To Explore the Clinical Diagnostic Value of CT Scan in Traumatic Adrenal Hematoma

IgG4-related autoimmune cholangitis (IgG4-AIC) is often difficult to distinguish from cholangiocarcinoma (CCA). This study aimed to determine a practical clinical strategy for distinguishing between IgG4-AIC and CCA to avoid unnecessary surgical resection. We retrospectively collected and compared the clinicopathological data between IgG4-AIC and CCA patients, including the clinical, serological, and radiological characteristics, to follow up on these patients to investigate the prognosis. Among the 377 patients who received surgical resection for suspecting CCA at the Sun Yat-Sen Memorial Hospital between June 2004 and June 2014, 14 patients were diagnosed as IgG4-AIC through histochemistry after surgery. Immunohistochemistry revealed that IgG4 was up-regulated in the plasma cells of IgG4-AIC tissues in 13 out of 14 patients. The serum CA19-9 level was significantly lower than in the CCA group. Patients with IgG4-AIC can only see slight or no enhancement under the contrast enhancement CT scan, while there are no signs of ring-like or delayed enhancement that is unique to CCA. Thirteen patients were followed up, and the time was 12 to 92 months. Three of them were regularly treated with prednisone after surgery, and original symptoms disappeared. Our study demonstrated that the combination of imaging with serum CA19-9 could improve the preoperative diagnostic value and reduce the rate of unnecessary resection.

This paper aims to investigate the clinical diagnostic value of two-dimensional and four-dimensional echocardiography in fetal cardiac tumor. Material and Methods: A total of 24 pregnant women whose fetus had suspected cardiac tumor in The No.4 Hospital 1946 Jinan Shandong from February 2011 to April 2018 were selected as objects of study, including 18 cases whose fetus was pathologically diagnosed with fetal cardiac tumor (+) and 6 cases whose fetus was diagnosed with fetal cardiac tumor (−). Results:These patients were retrospectively analyzed. The tumor site, size, activity and fetal edema predicted using two-dimensional and four-dimensional ultrasound images were compared with pathological examination results, and the diagnostic value of the two kinds of echocardiography in fetal cardiac tumor was analyzed. The echocardiographic results revealed that there were no statistically significant differences in the four-dimensional ultrasonic diagnosis and two-dimensional ultrasonic diagnosis results compared with pathological diagnosis results (p>0.05). In terms of the diagnostic value, the sensitivity, specificity and diagnostic accordance rate in the clinical diagnosis of fetal cardiac tumor were 66.67%, 50.00% and 62.5%, respectively, in two-dimensional echocardiography, and 94.44%, 83.33% and 91.67%, respectively, in four-dimensional echocardiography. Conclusions: Echocardiography can determine the activity, shape, site, size, etc., of primary cardiac tumor. Two-dimensional and four-dimensional ultrasonic diagnosis can be used to observe the fetal cardiac tumor, thus making an early diagnosis. The sensitivity, specificity and diagnostic accordance rate of four-dimensional echocardiography in the clinical diagnosis of fetal cardiac tumor are significantly higher than those of two-dimensional echocardiography, so four-dimensional echocardiography has higher diagnostic value in fetal cardiac tumor.

Abstract The Median Arcuate Ligament (MAL) syndrome is the symptomatic compression of the celiac trunk by the MAL and other ganglionic periaortic tissue. Despite its rarity, this condition is significant from a clinical, diagnostic and management standpoint and it is usually a diagnosis of exclusion. A 61-year-old female with history of intermittent postprandial epigastric pain was diagnosed with MAL syndrome during CT scan imaging (no other causes of pain were identified). Patient successfully underwent robotic MAL release with symptoms improvement after surgery. The robotic approach is feasible and may allow a very precise and delicate dissection with release of the MAL.

The article is devoted to problems of clinical-diagnostic value of determination of cardio-specific troponins in human biological fluids. Improvement of laboratory instrumentation and emergence of high sensitivity methods of analysis have allowed to identify troponins in urine, dialysate, and oral fluid. In the review we present actual information related to measurement of troponins in blood serum, data on testing of cardio-specific troponins in urine, dialysate, and oral fluid. Special attention is paid to determination of some cardiomarkers in oral fluid with thorough analysis of diagnostic value and effectiveness of the conducted studies.
