SUMMARY The aim of this clinical trial was to compare the clinical performance of three different adhesive systems over 18 months in noncarious cervical lesions (NCCLs). Thirty patients, with at least three noncarious cervical lesions, were enrolled in the study. One operator randomly restored a total of 90 lesions with resin composite (Herculite XRV). The restorations were bonded with either Optibond FL (OF), three-step total-etch; Optibond Solo Plus (OS), two-step total-etch; or Optibond All-In-One (OA), one step self-etch. The restorations were clinically evaluated at baseline and after six, 12, and 18 months using the eight United States Public Health Services criteria. Data were analyzed using Friedman and Wilcoxon signed ranks tests (p<0.05). After 18 months, the retention rate was (OF) 96.5%, (OS) 93.1%, and (OA) 89.7%. Differences among the three adhesive systems for evaluated criteria were not observed in comparison of the mean Alfa score percentages. There was a significant increase in marginal discoloration for (OA) adhesive after 18 months compared with baseline (p=0.011). Other restoration criteria had no statistically significant differences among the three adhesives (p>0.05). With the exception of marginal discoloration, the clinical effectiveness of three types of adhesive systems in NCCLs was acceptable after 18 months. However, using the one-step self-etch adhesive may lead to some marginal discolorations.

Introduction: Some studies have shown that laser irradiation on unpolymerized adhesives can improve composite-dentin adhesion. The aim of the present study was to evaluate the effect of the diode laser (810 nm) on the microleakage of multi-mode adhesive systems at enamel and dentin margins of composite restorations. Methods: Classic class V boxes were prepared on 48 sound premolar teeth and randomly divided into 6 groups (n=16). In the control groups, Scotchbond Universal (SBC), G-Premio (GBC), and Ambar U (AMC) were used by a self-etch mode. In the test groups (SBL, GBL, ABL), the 810 nm diode laser was irradiated (1 W) for 10 seconds before the polymerization of the adhesive. The boxes were restored by the resin composite. After finishing and polishing, the samples were thermocycled (5°C to 55°C) for 1000 cycles and then immersed in 0.1% methylene blue dye (48 hours). Dye penetration through the gingival and occlusal margins was measured by Stereomicroscope. The data were analyzed at the 5% significance level using Kruskal-Wallis and Mann-Whitney U tests. Results: Significant differences were found between the control and test groups (P<0.05). The occlusal margins of the SBL and GBL groups and the cervical margin of the SBL group exhibited the lowest microleakage (P<0.05). The AM control group showed maximum microleakage at cervical and occlusal margins. Conclusion: The irradiation of the 810 nm diode laser on the unpolymerized universal adhesive systems in a self-etch mode caused a significant reduction in enamel and dentin marginal microleakage of composite restorations.

Good margin integrity with a tight seal of the adhesive interface is considered one of the key factors for the clinical success of composite restorations. This study investigated the effect of enamel etching with phosphoric acid on the margin integrity of self-etch bonded composite restorations in demineralized enamel. Crowns of bovine incisors were assigned into 14 groups (n = 10 per group) of which ten groups (groups 1–5 and 8–12) were demineralized (21 days, acid buffer, pH 4.95) to create artificial carious lesions. Standardized Class V cavities were prepared in all specimens. Demineralized groups were either etched with phosphoric acid for 10, 30, 60, or 120 s (groups 2–5 and 9–12), or no etching was performed (groups 1 and 8). The non-demineralized (sound) groups were etched for 10 s (groups 7 and 14) or remained non-etched (groups 6 and 13). Resin composite restorations were then placed using either a one-step (iBond Self Etch, groups 1–7) or two-step self-etch adhesive (Clearfil SE Bond, groups 8–14). Margin integrity of the restorations was assessed after thermocycling (5000×, 5–55 °C) using scanning electron microscopy, and the percentage of continuous margins (%CM) was statistically analyzed (α = 0.05). Phosphoric acid etching significantly increased %CM in both demineralized and sound enamel. For iBond Self Etch, a significant increase in %CM in demineralized enamel was observed with increased etching times. All etched groups treated with Clearfil SE Bond and those etched for 60 or 120 s and treated with iBond Self Etch showed similar %CM in demineralized enamel as in etched sound enamel, and significantly higher %CM than in non-etched sound enamel. In conclusion, enamel etching with phosphoric acid improves margin integrity of composite restorations in demineralized enamel when bonded with the examined adhesives.

Clinical Relevance Decreased leakage was associated with Adper Scotchbond Multi-Purpose (total etch) and iBond (self-etch) systems in Class V resin composite restorations. All adhesive systems performed best when bonded to enamel compared to dentin surface anatomy. Clinicians should be aware that strict adherence to manufacturer instructions, when using these materials, is of primary importance.

Objective: This study investigated whether different aging protocols have effects on the bond strength of total-etch and self-etch adhesive systems to dentin substrate. Methods: Molars were sectioned exposing the dentin, which was submitted to restorative procedures using two-step total-etch (ASB, Adper SingleBond2), two-step self-etch (CLF, Clearfil SEBond), or one-step self-etch (OPT, Optibond All-in-One) system and resin composite. The obtained blocks were sectioned to result in sticks to be submitted to bond strength and failure mode tests after 24h storage in distilled water (DW), or 180-day aging protocols in DW, artificial saliva (AS), citric acid (CA), or thermal cycling (TC). The types of failures were classified as adhesive, cohesive in dentin, cohesive in composite resin, and mixed. Data were analyzed using ANOVA followed by Tukey’s test (α=5%). The failure modes were descriptive under the relative frequencies form. Results: It was observed that ASB presented the higher bond strength(p<0.05), while CLF and OPT did not demonstrate differences between them(p>0.05) after 24h. The bond strength obtained with ASB significantly overcame that of OPT when stored in DW or AS for 180 days (p<0.001). In these storing mediums, ASB did not differ from CLF(p>0.05), while in CA or TC the values of CLF were significantly lower (p<0.001). In TC aging, the values presented by OPT did not differ from ASB(p>0.05), both of which had higher values than CLF(p<0.05). The predominant failure mode was adhesive. Conclusion: For the two-step total-etch and two-step self-etch adhesives, the influence of aging in AS and TC was equivalent to that of storage in DW. The most deleterious effects occurred in CA, a situation in which self-etch adhesives displayed worse performance than the conventional type.

Abstract BACKGROUND: Dental adhesives are believed to influence marginal adaptation and marginal discoloration when used under posterior resin-based composite restorations. Studies on the latest adhesive systems reveal that the group of the three-step etch-and-rinse adhesive (3-E&RA) and the one-step self-etch adhesive (1-SEA) have entirely different bonding mechanisms, as well as different bond strength and resistance to chemical, thermal and mechanical factors. STUDY OBJECTIVES: A hypothesis that a 1-SEA would result in greater enamel marginal discoloration and poorer marginal adaptation than a 3-E&RA was tested. MATERIAL AND METHODS: One hundred restorations were placed with a 1-SEA and 100 restorations with a 3-E&RA. Teeth were restored with Filtek Supreme nanofilled resin-composite and were evaluated for marginal adaptation and marginal discoloration at baseline, and 6 months, 12 months, and 36 months postoperatively. RESULTS: The statistical analysis revealed significant differences in marginal integrity between test groups. The 1-SEA resulted in greater enamel marginal discoloration and poorer marginal adaptation than the 3-E&RA at any recall time. CONCLUSIONS: Marginal adaptation and marginal discoloration depend on the type of dentin adhesive used. The restorations with Filtek Supreme and Scotchbond MP are better than the restorations with Adper Prompt L-Pop with regard to the marginal adaptation and marginal discoloration at 6-, 12- and 36-month evaluations.

ABSTRACT Objective: To evaluate the hybridization quality and bond strength of adhesives to dentin. Materials and Methods: Ten human molars were ground to expose the dentin and then sectioned in four tooth-quarters. They were randomly divided into 5 groups according to the adhesive used: Two single-step self-etch adhesives - Adper Prompt (ADP) and Xeno III (XE), two two-step self-etching primer systems - Clearfil SE Bond (SE) and Adhe SE (ADSE), and one one-step etch-and-rinse system - Adper Single Bond (SB). Resin composite (Filtek Z250) crown buildups were made on the bonded surfaces and incrementally light-cured for 20 s. The restored tooth-quarters were stored in water at 37΀C for 24 h and then sectioned into beams (0.8 mm 2 in cross-section). Maximal microtensile bond strength (μ-TBS) was recorded (0.5 mm/min in crosshead speed). The results were submitted to one-way ANOVA and Tukey′s test (α = 0.05). Thirty additional teeth were used to investigate the hybridization quality by SEM using silver methenamine or ammoniacal silver nitrate dyes. Results: SE reached significantly higher μ-TBS (P < 0.05); no significance was found between ADSE and XE (P > 0.05), and between SB and ADP (P > 0.05); ADSE and XE were significantly higher than SB and ADP (P < 0.05). The bonding interface of SB showed the most intense silver uptake. SE and ADSE showed more favorable hybridization quality than that observed for ADP and XE. Conclusions: The bond strength and hybridization quality were affected by the interaction form of the adhesives with dentin. The hybridization quality was essential to improve the immediate μ-TBS to dentin.

Clinical Relevance High-viscosity glass-ionomer cements (HVGICs) used with atraumatic restorative treatment can be repaired with light- or self-cured adhesive systems; however, the repair bond strength of two-step, self-etching and one-step adhesives in the light-cure mode surpass one-step self-cure adhesives. Working on a feasible self-cure approach in the absence of such in rural areas as well as in war zones is of prime importance. SUMMARY Objectives: Despite the success rate of high-viscosity glass-ionomer cements (HVGICs) used in atraumatic restorative treatment (ART) restorations, partial or bulk fracture of the proximal portion has been recorded to be one of the main causes of proximal restoration failures. Repair of these restorative materials requires a practical solution, especially in cases where there is a lack of electricity. Thus, the purpose of this study was to evaluate the repair microshear bond strength (μSBS) of three HVGICs using a resin composite in association with adhesive systems having different curing modes (ie, light- vs self-curing mode). Methods and Materials: A total of 105 discs (12 mm in diameter and 2 mm thick) of three HVGICs: GC Fuji IX GP Fast (GC Corporation, Tokyo, Japan); Fuji IX GP glass-ionomer cement containing chlorhexidine (GC Corporation, Tokyo, Japan); and ChemFil Rock zinc-reinforced HVGIC (Dentsply De-Trey GmbH, Konstanz, Germany) were prepared. Each specimen was divided into three horizontal sections, according to the tested adhesive system or curing mode: Clearfil SE Bond 2 (two-step, self-etch adhesive); (Kuraray Noritake Dental Inc., Tokyo, Japan) in light-cure mode; Clearfil Universal Bond (one-step, self-etch adhesive); (Kuraray Noritake Dental Inc., Tokyo, Japan) in light-cure mode; or Clearfil Universal Bond (one-step, self-etch adhesive); (Kuraray Noritake Dental Inc., Tokyo, Japan) in self-cure mode, mixing it with Clearfil DC Activator (Kuraray Noritake Dental Inc., Tokyo, Japan). A resin composite microcylinder was bonded to each horizontal section of each specimen using starch tubes. The bonded discs were stored in artificial saliva at 37°C for 24 hours. A μSBS test was conducted using a universal testing machine, while failure modes were determined using scanning electron microscopy. Data were statistically analyzed using two-way analysis of variance (ANOVA), one-way ANOVA, and Bonferroni post hoc tests (α=0.05). Results: Two-way ANOVA revealed a statistically significant effect for the adhesive systems (p&lt;0.01) and not for the HVGICs (p=0.05) nor their interactions (p=0.99). When using Clearfil SE Bond 2 and Clearfil Universal in a light-cure mode, significantly higher μSBS values were found when compared with Clearfil Universal in a self-cure mode. Conclusions: The three tested HVGICs can be successfully repaired using two-step or one-step self-etch adhesive systems. The one-step self-etch adhesive system in light-cure mode is preferred when compared with the self-cure mode.
