Oral Health Status and Development of Ventilator-Associated Pneumonia: A Descriptive Study

• Background Ventilator-associated pneumonia is a significant cause of morbidity and mortality and may be influenced by oral health. • Objective To describe the relationship between ventilator-associated pneumonia and oral health status, changes in oral health status during the first 7 days after intubation, and microbial colonization of the oropharynx and trachea. • Methods A total of 66 patients were enrolled within 24 hours of intubation and were followed up for up to 7 days. Data on oral health measures and the Clinical Pulmonary Infection Score (CPIS) were collected at baseline, day 4 (n = 37), and day 7 (n = 21). A regression model was used to predict risk of pneumonia at day 4. • ResultsDental plaque and oral organisms increased over time. Correlations were significant for baseline and day 4 dental plaque (P < .001), baseline salivary lactoferrin and day 4 plaque (P = .01), and lower salivary volume and higher day 4 CPIS (P = .02). Potential pathogens were identified in oral cultures for 6 patients before or at the same time as the appearance of the organisms in tracheal aspirates. Correlations were significant with day 4 CPIS for score on the Acute Physiology and Chronic Health Evaluation (APACHE) II (P = .007), day 4 salivary volume (P = .02), interaction of APACHE II score and day 1 CPIS (P<.001), and interaction of day 1 CPIS and plaque (P=.01). • Conclusions Higher dental plaque scores confer greater risk for ventilator-associated pneumonia, particularly for patients with greater severity of illness. Salivary volume and lactoferrin may affect the risk.

Objective: To identify the prevalence of caries, plaque score, and periodontal disease and explore the correlations between oral health literacy scores and oral health status among undergraduate students. Materials and Methods: A cross-sectional study was conducted with a group of undergraduate students using the Malay version of the Oral Health Literacy (OHL) Index. A stratified random sampling technique was used to select the samples. Clinical oral examinations were conducted by a single trained examiner using a Community Periodontal Index (CPI) probe and a mouth mirror. Data analysis was conducted using statistical tests contained in SPSS version 26, and statistical significance was set at p<0.05. Results: A total of 280 students were recruited into the study. The prevalence of caries and periodontal disease was 58.9 and 13.2%, respectively. There was a significant association between OHL and field of study (p<0.001). There was also a significant negative but weak correlation between OHL and dental plaque scores (r= -0.147; p=0.014). A significant positive but weak correlation was found between dental plaque scores and dental caries (r= 0.135; p=0.024) and periodontal pocket depth (r= 0.168; p=0.005). Conclusion: The prevalence of dental caries was high, while the prevalence of periodontal diseases was low in this study. OHL correlated significantly with dental plaque scores and field of the study. Thus, OHL assessment is essential to understand a person’s OHL levels and can be considered a screening tool for early detection of poor oral hygiene.

ABSTRACTThe human oral cavity is home to a large and diverse community of viruses that have yet to be characterized in patients with periodontal disease. We recruited and sampled saliva and oral biofilm from a cohort of humans either periodontally healthy or with mild or significant periodontal disease to discern whether there are differences in viral communities that reflect their oral health status. We found communities of viruses inhabiting saliva and the subgingival and supragingival biofilms of each subject that were composed largely of bacteriophage. While there were homologous viruses common to different subjects and biogeographic sites, for most of the subjects, virome compositions were significantly associated with the oral sites from which they were derived. The largest distinctions between virome compositions were found when comparing the subgingival and supragingival biofilms to those of planktonic saliva. Differences in virome composition were significantly associated with oral health status for both subgingival and supragingival biofilm viruses but not for salivary viruses. Among the differences identified in virome compositions was a significant expansion of myoviruses in subgingival biofilm, suggesting that periodontal disease favors lytic phage. We also characterized the bacterial communities in each subject at each biogeographic site by using the V3 hypervariable segment of the 16S rRNA and did not identify distinctions between oral health and disease similar to those found in viral communities. The significantly altered ecology of viruses of oral biofilm in subjects with periodontal disease compared to that of relatively periodontally healthy ones suggests that viruses may serve as useful indicators of oral health status.IMPORTANCELittle is known about the role or the constituents of viruses as members of the human microbiome. We investigated the composition of human oral viral communities in a group of relatively periodontally healthy subjects or significant periodontitis to determine whether health status may be associated with differences in viruses. We found that most of the viruses present were predators of bacteria. The viruses inhabiting dental plaque were significantly different on the basis of oral health status, while those present in saliva were not. Dental plaque viruses in periodontitis were predicted to be significantly more likely to kill their bacterial hosts than those found in healthy mouths. Because oral diseases such as periodontitis have been shown to have altered bacterial communities, we believe that viruses and their role as drivers of ecosystem diversity are important contributors to the human oral microbiome in health and disease states.

Objectives. The aim of this study was to determine the oral health status of elderly persons in Tonga, West Region of Cameroon.Methodology. This is a cross-sectional study of persons of at least 65 years, living in Tonga village, West Region of Cameroon.Results. A total of 183 persons aged between 65 and 94 years, mean age of 73 years ±7 s.d., 83 (45,4%) males, and 100 (54,6%) females participated in the study. The most represented age range was 65–74 years (60.1%); 86 (47.3%) and elders above 65 constituted 1.8% of the total population. More than a third 117 (41.4%) had visible dental plaque, 117 (48,6%) had periodontal pockets >4 mm, 153 (54,1%) had teeth with total crown destruction, 70 (38.3%) had not lost a tooth, 23 (12.6%) had lost 1 tooth, 19 (10.4%) have lost at least 2 teeth, 100 (55.7%) were partially edentulous at the maxilla and 98 (53.6%) at the mandible, 2 (1.1%) were completely edentulous at the maxilla and 3 (1.6%) at the mandible, and 3.8% had removable dentures. The mean DMF index was 6.11 and 69.4% had dental caries. Risk factors to dental caries were toothbrushing and tobacco consumption while dental plaque was associated to pocket depth of 4–6 mm. Barriers to oral health care included ignorance 47 (25.7%), financial difficulties 124 (67.8%), and distance to the nearest clinic 12 (6.5%).Conclusion.The oral status of the elderly was generally poor.

Abstract Objective The aim of this case-control study was to assess the smoker and non-smoker patients’ oral health status versus students. Methods: The study was based on a clinical examination of 210 young adults (118 women, 92 men) aged between 22-32 years. 105 of these were students in the 5th and 6th grade of the Faculty of Dentistry in Tîrgu Mureș and the control group was represented by patients. Patients and students were classified into three groups: non-smokers, occasional- and active smokers. The clinical examination evaluated the dental caries, fillings, extractions, crowns, root remnants and dental plaque. DMF-S index scores were calculated, and with its help the index of treatment need, dental care index and intensity of caries were determined. Results: The dental treatment need index, DMF-S index and intensity of caries showed increased values in patients. Root remnants (p <0.0001), decay (p = 0.0473) and dental plaque (p = 0.0363) were much higher in the group of active smoker students as in the non-smoker students. Among active smoker patients the incidence of dental calculus (p = 0.0005), of root remnants (p = 0.0022) and of fillings (p = 0.0441) showed a higher value than in the non-smokers. Conclusions: This study showed that compared with non-student patients, in dental students, a better oral hygiene was coupled with healthier teeth and periodontal. Poor oral hygiene together with smoking seems to worsen the oral health status.
