AbstractThe real-ear-to-coupler difference (RECD) is an ANSI standardized method for estimating ear canal sound pressure level (SPL) thresholds and assisting in the prediction of real-ear aided responses. It measures the difference in dB between the SPL produced in the ear canal and the SPL produced in an HA-1 2-cc coupler by the same sound source. Recent evidence demonstrates that extended high-frequency bandwidth, beyond the hearing aid bandwidth typically measured, is capable of providing additional clinical benefit. The industry has, in turn, moved toward developing hearing aids and verification equipment capable of producing and measuring extended high-frequency audible output. As a result, a revised RECD procedure conducted using a smaller, 0.4-cc coupler, known as the wideband-RECD (wRECD), has been introduced to facilitate extended high-frequency coupler-based measurements up to 12.5 kHz.This study aimed to (1) compare test–retest repeatability between the RECD and wRECD and (2) measure absolute agreement between the RECD and wRECD when both are referenced to a common coupler.RECDs and wRECDs were measured bilaterally in adult ears by calculating the dB difference in SPL between the ear canal and coupler responses. Real-ear probe microphone measures were completed twice per ear per participant for both foam-tip and customized earmold couplings using the Audioscan Verifit 1 and Verifit 2 fitting systems, followed by measurements in the respective couplers.Twenty-one adults (mean age = 67 yr, range = 19–78) with typical aural anatomy (as determined by measures of impedance and otoscopy) participated in this study, leading to a sample size of 42 ears.Repeatability within RECD and wRECD was assessed for each coupling configuration using a repeated-measures analysis of variance (ANOVA) with test–retest and frequency as within-participants factors. Repeatability between the RECD and wRECD was assessed within each configuration using a repeated-measures ANOVA with test–retest, frequency, and coupler type as within-participants factors. Agreement between the RECD and wRECD was assessed for each coupling configuration using a repeated-measures ANOVA with RECD value, coupler type, and frequency as within-participants factors. Post hoc comparisons with Bonferroni corrections were used when appropriate to locate the frequencies at which differences occurred. A 3-dB criterion was defined to locate differences of clinical significance.Average absolute test–retest differences were within ±3 dB within each coupler and coupling configuration, and between the RECD and wRECD. The RECD and wRECD were in absolute agreement following HA-1-referenced transforms, with most frequencies agreeing within ±1 dB, except at 0.2 kHz for the earmold, and 0.2–0.25 kHz for the foam tip, where the average RECD exceeded the average wRECD by slightly >3 dB.Test–retest repeatability of the RECD (up to 8 kHz) and wRECD (up to 12.5 kHz) is acceptable and similar to previously reported data. The RECD and wRECD are referenced to different couplers, but can be rendered comparable with a simple transform, producing values that are in accordance with the ANSI S3.46-2013 standard.

This study examined the attenuation characteristics of five FM system sound delivery options for a group of 10 adults and 15 children (5–13 years). Sound delivery options included a tube-fitting, lightweight headphones, a CROS earmold with tubing, a CROS earmold with a snap-ring, and a standard snap-ring earmold with a vent. Attenuation was defined as the difference between probe-tube microphone measures of the ear canal resonance and the SPL in the ear canal with each sound delivery option in place. A statistically significant but clinically inconsequential difference in attenuation for the CROS earmold with tubing was noted between adults and children. No significant differences in attenuation for any of the other sound delivery options were noted between adults and children. An investigation of the relationship between magnitude of attenuation and percentage of the ear canal occluded suggests that degree of occlusion is a major factor in determining degree of attenuation provided by a particular sound delivery option. Results also indicate that significant attenuation of high-frequency signals can occur with earmolds commonly considered nonoccluding. Caution should be used in fitting hearing aids or FM systems to individuals with normal high-frequency hearing sensitivity to prevent attenuation of unamplified high-frequency speech information.

A custom in-the-ear (ITE) hearing aid fitting was compared to two over-the-ear (OTE) hearing aid fittings for each of 9 subjects with mild to moderately severe hearing losses. Speech intelligibility via the three instruments was compared using the Speech Intelligibility Rating (SIR) test. The relationship between functional gain and coupler gain was compared for the ITE and the higher rated OTE instruments. The difference in input received at the microphone locations of the two types of hearing aids was measured for 10 different subjects and compared to the functional gain data. It was concluded that (a) for persons with mild to moderately severe hearing losses, appropriately adjusted custom ITE fittings typically yield speech intelligibility that is equal to the better OTE fitting identified in a comparative evaluation; and (b) gain prescriptions for ITE hearing aids should be adjusted to account for the high-frequency emphasis associated with in-the-concha microphone placement.

Abstract Background Investigations of β-alanine supplementation shows effects on metabolic (aerobic and anaerobic) participation and performance on swimming by a possible blood acidosis buffering. Considering this background, the objective of the present study was to analyze the effects of β-alanine supplementation on metabolic contribution and performance during 400-m swim. Methods Thirteen competitive swimmers underwent a 6-week, double-blind placebo-controlled study, ingesting 4.8 g.day− 1 of β-alanine or placebo. Before and after the supplementation period, the total anaerobic contribution (TAn) and 30-s all-out tethered swimming effort (30TS) were assessed. Anaerobic alactic (AnAl) and lactic energy (AnLa) was assumed as the fast component of excess post-exercise oxygen consumption and net blood lactate accumulation during exercise (∆[La−]), respectively. Aerobic contribution (Aer) was determined by the difference between total energy demand and TAn. In addition to conventional statistical analysis (Repeated measures ANOVA; p > 0.05), a Bayesian repeated measures ANOVA was used to evidence the effect probability (BFincl). Results No differences and effects were found between groups, indicating no supplementation effects. Repeated measures ANOVA, with confirmation of effect, was indicate reduce in ∆Lactate (p: 0.001; BFincl: 25.02); absolute AnLa (p: 0.002; BFincl: 12.61), fatigue index (p > 0.001; BFincl: 63.25) and total anaerobic participation (p: 0.008; BFincl: 4.89). Conclusions Thus, the results demonstrated that all changes presented were evidenced as a result of exposure to the training period and β-alanine supplementation doesn’t affect metabolic contribution and performance during 400-m freestyle.

AbstractAccess to aided high-frequency speech information is currently assessed behaviorally using recognition of plural monosyllabic words. Because of semantic and grammatical cues that support word+morpheme recognition in sentence materials, the contribution of high-frequency audibility to sentence recognition is less than that for isolated words. However, young children may not yet have the linguistic competence to take advantage of these cues. A low-predictability sentence recognition task that controls for language ability could be used to assess the impact of high-frequency audibility in a context that more closely represents how children learn language.To determine if differences exist in recognition of s/z-inflected monosyllabic words for children with normal hearing (CNH) and children who are hard of hearing (CHH) across stimuli context (presented in isolation versus embedded medially within a sentence that has low semantic and syntactic predictability) and varying levels of high-frequency audibility (4- and 8-kHz low-pass filtered for CNH and 8-kHz low-pass filtered for CHH).A prospective, cross-sectional design was used to analyze word+morpheme recognition in noise for stimuli varying in grammatical context and high-frequency audibility. Low-predictability sentence stimuli were created so that the target word+morpheme could not be predicted by semantic or syntactic cues. Electroacoustic measures of aided access to high-frequency speech sounds were used to predict individual differences in recognition for CHH.Thirty-five children, aged 5–12 yrs, were recruited to participate in the study; 24 CNH and 11 CHH (bilateral mild to severe hearing loss) who wore hearing aids (HAs). All children were native speakers of English.Monosyllabic word+morpheme recognition was measured in isolated and sentence-embedded conditions at a +10 dB signal-to-noise ratio using steady state, speech-shaped noise. Real-ear probe microphone measures of HAs were obtained for CHH. To assess the effects of high-frequency audibility on word+morpheme recognition for CNH, a repeated-measures ANOVA was used with bandwidth (8 kHz, 4 kHz) and context (isolated, sentence embedded) as within-subjects factors. To compare recognition between CNH and CHH, a mixed-model ANOVA was completed with context (isolated, sentence-embedded) as a within-subjects factor and hearing status as a between-subjects factor. Bivariate correlations between word+morpheme recognition scores and electroacoustic measures of high-frequency audibility were used to assess which measures might be sensitive to differences in perception for CHH.When high-frequency audibility was maximized, CNH and CHH had better word+morpheme recognition in the isolated condition compared with sentence-embedded. When high-frequency audibility was limited, CNH had better word+morpheme recognition in the sentence-embedded condition compared with the isolated condition. CHH whose HAs had greater high-frequency speech bandwidth, as measured by the maximum audible frequency, had better word+morpheme recognition in sentences.High-frequency audibility supports word+morpheme recognition within low-predictability sentences for both CNH and CHH. Maximum audible frequency can be used to estimate word+morpheme recognition for CHH. Low-predictability sentences that do not contain semantic or grammatical context may be of clinical use in estimating children’s use of high-frequency audibility in a manner that approximates how they learn language.

Background: Despite evidence suggesting inaccuracy in the default fittings provided by hearing aid manufacturers, the use of probe-microphone measures for the verification of fitting accuracy is routinely used by fewer than half of practicing audiologists. Purpose: The present study examined whether self-perception of hearing aid benefit, as measured through the Abbreviated Profile of Hearing Aid Benefit (APHAB; Cox and Alexander, 1995), differed as a function of hearing aid fitting method, specifically, manufacturer's initial-fit approach versus a verified prescription. The prescriptive fit began at NAL-NL1 targets, with adjustments based on participant request. Each of the two fittings included probe-microphone measurement. Research Design: A counterbalanced, cross-over, repeated-measures, single-blinded design was utilized to address the research objectives. Study Sample: Twenty-two experienced hearing aid users from the general Bay Pines VA Healthcare System audiology clinic population were randomized into one of two intervention groups. Intervention: At the first visit, half of the participants were fit with new hearing aids via the manufacturer's initial fit while the second half were fit to a verified prescription using probe-microphone measurement. After a wear period of 4–6 wk, the participants' hearing aids were refit via the alternate method and worn for an additional 4–6 wk. Participants were blinded to the method of fitting by utilizing probe-microphone measures with both approaches. Data Collection and Analysis: The APHAB was administered at baseline and at the end of each intervention trial. At the end of the second trial period, the participants were asked to identify which hearing aid fitting was “preferred.” The APHAB data were subjected to a general linear model repeated-measures analysis of variance. Results: For the three APHAB communication subscales (i.e., Ease of Communication, Reverberation, and Background Noise) mean scores obtained with the verified prescription were higher than those obtained with the initial-fit approach, indicating greater benefit with the former. The main effect of hearing aid fitting method was statistically significant [F (1, 21) = 4.69, p = 0.042] and accounted for 18% of the variance in the data (partial eta squared = 0.183). Although the mean benefit score for the APHAB Aversiveness subscale was also better (i.e., lower) for the verified prescription than the initial-fit approach, the difference was not statistically significant. Of the 22 participants, 7 preferred their hearing aids programmed to initial-fit settings and 15 preferred their hearing aids programmed to the verified prescription. Conclusions: The data support the conclusion that hearing aids fit to experienced hearing aid wearers using a verified prescription are more likely to yield better self-perceived benefit as measured by the APHAB than if fit using the manufacturer's initial-fit approach.

Background: Frequency transposition has gained renewed interest in recent years. This type of processing takes sounds in the unaidable high-frequency region and moves them to the lower frequency region. One concern is that the transposed sounds mask or distort the original low-frequency sounds and lead to a poorer performance. On the other hand, experience with transposition may allow the listeners to relearn the new auditory percepts and benefit from transposition. Purpose: The current study was designed to examine the effect of linear frequency transposition on consonant identification in quiet (50 dB SPL and 68 dB SPL) and in noise at three intervals—the initial fit, after one month of use (along with auditory training), and a further one month of use (without directed training) of transposition. Research Design: A single-blind, factorial repeated-measures design was used to study the effect of test conditions (three) and hearing aid setting/time interval (four) on consonant identification. Study Sample: Eight adults with a severe-to-profound high-frequency sensorineural hearing loss participated. Intervention: Participants were fit with the Widex m4-m behind-the-ear hearing aids binaurally in the frequency transposition mode, and their speech scores were measured initially. They wore the hearing aids home for one month and were instructed to complete a self-paced “bottom-up” training regimen. They returned after the training, and their speech performance was measured. They wore the hearing aids home for another month, but they were not instructed to complete any auditory training. Their speech performance was again measured at the end of the two-month trial. Data Collection and Analysis: Consonant performance was measured with a nonsense syllable test (ORCA-NST) that was developed at this facility (Office of Research in Clinical Amplification [Widex]). The test conditions included testing in quiet at 50 dB SPL and 68 dB SPL, and at 68 dB SPL in noise (SNR [signal-to-noise ratio] = +5). The hearing aid conditions included no transposition at initial fit (V1), transposition at initial fit (V2), transposition at one month post-fit (V3), and transposition at 2 months post-fit (V4). Identification scores were analyzed for each individual phoneme and phonemic class. Repeated-measures ANOVA were conducted using SPSS software to examine significant differences. Results: For all test conditions (50 dB SPL in quiet, 68 dB SPL in quiet, and 68 dB SPL in noise), a statistically significant difference (p < 0.05 level) was reached between the transposition condition measured at two months postfitting and the initial fitting (with and without transposition) for fricatives only. The difference between transposition and the no-transposition conditions at the 50 dB SPL condition was also significant for the initial and one-month intervals. Analysis of individual phonemes showed a decrease in the number of confusions and an increase in the number of correct identification over time. Conclusions: Linear frequency transposition improved fricative identification over time. Proper candidate selection with appropriate training is necessary to fully realize the potential benefit of this type of processing.

Background and aims: Postoperative pain has always been considered by surgeons because of its various complications. The aim of this study was to compare the effect of intravenous, subcutaneous and suppository morphine in reducing post-hysterectomy pain. Materials and Methods: In this clinical trial, 90 patients undergoing hysterectomy were randomized into three groups of 30 each using simple randomization, namely, intravenous, subcutaneous, and suppository morphine (10 mg). Before intervention and 4, 8, 12, and 16 hours after intervention, pain intensity was measured using visual analogue scale (VAS). Relative frequency of nausea, vomiting, itching, bradypnea, and apnea in all groups was recorded. Data were analyzed by SPSS version16.0. Results: Mean pain severity at 0 hour postoperatively (P=0.004), 4 hours postoperatively (P=0.009), 8 hours postoperatively (P=0.009), and 12 hours postoperatively (P=0.001) was significantly higher in the suppository morphine group than in the other two groups. There was no significant difference in pain severity at 16 hours postoperatively among the three groups (P=0.446). According to the results of repeated measures ANOVA, changes in pain severity at the five intervals were statistically significant in all three groups (subcutaneous, intravenous, and suppository morphine groups) (P<0.001). There was also a statistically significant difference in pain severity at the studied intervals among the three groups (P<0.001). The frequency of nausea (P=0.05) and vomiting (P=0.84) was higher in the suppository group than in the other two groups, although the difference was not statistically significant (P=0.05). Conclusion: The results of this study indicated better efficacy of subcutaneous and intravenous morphine in reducing post-hysterectomy pain compared with suppository morphine.

Background: SoundTracker is an algorithm in Widex's Compass fitting software that could potentially be used to estimate a patient's aided sensation level (SL). SoundTracker's accuracy of estimating a patient's SL has never been verified in comparison to SL measured with commercially available real-ear analyzers. Purpose: Determine whether statistically significant differences are present between the estimated SL of the Widex SoundTracker software application and the measured SL of the Audioscan Verifit and Frye 6500 real-ear analyzers at 500, 1000, 2000, and 4000 Hz. Research Design: This study used a randomized repeated measures design to determine differences in SL between SoundTracker and the Verifit and 6500. Study Sample: Ten subjects (N = 20 ears) were recruited who were experienced users of behind-the-ear hearing aids with conventional vented earmolds and had bilateral sensorineural hearing loss that was >30 dB HL below 1000 Hz and ≤70 dB HL to 4000 Hz. Data Collection and Analysis: Real-ear in-situ thresholds (dB sound pressure level [SPL]) and real-ear aided responses (REAR; dB SPL) were measured at 500, 1000, 2000, and 4000 Hz to determine differences in SL between SoundTracker, Verifit, and 6500. A three-factor repeated measures analysis of variance (ANOVA) was utilized to determine differences between method (real-ear analyzers and SoundTracker), analyzer (Verifit and 6500), and frequency (500, 1000, 2000, and 4000 Hz). Results: Mean differences in measured SL for the Verifit and 6500 were ≤2 dB when compared to the estimated SL using SoundTracker. A statistically significant difference in SL was present between the Verifit and SoundTracker at 2000 Hz (p < 0.01), but no significant differences were present at 500, 1000, and 4000 Hz. A statistically significant difference in SL was present between the 6500 and SoundTracker at 4000 Hz (p < 0.01), but no significant differences were present at 500, 1000, and 2000 Hz. Mean differences in measured SL between the real-ear analyzers (difference of SoundTracker SL minus Verifit SL compared to the difference of SoundTracker SL minus 6500 SL) were ≤2 dB with a statistically significant difference present at 2000 Hz (p < 0.01), but no statistically significant differences were present at 500, 1000, or 4000 Hz. Conclusions: Nearly 85% of the differences between the estimated SoundTracker SL and the measured SLs of the Verifit and 6500 were ≤2 dB. Despite some limitations of this study, SoundTracker could be useful as a counseling tool to illustrate to patients which sounds are audible or inaudible when unaided and aided.

Background:The PACER test is a valid and reliable assessment of aerobic capacity in children. However, many schools lack adequate space to administer the test. This study compared the utility of the standard 20m PACER test with an alternative 15m PACER protocol in 5th and 8th grade students.Methods:A total of 171 students completed both PACER protocols in a counterbalanced design. Agreement between the two protocols was assessed with correlations, repeated-measures ANOVA, and classification agreement into the FITNESSGRAM® healthy fitness zones.Results:The difference in estimated VO2max between the two tests was slightly larger for boys (5th grade, 1.32 ml/kg/min; 8th grade, 1.72 ml/kg/min) than girls (5th grade, 0.14 ml/kg/min; 8th grade, 1.11 ml/kg/min), but these differences are probably not of practical significance. Classification agreement was 88% for boys and 91% for girls.Conclusions:Collectively, the results suggest that the 15m and 20m PACER provide similar information about aerobic fitness in youth. The 20m test is recommended when possible, but the 15m provides a useful alternative for schools with smaller gymnasiums.

This paper reports on the results of a listening instruction intervention for Japanese EFL university students aimed at improving their ability to correctly discern the phonetic and phonological aspects of English sounds. In the background of this project lies our belief that the phonetic/phonological instructions are likely to be helpful (even) for Japanese EFL students who do not major in English linguistics or literature, although these instructions are usually offered to those who are English majors. The goal of the study, thus, is to show that phonetics/phonology-based English teaching is effective for Japanese EFL students in improving their listening ability in general. To achieve the goal, we utilized a set of exercises devised for a 15-week listening course (i.e., “Sound Focus for Effective Listening”; hereinafter, “Sound Focus”). Sound Focus includes six phonetic/phonological aspects of English that are considered by the authors (= instructors) to be essential and important for improvement of listening ability. The participants were 331 freshmen at a national university: 254 were instructed in a CALL (computer-assisted language learning) classroom situation and 77 in a traditional classroom situation. Sound Focus was given with the help of a learning management system (LMS), Moodle, in the CALL classroom situation. In the traditional classroom, the Sound-Focus materials and listening exercises were provided in the form of paper-based handouts used with a CD. To understand the effects of Sound Focus instruction on student achievement and the difference between the two classroom situations, we conducted pre- and post-listening tests and administered a Can-do-statements questionnaire and a free-description questionnaire. The listening tests, which were based on Sound Focus, measured the improvement in students’ listening ability during the course; the Can-do-statements questionnaire evaluated their confidence in their listening ability; and the free description questionnaire aimed to identify the aspects of the instruction that was positively or negatively accepted by the learners. The results of the pre- and post-listening tests and the Can-do-statements questionnaire were analyzed by two-way repeated-measures ANOVA. The free description questionnaire was analyzed with a text-mining technique (SPSS Text Analytics for Surveys 3.0). The two-way repeated-measures ANOVA analysis on the difference between the scores of the pre- and post-listening tests suggested that students in each classroom situation improved their listening ability. The combined analysis of the results of the pre- and post-test scores and the Can-do-statements questionnaire further suggested that the instruction was effective for students with all levels of confidence. We analyzed the free description questionnaire to explicate what aspect of the instruction showed greater effectiveness. The results revealed that among the instructional materials, including the textbook conversations and TOEIC exercises, Sound Focus was considered by the students to be the most effective for their learning, regardless of their classroom situation. The students in the traditional classroom situation reported that the textbook conversations were also helpful. Regarding the presentation of the instructional materials, on the other hand, learners showed a sharp perceptual difference: Those in the CALL classroom situation accepted the LMS (Moodle) more positively, while in the traditional classroom situation, the presentation of the materials with the help of a projector was negatively scored. The analysis also showed that Moodle was also regarded as the best activity for the improvement of their listening ability among all the classroom activities (e.g., role-play conversations, dictations, shadowing). 本実践報告では,英語を専攻としていない日本の大学1年生を対象とした英語リスニングの授業において,英語の音声学・音韻論的特徴を指導した効果を検証した。授業はSound Focusと名付けた教材を使用し,普通教室またはCALL教室で行われ,教室環境の違いも考慮に入れた効果検証を行った。プレ・ポストテストにおける音声学・音韻論的特徴の聞き取りに関するパフォーマンスの違いと英語に対する自信の自己評価(Can-Do調査)との関連を2要因の分散分析により検討した。また,授業終了時の自由記述もテキストマイニングの手法を用いて検討した。分散分析の結果,Sound Focusを用いた英語音声学・音韻論的な指導の効果は,教室環境(普通教室,CALL教室)の違いにかかわらず,大学1年生の聞き取りパフォーマンスの向上に効果があることがわかった。またテキストマイニングの分析からは,両教室環境に対する学生の認識の違いが示された。
