan bagi lansia melalui modifikasi faktor risiko atau pengelolaan gaya hidup serta pencegahan melalui terapi tertawa. Kegiatan ini dilaksanakan di Negeri Waai Kecamatan Salahutu Kabupaten Maluku Tengah. Metode yang digunakan yaitu mulai dari pengukuran tekanan darah, penyuluhan, training of trainer terapi tertawa bagi kader posyandudan lansia serta praktek terapi tertawa. Hasil dari kegiatan ini adalah lansia mengalami penurunan tekanan darah setelah diberikan terapi tertawa serta mampu melakukan terapi tertawa secara mandiri. Selain itu lansia semakin antusias untuk datang ke lokasi kegiatan, hal ini terbukti dari daftar kunjungan dan keaktifan lansia dalam mengikuti seluruh rangkaian kegiatan mulai dari registrasi, pengukuran tekanan darah, praktek terapi tertawa dan dilanjutkan dengan pemeriksaan kesehatan secara rutin.Kata kunci: Lansia; Penyuluhan; Training of trainer; Pengukuran Tekanan Darah; Terapi Tertawa.AbstractThe elderly period is a period of life that is susceptible to illness due to changes in bodily functions and has an impact on the occurrence of degenerative diseases such as hypertension. Hypertension is a chronic disease which, if not managed properly, can cause other complications. Hypertension requires comprehensive treatment in controlling blood pressure both pharmacologically and nonpharmacologically, one of which is health education and laughter therapy. This community service activity aims to broaden insights on the importance of health for the elderly through modification of risk factors or lifestyle management and prevention through laughter therapy. This activity was carried out in Waai Negeri Salahutu District, Central Maluku Regency. The method used is starting from measuring blood pressure, counseling, laughter therapy training for trainers for elderly cadres and practice of laughter therapy. The results of this activity are the elderly experiencing a decrease in blood pressure after being given a laugh therapy and being able to do laugh therapy independently. In addition, the elderly are more enthusiastic to come to the location of the activity, this is evident from the list of visits and activeness of the elderly in participating in a whole series of activities ranging from registration, blood pressure measurement, laughter therapy practice and continued with routine health checks.Keywords: Elderly; Counseling; Training of trainers; Blood Pressure Measurement; Laughter Therapy.
