Learning to Accept Uncertainty as a Quality of Care Dimension

Uncertainty in clinical decision-making is integral to the pathways chosen while applying available knowledge to a patient’s care process. This chapter explores the ways in which uncertainty can be incorporated into the understanding of better performance approaches, and is thus proposed as an enabling dimension of performance. Tracing the keystone definitions of uncertainty from Hippocrates to Osler, the discussion addresses the dimensions of decision - making appropriateness, its timeliness, the expected and actual value of the care services, and the role of systematic communication between providers of care as well as with patients. The crucial role of Health Information Technology is emphasized, and a unifying model is proposed where the inclusion of uncertainty as a dimension of performance promotes an encompassing evaluation of the quality of health care services.

This article explores the ways in which uncertainty can be incorporated into the understanding of better performance approaches, and is thus proposed as an enabling dimension of performance. The enabling characteristic of uncertainty is of special interest to designing educational programs for providers in assistance to the amelioration of care through the application of available knowledge, be that from data relating processes to outcomes, or based on feedback providers get from patients and families about their expectations from the care. Tracing the keystone definitions of uncertainty from Hippocrates to Osler, the discussion addresses the dimensions of decision - making appropriateness, its timeliness, the expected and actual value of the care services, and the role of systematic communication between providers of care as well as with patients. The crucial role of Health Information Technology is emphasized, and a unifying model is proposed where the inclusion of uncertainty as a dimension of performance promotes an encompassing evaluation of the quality of health care services. Evaluation, influenced by tradition and precedent, is discussed as requiring the inclusion of health care and caring services.

Background: The quality of care perceived by the users of health care services is an important indicator of the quality of care. The aim of this study was to assess the satisfaction of patients received in obstetric and gynecological emergencies department of Yalgado Ouedraogo Teaching Hospital before and after the introduction of free care.Methods: This was a cross-sectional investigation. Data collection was carried out from February to July 2016, covering the last three months before the start of free care and the first three months of implementation of this free policy in Burkina Faso.Results: A total of 620 patients formed the sample. The reception (p=0.0001), the waiting period (p=0.0001), respect for treatment schedules (p=0.0001), respect for intimacy (p=0.0001), communication between providers and patients (p=0.007), the comfort of the delivery room (p=0.003) and the comfort of the ward room (p=0.002) were more favorably appreciated by patients before the free treatment than during that period. Overall patient satisfaction was better before the effectiveness of free care (p=0.003).Conclusions: The realization of free care process was followed by a lower patient’s satisfaction reflecting an alteration in the quality of health care services. A situational analysis of this free health care process is necessary in order to make corrective measures. Also adequate preventive measures should be adopted before any implementation to a larger scale of this free policy.

The article defines the scientific and terminological problems of researching services in the field of services, the basic problems of the evaluation of services in health care organizations. On the basis of a systematic approach to the category of quality of medical services the article investigates patient satisfaction with the perceived quality of service, shows the role of consumer expectations in the evaluation of the perceived quality of services and finds that the requirements for the service in medical institutions are underestimated. On the basis of studies the authors identify behaviors of consumers of budgetary medical services organizations, develop a model of consumer activities, which differs from the existing ones that along with the economic component the model is complemented with communication components. The approbation of the authors´ model for health care services shows a characteristic pattern of consumer activity of budgetary organizations. The article highlights the factors of subjective judgment of health care consumers in assessing perceived quality. Based on the study of patient satisfaction the authors develop a hierarchical model of the perceived quality of health services, as well as the place of services defined in the model.

<p>The emergence of knowledge management (KM) technologies has been enabling health care sectors to promote effective management of knowledge and provide evidence-based medicine for improving service deliveries. As a result, the health care industry is increasingly becoming a knowledge-based community that depends critically on using technologies to KM activities. This paper attempts to review various literatures and gives an intellectual background to the study from various studies conducted by different researchers on the thoughts of various KM technologies in health systems. According to the study, technologies play a prominent role in health care sectors in utilizing their organizational and personal knowledge effectively which in turn enhances the quality of health care services. The usage and practice of KM in the health sector is increasingly promoted due to the emergence of health information technology. There are number of information technology solutions that assist healthcare sectors in knowledge management. Health care portals, search engines, retrieval and classification tools, e-collaboration tools, content management systems and other technologies are widely employed to manage the massive and ongoing health information and share it with their users. Therefore, it is indispensable to pay special attention in using these technologies to manage organizational and individual knowledge available within and outside healthcare organization. Overall, this study has proved that information and communication technology are valuable to support and scale up knowledge management processes for the effectiveness and quality of health care services.</p>

Internet of Thing (IoT) has influenced several fields these days. Healthcare is one among them. The field of health care has been changed forever with the help of smart devices, wearable along with the overall level of inventions and connectivity in terms of the modern medical equipment. IoT, Cloud computing and other emerging technologies use data from different devices distributed across the network. Among those applications that are facilitated by the IoT, applications related to health care are most significant ones. Predictive analysis is carried out on the real-time data of patients to analyze their current situation for the purpose of effective and accurate clinical-decision making. Generally, internet of thing has been extensively utilized for interconnecting the advanced medical resource as well as for providing effective and smart health care services to the people. In order to monitor the condition of the patient, advanced sensors can be embedded or worn within the patient’s body. The data accumulated to such an extent that those data can be examined, aggregated as well as mined to do the initial predictions of diseases. Moreover, physicians are assisted by the processing algorithm for the personalization of treatment and at the same time thereby making the field of heath care more economical. This literature review is carried out by using the secondary data obtained from peer-reviewed journals and other sources on the web. This review aims to explain the use of IoT for providing smart healthcare solutions. The limitation of this study is that the major focus is on application side there by excluding the hardware and theoretical aspects related to the subject.

The aim of the study was to assess the inpatients’ opinion on the quality of hospital care based on the factor analysis and to identify the problem-oriented fields in quality of care. Material and methods. A multistage stratified probability sampling was performed in 22 general hospitals in Lithuania. A total of 2060 questionnaires were distributed during November 2006 and February 2007. The response rate was 97.38%; 2006 inpatients responded to the questionnaire; 1917 questionnaires (93.06%) were eligible for analysis. The modified survey instrument of Picker Institute Europe was used for inpatients. The method of survey was follows: each discharged inpatient filled out the questionnaire on the day of his/her discharge. The assessment of quality of care involved such aspects as patients’ communication with medical personnel, organizational issues and coordination of care, patients’ possibility of participation in medical decision-making, physical environment, accessibility to services, and safety of health care. Results. Six dimensions were identified from the factor analysis, explaining 51.48% of the variance. Cronbach alpha was 0.7931 for all dimensions. The majority (91.9%) of respondents evaluated health care services as good and very good. Inpatients were most satisfied with communication with their doctors. Even 91.5% of respondents noted that the doctors provided enough information about their health and treatment. The majority of inpatients gave high positive responses on respect showed to them and confidence with doctors. Correlation analysis confirmed a stronger positive correlation among three items of the global assessment of the quality and patients’ ratings on respect and confidence. Several problems were highlighted in the field of patients’ autonomy. The possibility of participation in medical decision-making was the dimension with the lowest level of patients’ rating. Less than half (42.3%) of inpatients noted that they did not have a possibility of participation in medical decision-making to the extent they were willing. Conclusions. Patients gave highly positive responses on the overall evaluation of the quality of health care services. The priority field in the improvement of health care quality is to create more possibilities for patients’ participation in medical decision-making. Results of the present study indicate that future studies need to include more detailed measurements of patients’ autonomy as dynamic changes are observed today in this field.

Pandemic is an epidemic that has becomes very widespread and affects the entire vicinity, a continent, and the world because of the vulnerable population. Management of pandemic consequence puts a great load on the healthcare sector. It demands great manpower from every sector for the smooth run of the daily essential services. Health care workers play a crucial role in delivering health care services during such pandemics. Physiotherapists are the allied health care professionals which provide primary, secondary, and tertiary level of health care services. The report provides a brief idea about the different roles played by physiotherapists for restoring ability and better functioning of an individual and society as a whole during various epidemics and pandemics.
