Sixty-four cases of discharge against medical advice (DAMA) from 3 paediatric units in the University of Benin Teaching Hospital within a period of 18 months were analysed. The incidence was highest among the very young, even when they were still very ill. Financial problems accounted for 65.6% of the cases. Other reasons included difficulties related to the siblings (9.4%), the wish to try traditional methods (7.8%), family opposition (1.6%) and distance from the hospital (4.1%). It is recommended that greater attention be given to preventive health care, and placing the cost of hospital care within the reach of the poor. Improved communication is also recommended as a way of helping parents seek alternatives to DAMA.

The paper deals with well cementing, which is a complex engineering task requiring special tools and technologies. The authors give an example of complications that arise due to the poor quality of well cementing. The aim of the work is to study the mineral ultrafine additive-nanosilicates, which increase the strength of cement stone and reduce its porosity. Nanosilicate is formed during the cleaning of ore-thermal furnaces in metallurgical plants. One of such technologies is application of metallurgical waste, since it can improve the environment by cleaning the sludge fields from waste and reduce the cost of cement slurry by reducing the amount of cement in the composition. The study provides practical data showing that nanosilicates positively affect the strength of cement stone, reducing its porosity.

The normal English child has thriven apace of recent years, as the result of a healthy popular tendency to foster his mental and physical well-being as thoroughly as may be. But there exists among the poor a race of unfortunates–happily a small one–whose interests have rarely met with adequate recognition. It consists of the imbeciles who find their way into pauper lunatic asylums–the children legally certifiable as idiots. They cannot, I admit, pay a dividend in hard cash on the money sunk in their education, and their appeal for special attention mainly rests on their helplessness. My aim is to speak briefly of the measures already taken in their interest elsewhere, and to describe in a little more detail those adopted with success at Hellingly. Lastly, I propose to discuss the cost of our scheme, and to attempt to analyse the benefits derived therefrom.

Chapter 19 describes the journey that Ghana has been on since 2003 as it sought to bring together existing community based health insurance schemes of many sorts within a single national framework, which offered a package of care to all its citizens—however poor and from whatever background. It covers the challenges in integrating the existing schemes, in applying different aspects of the policy, in funding, and in reaching the poorest. It also discusses how the government has changed and with it some aspects of policy, and how, after 10 years, around a third of the population are active members of the National Health Insurance Scheme and there is a foundation in place for continuing the journey to ensure that health services are available to everyone as the country continues to grow and prosper.

In his book, The Roman Empire (Fontana History of the Ancient World Series), Colin Wells mentions incomes and the cost of living in first-century Italy. He concludes that a wage of 4 sesterces a day was an ‘absolute maximum’ and goes on to comment that ‘it is hard to tell whether the discrepancy between the really rich and the labouring poor was greater in some third-world countries today, or in Victorian England… To prove his point Wells gives £300,000 per annum for the Marquess of Bute as an example of an enormous income in the nineteenth century, though he does not tell us what the poor would have earned then.

This paper touches problem of transportation pollution focusing on NOx reducing in the India. There were India country chosen for study as the second populated country and most polluted cities in the world. As from statistics, it is known that more than 65% of Indian cars are old and they are the main reason of air pollution. Most of the old cars do not have any control measures for eliminating deadli-est gases. Dumping the old cars is not possible, as the poor owns most of the old cars. For eliminating the pollution from old cars, the possible remedy is to control the emission of pollutants. Comparing to all gases in exhaust, nitrogen oxides are the dangerous one. It can may cause up to death. The best method to control the NOx gas is EGR (Exhaust Gas Recirculation) valves. This research mainly focuses on the possible ways of installing EGR in old engines and fabricate an EGR in one of the Indian cars. Maruti Suzuki 800 is best of for fabricating EGR, as it is a base model and most sold Indian car. As the result, there tremendous decreasing in NOx emission also the emission CO2 was reduced. The investigation about fabricating EGR in old vehicle results a positive output after calculated the cost of the fabrication, time consumption, work challenges and other facts. As Indian government adopt this concept, they can reduce the pollu-tion from all types of vehicles to a great extend in few years of time with low investment.

Pharmacy costs are one of the components that absorb the largest costs from the hospital's health budget. At present, there is still little information and data on the percentage of pharmaceutical costs to the costs of medical devices and non-medical devices. It is very useful in making financial planning. It can illustrate of pharmaceutical financing against other operational costs in hospitals, as financial planning, as scientific evidence for improving drug policy. This research was an analysis on the Indonesia Health Financing Survey data in the referral health care Facilities (FKRTL) in 2016. Data from 84 hospitals in 3 regions. The details were 9 class A hospitals, 37 class B hospitals, 38 class C hospitals. The analysis used a descriptive statistical approach. Pharmacy costs in 3 types of hospitals were 169.07 Billion, 23.66 Billion, 10.79 Billion. The percentage of the proportion of pharmaceutical costs to the cost of non-medical equipment in 3 types of hospitals is 1073.05%, 665.42%, 292.29%. The proportion of pharmaceutical costs to the cost of medical devices in 3 types of hospitals is 208.12%, 172.20%, 148.96%. In all types of hospitals the total pharmaceutical costs are greater than the costs of medical equipment and non-medical equipment costs. The proportion of pharmaceutical costs to the costs of non-medical devices and medical devices is high. This is due to the depreciation of the cost of medical devices and non-medical devices while the pharmaceutical costs always increase in price from the previous year.
