Psoriasis is a chronic inflammatory papulo-squamous disorder of skin characterized by sharply defined erythematous-squamous lesion varying from pinpoint to large plaque, affecting 1-3% of population worldwide. Psoriasis is well known for its course of remission and exacerbation. Here we report a case of 14-year-oldmale 42 kg weight, rotund physique child, diagnosed as Guttate psoriasis, Guttate psoriasis can be closely correlated with Ekkushtha (one of subtype of Kustha) in Ayurveda. Management of patient with, shodhan therapy (Biological purification of body) like mriduvirechan (Mild laxative) and shaman therapy (conservative therapy) showed remarkable improvement in patient condition. There is appreciable psychological health impairment found in patient, which adversely affect patient quality of life. This case study shows that Ayurveda line of treatment (Shodhan and shaman) gives significant result in the treatment of Guttate psoriasis.

Objective. To estimate the changes of the patients’ quality of life in those, suffering ischemic cardiomyopathy after surgical revascularization and transplantation of the stem cells, obtained from umbilical blood. Materials and methods. There was analysis conducted on changes in quality of life in 113 patients, suffering cardiomyopathy of ischemic genesis. In accordance to the therapeutic method applied, the patients were divided into four groups: first - 38 (33.6%) patients, in whom coronary shunting was conducted, the second - 35 (31.0%) patients, in whom coronary arteries stenting was done, the third - 20 (17.7%) patients, in whom surgical revascularization was added by transplantation of the umbilical blood stem cells; and the fourth - 20 (17.7%) patients, in whom conservative therapy only was administered. Results. Estimation of efficacy of the stem cells, obtained from umbilical blood, was conducted, in accordance to the points quantity reduction, using the data from The Minnesota Living with Heart Failure Questionnaire (MLHFQ), by 50.0% in 12 mo of observation. This enhancement of therapeutic efficacy in aspect of the quality of life improvement after complex application of the myocardium revascularization and transplantation of the umbilical blood stem cells have constituted 75.0%, comparing with conservative therapy, 53.7% - comparing with coronary shunting and 48.6% -comparing with the coronary arteries stenting. There was proved the essential raising of probability for achievement of this effect, as well as a probable cumulative restoration of the chances - 4.85 [2.44 - 964]. In accordance to the SF-36 questionnaire answers data; trustworthily higher indices for the physical and psychological health components were obtained in patients, in whom myocardial revascularization were added by the umbilical blood stem cells. Conclusion. After transplantation of stem cells, obtained from umbilical blood, as the additive to the myocardial revascularization for treatment of ischemic cardiomyopathy, the signs of cardiac insufficiency have diminished, tolerance for any physical loading partly renewed, including solemn round of industry and a self-servicing, as well as the indices for the physical and psychological health improved.

Purpose Misophonia is a sound tolerance disorder condition in certain sounds that trigger intense emotional or physiological responses. While some persons may experience misophonia, a few patients suffer from misophonia. However, there is a dearth of literature on audiological assessment and management of persons with misophonia. The purpose of this report is to discuss the assessment of misophonia and highlight the management option that helped a patient with misophonia. Method A case study of a 26-year-old woman with the complaint of decreased tolerance to specific sounds affecting quality of life is reported. Audiological assessment differentiated misophonia from hyperacusis. Management included retraining counseling as well as desensitization and habituation therapy based on the principles described by P. J. Jastreboff and Jastreboff (2014). A misophonia questionnaire was administered at regular intervals to monitor the effectiveness of therapy. Results A detailed case history and audiological evaluations including pure-tone audiogram and Johnson Hyperacusis Index revealed the presence of misophonia. The patient benefitted from intervention, and the scores of the misophonia questionnaire indicated a decrease in the severity of the problem. Conclusions It is important to differentially diagnose misophonia and hyperacusis in persons with sound tolerance disorders. Retraining counseling as well as desensitization and habituation therapy can help patients who suffer from misophonia.

So far, in implementing school strategies, they tend not to utilize Information and Communication Technology (ICT), despite the availability of ICT resources available. Stages of strategic management are needed to generate the vision, mission, objectives, policy, program, budget, and procedures as well as control and evaluation process as an effort to utilize ICT to improve school quality. Based on the interpretation and the results of the study, it is concluded that schools have organized stages in strategic management that enable schools to have a quality profile. The impact of effective utilization of ICTs for schools is the achievement of effective school management, as per the National Education Standards, which is characterized by effective planning, implementation, control, and evaluation of school ICTs.The purpose of this study is to get a general description, describe, and reveal the Strategic Management of Information and Communication Technology Utilization to Improve the Quality of School Learning in Ciledug Al Musaddadiyah Vocational High School and Garut 1 Vocational High School, both on environmental analysis, strategic formulation, implementation and strategic evaluation. The research method used in this research is the case study method, because the problems studied occur in the place and situation of Ciledug Al Musaddadiyah Vocational School and Vocational High School 1 Garut. The use of case study models in this study is based on the consideration that to provide an overview of the strategic management activities of the use of ICTs carried out at vocational high schools with the ultimate goal of being able to improve the quality of school learning. Based on observations in the field of SMK 1 Garut and SMK Al Musaddadiyah Ciledug Garut is one of the public schools and private schools that have these advantages.

In this article, we evaluate the effectiveness of Cone Beam Computed Tomography, through a case study, in assessing the complication of intracranial bleeding during an endovascular treatment of brain arteriovenous malformation when compared to Multislice-Detector Computed Tomography performed immediately after the procedure. The image quality of Cone Beam Computed Tomography has enough diagnostic value in differentiating between haemorrhage, embolic materials and the arteriovenous malformation nidus to facilitate physicians to decide for further management of the patient.

Animal feed knife is a tool that serves to cut and chop animal feed consisting of grass as the main ingredient with additives such as bran, herbs, centrate, cassava, tofu pulp and others. Therefore, as a cutting tool must have the properties of friction resistance, impact resistance, and have good sharpness, so that the material chosen is Ni-Hard 1. The use of centrifugal casting method was chosen because it has the advantage of being able to make castings with relatively thin thickness this is due to the influence of the centrifugal force on the distribution of metal liquids throughout the cavity in the mold. Case study in this study is the use of centrifugal casting methods as an alternative to gravity casting methods to overcome defects of misruns. This research was conducted to investigate the effect of speed on the formation of castings products. The method that was carried out began with a literature study on centrifugal casting, and continued by determining the material, the temperature of the cast is in the range 1250ºC - 1300ºC, and the type of mold. The next step is to do work drawings, pattern making, mold making, casting processes, fettling processes, and analysis. With variations in speed of 200 rpm, 300 rpm and 400 rpm, it can be seen the optimal speed for making this product. The results of this study obtained optimal speed at a speed of 300 rpm to make good quality of animal feed knife products.
