Dermatomyositis (DM) is the most frequently occurring Idiopathic Inflammatory Myopathy (IIM) of skeletal muscles causing proximal muscles weakness. The average age at which this disease affects is 40 years and almost twice as many women are affected as men. Its prevalence rate is 2-10 per 1,00,000 in the general population. The actual cause is unknown but the disease has much common with autoimmune disorder in which your immune system mistakenly attacks our body tissue. Small blood vessel in muscular tissue are particularly affected in dermatomyositis. In this disease the ESR and CPK is usually raised. Due to similarity in the signs and symptoms, we can correlate this disease with Mamsavrita-Vata and its treatment can be planned according to it. Here is the case study of 40 years old female patient, diagnosed as Dermatomyositis since 3 months. Patient admitted in Panchakarma ward of Parul Ayurved Hospital, Vadodara. The Panchakarma procedures like Udvartana, Nitya Virechana, Basti Chikitsa, Shasti Shali Pinda Sweda and Shamana Aushadi like Cap Palsineuron and Guduchi Rasayana etc. are given. At the end of the treatment marked improvements were seen in the patient, like reduced ESR and CPK levels. Also the symptoms of the disease reduced significantly.

Abstract: SLE an autoimmune disorder where mulitisystem is involved.Its management is done by analgesics and cortiocosteriods . Drug resistance ,increasing doses of haepatotoxic drugs and imunosupressants steroids are hazards of the management .Here the patient suffering from SLE  was unable to do her day today work. Panchakarma treatment is found very effective in this case to improve her quality of life and to live with her own.

Dermatomyositis (DM) is an idiopathic inflammatory myopathy, that can be associated with malignancy. We report the case of a 60-years-old woman, diagnosed and treated for left breast cancer, with residual neoplasm following treatment, who was admitted for generalized myalgias and tetraparesis with predominance of paraparesis. Clinical exam revealed heliotrope rash with bilateral palpebral edema and an erythematous eruption on both thighs and anterior thorax. Motor deficits were predominantly proximal. Paraclinical evaluation excluded other etiologies and revealed elevated serum muscle enzymes and electromyographic pattern of myopathy. An MRI was performed and showed diffuse edematous infiltration of skeletal muscles. Symptoms diminished under corticotherapy, intravenous immunoglobulins and cyclophosphamide. Left mastectomy was eventually performed, but DM symptoms reappeared a few months later. Despite extensive search, no recurrence of tumor was found. However, under treatment, the patient once again recovered, and later resumed her activity.

Skeletal muscle metastases are very rare events in colorectal carcinoma. By contrast, dermatomyositis is an idiopathic inflammatory myopathy with characteristic cutaneous manifestations and a well-recognized association with several human malignancies and, among others, colorectal cancer. Here, we report the case of a 71-year-old woman with paraneoplastic dermatomyositis followed by the development of a metastatic colon cancer. Interestingly, this patient developed multiple skeletal metastases which were preceded by the worsening of systemic symptoms of dermatomyositis. This observation suggests that, while muscle tissue is usually resistant to the development of tumor metastases, the inflammatory and immune response which characterizes and boosts paraneoplastic myopathy may represent a favorable soil for tumor cell invasion and metastasization to skeletal muscles.

The antifibrotic therapies nintedanib and pirfenidone were first approved by the United States for the treatment of idiopathic pulmonary fibrosis in 2014. In 2020, nintedanib received U.S. Food and Drug Administration (FDA) approval for the treatment of all progressive fibrosing interstitial lung disease (ILD). Given that a major cause of mortality and morbidity in the idiopathic inflammatory myopathies (IIM) is progressive interstitial lung disease and respiratory failure, antifibrotic therapies may be useful as adjuvant to traditional immunosuppression. However, randomized controlled trials of antifibrotic therapies in IIM are lacking. The purpose of this review is to (1) summarize the mechanism of action of nintedanib and pirfenidone in ILD with possible role in IIM-ILD, (2) review the clinical data supporting their use in interstitial lung disease in general, and more specifically in connective tissue disease associated ILD, and (3) discuss the evidence and remaining challenges for using antifibrotic therapies in IIM-ILD.

A substantial and increasing body of clinical evidence supports the role of specific strains and mixtures of probiotics in the prevention and treatment of certain diseases. Several general mechanisms of action have been proposed, including supporting repair of hyperpermeable epithelial barriers, interfering with infection by pathogens, and restoring a healthful balance of commensal microbes to affect metabolism. Emerging evidence supports an additional role of probiotics as important modulators of immune system responses, including inflammation, at mucosal surfaces. In particular, by preventing or repairing ‘leaky’ epithelial barriers, probiotics can indirectly affect the inflammatory response by negating the source of pro-inflammatory stimuli associated with low-grade endotoxemia. They also enhance production of short chain fatty acids with anti-inflammatory properties (e.g. butyrate) as well as increase synthesis of antimicrobial peptides that influence inflammation resolution pathways in the mucosa. Furthermore, probiotics and some of their secreted metabolic products can act as ligands for innate immune system receptors, directly influencing key pro-inflammatory pathways. They also stimulate the differentiation and activity of important immune cells (e.g., dendritic cells, T cells), and subsequently increase production of important regulatory cytokines, including interleukin-10 (IL-10) and transforming growth factor-beta (TGF-b). Finally, there are limited but increasing animal studies and clinical trials demonstrating probiotics do affect common biomarkers of inflammation, including C-reactive protein, as well as signs and symptoms of the associated diseases suggesting they can have therapeutic benefit in the treatment of chronic inflammatory disease.Keywords: probiotics, inflammation, endotoxemia, epithelial, cytokines, immune, butyrate, antimicrobial peptides

Dandruff is a common scalp ailment that causes flaking of the skin. It is neither communicable nor dangerous. It can, however, be humiliating and difficult to treat. Darunaka is a Kapalagataroga, but Acharya Sushruta described it as a Kshudraroga due to the vitiation of Vata and Kapha Doshas, with symptoms such as Kandu (itching on the scalp), Keshachyuti (hair loss), Swapa (abnormalities of touch sensation on the scalp), Rookshata (roughness or dryness of the scalp), and Twaksphutana (breaking or cracking of the scalp skin). Seborrheic Dermatitis is an irri- tative scalp illness characterized by the shedding of dead tissue from the scalp, as well as an itchy sensation. Seb- orrheic Dermatitis affects roughly 4% of the population, while dandruff (mild seborrheic dermatitis of the scalp) affects about half of all adults, according to studies. It can begin at any point after puberty and is more common in men. It might lead to social and self-esteem issues. A 25-year-old male patient from Jodhpur came to OPD of Panchakarma, with a chief complaint of Shira Kandu (itching on the scalp), Rukshata (dryness on the scalp), Twak- sphutana (cracks in the skin) and Keshachyuti (hair fall). In this case, Ayurvedic formulations and Panchakarma therapy gave the patient till 30 days. The improvement provided by the therapy was assessed based on signs and symptoms before and after the treatment, The therapy's effectiveness was determined by comparing signs and symp- toms before and after treatment. For evaluating signs and symptoms, a unique scoring scheme was created. Themedicine was proven to be both safe and effective at treating signs and symptoms as well as enhancing the general health of the scalp. The role of the immune system is to protect the body from germs and diseases. A food allergy is the overreaction by the immune system to a food protein. People are allergic to many kinds of food, majorly nuts. Anaphylaxis is a severe allergic reaction that comes on quickly and could even cause death in certain cases. The aim of this study was to assess awareness on nut allergies and to study the already existing knowledge on the topic among school students. Self administered questionnaire was created based on the awareness of the presence of nut allergy, their signs and symptoms. The questionnaire was distributed by person as fill out sheets at a nearby school. The study population included 200 school students belonging to the age group of 8 to 16 and years. The questions were carefully studied and corresponding answers were marked by the participants. The data was collected and statistically analysed. According to the survey results, about 81% of the total school population were aware about the presence of nut allergy. About 85% voted prevention is better to manage nut allergy.70.8% were aware of signs and symptoms of nut allergy .63% said their family members or friends have some kind of allergy to various nuts. Most of the school students had the knowledge and were aware about the presence of nut allergies, but more awareness has to be spread as the allergies could turn out to be very serious or even fatal and can only be controlled only through prevention.

In Ayurveda skin disease are described under Kushtha. It is further divided into Mahakushtha and Kshudrakushtha. Rakta Dosha is predominant in Kushtha. Signs and symptoms of Ekakushtha which comes under Kshudrakushtha can be correlate with Psoriasis.  It produces significant adverse effects on psychological and social aspects of life mainly because of visibility. Psoriasis is chronic re-occurring condition that varies in severity from minor localised patches to complete body coverage. Kushtha Chikitsa is classified into Shodhan, Shamana and Nidanparivarjana. Amongst which Shodhan Chikitsa is of greater significance in skin diseases. Raktamokshana is an effective and safe treatment for Kushtha. Acharya Sushruta has mentioned detailed practical guidelines for bloodletting which is considered as Ardhachiktsa. Disease which does not get pacified so quickly by therapeutic measures like Snehana, Swedana etc. are specified by Siravyadha. This is a single case study to evaluate the efficacy of Shodhana in terms of Siravyadha in the management of psoriasis.
