This narrative review evaluates strength or resistance training on cardiorespiratory endurance, blood pressure, contractile function, contractile protein synthesis rate, bone turnover, gait and balance, and neuromuscular adaptations in elderly populations. Seventy-eight studies spanning from 1999 through 2020 were reviewed. Database sources including PubMed, Science Direct, Web of Knowledge and Google Scholar were searched in accordance with the purpose of the study. A majority of the studies reported that resistance training reduces blood pressure and increases contractile functions, contractile protein synthesis rate, bone turnover, gait and balance, cardiorespiratory endurance, and neuromuscular adaptations in the elderly. Furthermore, combined training (CT), also known as concurrent training (strength plus endurance training) may also be as effective as traditional endurance training or traditional strength/resistance training alone for improving cardiorespiratory endurance and functional performance. According to the evaluation of studies included in this review, we concluded that training modalities that involve low-load, high velocity strength training combined with endurance training might be the best training strategy in improving cardiovascular fitness, functional capacity and musculoskeletal health in the elderly populations. Elderly people should be encouraged to participate in a concurrent training or a combination of strength and endurance training to delay, or even reverse the negative effects of aging. <p> </p><p><strong> Article visualizations:</strong></p><p><img src="/-counters-/edu_01/0875/a.php" alt="Hit counter" /></p>

Abstract Background The effect of concurrent training on the development of maximal strength is unclear, especially in individuals with different training statuses. Objective The aim of this systematic review and meta-analysis study was to compare the effect of concurrent resistance and endurance training with that of resistance training only on the development of maximal dynamic strength in untrained, moderately trained, and trained individuals. Methods On the basis of the predetermined criteria, 27 studies that compared effects between concurrent and resistance training only on lower-body 1-repetition maximum (1RM) strength were included. The effect size (ES), calculated as the standardised difference in mean, was extracted from each study, pooled, and analysed with a random-effects model. Results The 1RM for leg press and squat exercises was negatively affected by concurrent training in trained individuals (ES =  – 0.35, p < 0.01), but not in moderately trained ( – 0.20, p = 0.08) or untrained individuals (ES = 0.03, p = 0.87) as compared to resistance training only. A subgroup analysis revealed that the negative effect observed in trained individuals occurred only when resistance and endurance training were conducted within the same training session (ES same session =  – 0.66, p < 0.01 vs. ES different sessions =  – 0.10, p = 0.55). Conclusion This study demonstrated the novel and quantifiable effects of training status on lower-body strength development and shows that the addition of endurance training to a resistance training programme may have a negative impact on lower-body strength development in trained, but not in moderately trained or untrained individuals. This impairment seems to be more pronounced when training is performed within the same session than in different sessions. Trained individuals should therefore consider separating endurance from resistance training during periods where the development of dynamic maximal strength is prioritised.

The purpose of this study was to determine whether growth hormone (GH) administration enhances the muscle anabolism associated with heavy-resistance exercise. Sixteen men (21-34 yr) were assigned randomly to a resistance training plus GH group (n = 7) or to a resistance training plus placebo group (n = 9). For 12 wk, both groups trained all major muscle groups in an identical fashion while receiving 40 micrograms recombinant human GH.kg-1.day-1 or placebo. Fat-free mass (FFM) and total body water increased (P less than 0.05) in both groups but more (P less than 0.01) in the GH recipients. Whole body protein synthesis rate increased more (P less than 0.03), and whole body protein balance was greater (P = 0.01) in the GH-treated group, but quadriceps muscle protein synthesis rate, torso and limb circumferences, and muscle strength did not increase more in the GH-treated group. In the young men studied, resistance exercise with or without GH resulted in similar increments in muscle size, strength, and muscle protein synthesis, indicating that 1) the larger increase in FFM with GH treatment was probably due to an increase in lean tissue other than skeletal muscle and 2) resistance training supplemented with GH did not further enhance muscle anabolism and function.

Abstract Introduction: Osteoarthritis (OA), the most common form of arthritis, is considered the main cause of pain and disability in the elderly. Objective: To evaluate the effect of systematic muscle strength training on functional performance and quality of life in individuals with knee OA. Methods: Subjects with knee OA (n = 27, 46 - 76 years) completed the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), Medical Outcomes Short-Form 36-item Health Survey (SF-36), and visual analog scale (VAS) questionnaires, musculoskeletal assessments, and 10-repetition maximum and timed 10-meter walk tests both before and after training. The training consisted of an exercise resistance program and stretches for 12 weeks (three sessions of 80 each per week). Results: Twenty-two subjects completed the training. Reduced overall scores and WOMAC physical function indicated improved functional performance (p < 0.001) as well as increased gait speed (p < 0.001). The perception of pain decreased after training, as evidenced by the VAS, WOMAC pain domain, and SF-36 scores (p < 0.001). Quality of life improvements occurred primarily in the areas of pain, functional capacity, and SF-36 physical aspects. No change in body mass index was noted (p = 0.93). Conclusion: Our results indicate that the combination of resistance training for the quadriceps, gluteus, and abdominal muscles could be a viable alternative to improving functionality and quality of life in patients with knee OA. However, more studies are necessary to confirm our findings.

Endurance training is associated with increases in mitochondrial density, of which cytochrome c protein is an index. Increases in the synthesis rates of cytochrome c protein in skeletal muscle during endurance training have been inferred (Biochem. Biophys. Res. Commun. 66: 173, 1975; J. Biol. Chem. 252: 416, 1977). One purpose of the present study was to test these indirect approximations with direct measurements of the synthesis rates of cytochrome c protein in skeletal muscles postexercise. No change in the fractional synthesis rate of cytochrome c was detected in the red quadriceps muscle of rats either 2–7 h after a 104-min run on a motor-driven treadmill or 17–22 h after the final bout of 4 days of running 100 min/day. If the 16% increase in cytochrome c protein concentration in the red quadriceps muscle on the 5th day of training is used to calculate the nanomoles of cytochrome c synthesized per gram of wet muscle weight, the normalized rate of cytochrome c protein synthesis is increased 29% on the 5th day of training. The observation of no significant alteration in cytochrome c mRNA in the red quadriceps muscle of rats during the 1st wk of training implies that the initial increase in the synthesis rate of cytochrome c protein normalized per unit of muscle mass during treadmill training is likely to occur at a translational or posttranslational step. These results suggest that the control of increased cytochrome c expression in skeletal muscle during exercise training involves a complex mechanism.

Muscle atrophy (sarcopenia) in the elderly is associated with a reduced rate of muscle protein synthesis. The purpose of this study was to determine if weight-lifting exercise increases the rate of muscle protein synthesis in physically frail 76- to 92-yr-old women and men. Eight women and 4 men with mild to moderate physical frailty were enrolled in a 3-mo physical therapy program that was followed by 3 mo of supervised weight-lifting exercise. Supervised weight-lifting exercise was performed 3 days/wk at 65–100% of initial 1-repetition maximum on five upper and three lower body exercises. Compared with before resistance training, the in vivo incorporation rate of [13C]leucine into vastus lateralis muscle protein was increased after resistance training in women and men ( P < 0.01), although it was unchanged in five 82 ± 2-yr-old control subjects studied two times in 3 mo. Maximum voluntary knee extensor muscle torque production increased in the supervised resistance exercise group. These findings suggest that muscle contractile protein synthetic pathways in physically frail 76- to 92-yr-old women and men respond and adapt to the increased contractile activity associated with progressive resistance exercise training.

Objective: Aging is one of the stages of life that needs special attention due to the special conditions of this period of life. Inactivity and myasthenia due to aging are important factors in reducing the physical and functional activities in the elderly, which can affect their quality of life. On the other hand, electromyostimulation (EMS) is one of the modern training methods that can be suitable for disabilities in the elderly. Also, the training sequence in concurrent training is one of the variables of training that can affect its adaptations. This study aimed to compare the effect of exercise sequence in concurrent training with EMS on the motor performance of elderly women. Materials & Methods: In this semi-experimental single-blinded study, 50 healthy elderly female volunteers, age range of 60-70 years, were selected by random sampling divided randomly into the following groups (each group of 10 people): Aerobic-resistance training with EMS, resistance-aerobic exercise with EMS, rotational exercise (change of priority periodically in training sessions) with EMS, and rotational and control (without training) groups. The training protocol consisted of twelve weeks of exercise, three sessions per week, and each session three stages of warm-up (10-15 minutes), main exercises (20 minutes) and cooling (10 minutes). The main training program consisted of 20 minutes of parallel combination exercises, which were two 10-minute steps with a 3-5 minute intervals. Aerobic exercises were performed with the intensity of 70%-50% of maximum oxygen consumption and resistance exercises using body weight and elastic bands for different muscle groups. The training schedule was the same for all groups in terms of volume and intensity, with the only difference being in the exercise sequence and EMS presentation. Strength of the upper and lower torso muscles were measured with chest press and seated leg extension, respectively, maximum oxygen consumption (VO2 max) with a one-mile Rockport walking test, muscular endurance with a 2-minute marching on a spot-test and lower torso strength with a walking test on the slope in two stages before and after the intervention. Statistical analysis was performed using dependent t-tests and Covariance Analysis (ANCOVA) and Bonferroni post hoc test using SPSS software V. 22 and P≥0.05 was considered statistically significant. Results: Significant increase was observed in all measured dependent variables relative to the baseline values (P<0.001). In the study of intergroup changes, a significant increase was observed in VO2 max level, upper and lower torso strength, and muscular endurance in all of the intervention groups compared to the control group (P<0.001), but lower torso strength was significant only in the intervention groups with EMS compared to the control group (P<0.001). VO2 max changes and the lower torso strength in training groups with EMS were significantly higher than the groups without EMS (P<0.01). Also, the upper torso strength and the endurance of the lower torso muscles in the group with the priority of resistance training with EMS were higher than the combination group without EMS (P<0.01). Conclusion: Based on the results, it can be said that combined concurrent exercises are a useful method in promoting physical fitness and physical performance of elderly women, and the use of EMS can increase the effectiveness of adaptations resulting from exercise, especially lower torso strength in the elderly and can prevent complications associated with myasthenia. On the other hand, the training sequence with the priority of resistance training can be effective in increasing the effect of exercise on the variables of upper torso muscle strength and muscular endurance, but it does not have a significant effect on lower torso strength, cardiorespiratory endurance and lower torso strength.

Background: The losses of strength, agility, balance, and functionality caused by aging are harmful to the elderly population. Resistance training (RT) may be an efficient tool to mitigate such neuromuscular decline and different RT methods can be used. Therefore, it is important to investigate the different responses to different training methods. Hypothesis: Eight weeks of traditional resistance training (TRT) are expected to promote similar results to high-speed training (HST) in physical functional performance (PFP) and quality of life in the elderly. Study Design: A clinical trial. Level of Evidence: Level 3. Methods: Participants (n = 24) with a mean age of 67.8 ± 6.3 years completed 8 weeks of RT. They were allocated into HST (n = 12) and TRT (n = 12). TRT involved training with 10 to 12 repetitions at controlled velocity until momentary muscle failure, while HST involved performing 6 to 8 repetitions at 40% to 60% of 1 repetition maximum (1RM) at maximum velocity. Pre- and posttraining, the participants were tested for (1) maximum strength in the 45° leg press and chest press; (2) PFP in the 30-second chair stand, timed-up-and-go (TUG), and medicine ball throw test; and (3) quality of life. Results: Both groups improved muscle strength in the 45° leg press, with greater increases for TRT (HST: +21% vs TRT: +49%, P = 0.019). There was no change in chest press strength for HST (−0.6%) ( P = 0.61), but there was a significant increase for the TRT group (+21%, P = 0.001). There was a similar improvement ( P < 0.05) for both groups in TUG (HST: 7%; TRT: 10%), chair stand (HST: 18%; TRT: 21%), and medicine ball throwing performance (HST: 9%; TRT: 9%), with no difference between groups ( P = 0.08-0.94). Emotional aspect significantly increased by 20% ( P = 0.04) in HST and 50% ( P = 0.04) in TRT. Conclusion: Both TRT and HST are able to promote improvements in functional performance in the elderly with greater in strength gains for TRT. Therefore, exercise professionals could choose based on individual characteristics and preferences. Clinical Relevance: The findings provide important insights into how health care professionals can prescribe HST and TRT, considering efficiency, safety, and individual aspects.
