Estimating Key Performance Indicators of a New Emergency Department Model

In this article, a real-life Emergency Department (ED) is studied and analyzed in order to propose areas for improvement in its operations and patient flow. EDs are in native very busy and complex systems where medical facility treatments are provided to arriving patients without any prior appointment. ED, a 24/7 open facility, interacts with the majority of other departments of the healthcare system. Due to this complexity and unplanned nature of patient surge, simulation modeling is proven to be very effective in order to study the necessary changes needed for better performance. As a consequence of these challenges, the patient LoS (Length of Stay) and the human-resource utilization rates are increased and thus leading to staff and customer dissatisfaction which need to be addressed for better performance. An emergency department of a hospital in Lebanon is chosen for simulation using Arena software where a model is designed to match the real system. This model is then verified, validated and enhanced by proposing some modifications in the resource allocation levels. These improvements are achieved by running different scenarios using Arena Process Analyzer and suggesting an optimal solution using Arena OptQuest tool without the need of interrupting the real system.

Purpose – The purpose of this paper is to introduce a method of the bottleneck detection for Emergency Department (ED) improvement using benchmarking and design of experiments (DOE) in simulation model. Design/methodology/approach – Four procedures of treatments are used to represent ED activities of the patient flow. Simulation modeling is applied as a cost-effective tool to analyze the ED operation. Benchmarking provides the achievable goal for the improvement. DOE speeds up the process of bottleneck search. Findings – It is identified that the long waiting time is accumulated by previous arrival patients waiting for treatment in the ED. Comparing the processing time of each treatment procedure with the benchmark reveals that increasing the treatment time mainly happens in treatment in progress and emergency room holding (ERH) procedures. It also indicates that the to be admitted time caused by the transfer delay is a common case. Research limitations/implications – The current research is conducted in the ED only. Activities in the ERH require a close cooperation of several medical teams to complete patients’ condition evaluations. The current model may be extended to the related medical units to improve the model detail. Practical implications – ED overcrowding is an increasingly significant public healthcare problem. Bottlenecks that affect ED overcrowding have to be detected to improve the patient flow. Originality/value – Integration of benchmarking and DOE in simulation modeling proposed in this research shows the promise in time-saving for bottleneck detection of ED operations.

Abstract Network information mining is the study of the network topology, which may answer a large number of application-based questions towards the structural evolution and the function of a real system. The question can be related to how the real system evolves or how individuals interact with each other in social networks. Although the evolution of the real system may seem to be found regularly, capturing patterns on the whole process of evolution is not trivial. Link prediction is one of the most important technologies in network information mining, which can help us understand the evolution mechanism of real-life network. Link prediction aims to uncover missing links or quantify the likelihood of the emergence of nonexistent links from known network structures. Currently, widely existing methods of link prediction almost focus on short-path networks that usually have a myriad of close triangular structures. However, these algorithms on highly sparse or long-path networks have poor performance. Here, we propose a new index that is associated with the principles of Structural Equivalence and Shortest Path Length (SESPL) to estimate the likelihood of link existence in long-path networks. Through 548 real networks test, we find that SESPL is more effective and efficient than other similarity-based predictors in long-path networks. Meanwhile, we also exploit the performance of SESPL predictor and of embedding-based approaches via machine learning techniques. The results show that the performance of SESPL can achieve a gain of 44.09% over GraphWave and 7.93% over Node2vec. Finally, according to the matrix of Maximal Information Coefficient (MIC) between all the similarity-based predictors, SESPL is a new independent feature in the space of traditional similarity features.


Herewith a methodology is proposed towards obtaining optimum level as solution for the real-life problem using Transportation. This paper is composed for a travel agency company to get maximum profit and also to minimize the transportation cost. In the proposed method maximum value is marked both in row-wise and column-wise for maximization type and minimum valueis marked both in row-wise and column-wise for minimization type and the greatest maximum value is given allocation. Here a New algorithm is used to solve the transportation model to make maximum profit for the Company. This method gives the optimal solution in less iteration.

Purpose – The purpose of the paper is to analyze the development needs and opportunities in the distribution system operators’ (DSO) tariff structures in the smart-grid environment. Design/methodology/approach – The impacts of the distribution pricing schemes for the stakeholders and their requirements for the tariff structures are evaluated by qualitative analyses. Furthermore, there is a case analysis concerning the practical development possibilities of the DSO tariff structures in Finland. Findings – Major finding of the paper is that the demand-based power band tariff is the optimal solution for the DSO pricing structure, when taking into account the real-life limitations and the requirements of the stakeholders. Practical implications – Outcomes of the paper can be applied in practice in design of the pricing schemes in the electricity distribution. Incentive provision impacts and cost reflectivity of the DSO tariffs can be improved by implementing the suggested pricing structure. Originality/value – The paper provides a novel viewpoint on the study of the DSO tariff design by considering the impacts of the pricing for the stakeholders and their requirements for tariff structure. Furthermore, the real-life limitations in the tariff design have been taken into account by analyzing the development options in Finland. Results are useful, especially for the DSOs, regulators and academics, who are working with the tariff development issues.

In Italy, at the end of the 1970s, methadone hydrochloride was introduced for the treatment of opioid use disorder, in the form of a racemic mixture consisting of levomethadone and dextromethadone.In 2015 Levometadone was introduced, a new formulation marketed in Italy for the treatment of opioid use disorder in 2015.The article aims to bring the experience of an Italian Addiction Centre back to the use of this new formulation in the "real life" analyzing the efficacy, the trend of adverse events and pharmacological iterations in a context in which the treated population often uses besides the opiates, cocaine and alcohol, are burdened by a relevant physical and psychic comorbidity and frequently have a prescribed polypharmacy.

Abstract We are in the fifth era of marketing development, that of relationship marketing, where the environment is influenced by globalization, informatization, market liberalization, and the overcapacity of production, and the focus will be not on increasing the volume of production, but on obtaining profit using relationship with profitable customers. Customer-centricity, customization, one-to-one relationship and measuring the value of each customer is an advantage in the fearless market. Organizations if they knew what customers wants would offering only those products or services they want, but to help them to know the customers’ needs better, they use models, new methods and calculation of performance indicators. One of the models which help organizations to determine the customer value and centricity is Net Promoter Score (NPS), its formula being developed by Reichheld F. in 2006. Our research is made on 10 organizations and on 150 respondents and is consisting in using NPS in order to observe customers’ perception of the analysed brands, to measure how well the brand is having satisfayed and loyal customers, and the desire for recommendation of the analysed brand. Our article is using simulation, modeling and IT programs to observe possible relationships and future trends.
